AFFIDAVIT

I, Neha Sharma, wife of Shri Rohit Agarwal, daughter of Shri Sohan Lal Sharma,

resident of 235/8, Mali Mohalla, Patti Katla Ajmer, Rajasthan 305001 and hereby declare on

oath that :-

1. That, in some documents my name is recorded as Neha Sharma and in some

documents my name is recorded as Neha Agarwal. Both these names belong to the

same woman. But after Marriage my actual name is Neha Agarwal.

2. That from now onwards I should be known and recognized by the name of Neha

Agarwal instead of Neha Sharma.

3. That in all the documents my name should be mentioned as Neha Agarwal instead of

Neha Sharma.

4. This is that I want to get my name registered as Neha Agarwal instead of Neha

Sharma in my Aadhar Card, Jan Aadhar Card, Marriage Certificate and all

government and semi-government documents. In support of which I am Submitting

this affidavit for gazette notification.
