This is for general information that I, Mamta Petar W/o Shishpal Pareek, resident of

Village-Barjangsar, VPO-Barjangsar, Distt-Churu, Rajasthan-331411, India declare that my

name in Aadhar No. 6107 0346 0507 issued by UIDAI is MAMTA PETAR age 33 years.

Now I have changed my name and shall hereafter be known as MAMTA PAREEK ¼eerk
