Khurd, Tehsil-Buhana, District- Jhunjhunu-333514, Rajasthan do hereby solemnly affirm and

declare as under:-

1. That my name is Garima Choudhary which is correctly written as Garima Choudhary

in all my government and official documents, such as Aadhaar Card, PAN Card,

Driving Licence etc.

2. That my name is also written as Garima Chaudhary in all my academic

records/documents, including 10th & 12th mark sheets/certificates, BHMS

marksheet/degree certificate etc.

3. That the spelling variation between "Garima Choudhary" and "Garima Chaudhary"

pertains only to the transliteration/spelling difference, and both names belong to me,

one and the same person.

4. That for purpose of evidencing hereinafter in all records, deeds and writings and in all

proceedings, dealings and transactions be it private or government, my name i.e.,
