I, Ilamdeen S/o Ismail R/o Ward no. 02, Chak 1 ARW, Dudhwali dhani, 32 NDR,

Chohilawali, Hanumangarh, Rajasthan-335803, do hereby affirm and declare as under:-

1- That My Name is Mentioned as Ilamdeen in my Adhaar card, Jan Adhaar and other

official documents and my son 8th certificate/marksheet my name mention as

ILMUDEEN.

2- That I want to change my name Ilmudeen to Ilamdeen in my son's 8th,

certificate/marksheet.

3- That I am excecuting this declaration to be submitted to the concerned authorities for

the change of name.

4- That I will be known as Ilamdeen in the Future.

5- That I have not published in the Gazette for change of name yet.
