AFFIDAVIT

I AMISHA PRAJAPAT D/o DEENDAYAL Behind Hariram Temple, Ward No. 20,

Gangashah I hereby state on oath the following facts that :-

1- That the correct spelling of my surname is PRAJAPAT.

2- That in my class 12th marksheet my Surname is mentioned as PARAJAPAT, which is

incorrect.

3- That in future in all my documents, wherever my surname is mentioned as

PARAJAPAT there should be my correct surname PRAJAPAT written and

understood i.e. my correct name is AMISHA PRAJAPAT D/O DEENDAYAL.

4- That I am submitting the above affidavit in support of change of surname spelling for

publication in the Gazette.

The facts mentioned in items 1 to 4 of the above affidavit are true and correct and no

facts have been omitted, enhanced or concealed.
