AFFIDAVIT

I, RAJA MEENA, S/o Mr. RAJU MEENA, aged about 19 years, resident of

BADIDORI, BARA VARDA, Pratapgarh, Rajasthan-312616, do hereby solemnly affirm and

declare as under:-

1. That my name is "RAJA MEENA" as recorded in my existing documents.

2. That I have changed my name from "RAJA MEENA" to "SUNITA MEENA" of my

own free will and without any pressure or influence.

3. That both names "RAJA MEENA" and "SUNITA MEENA" refer to one and the same

person, i.e., myself.

4. That this affidavit is executed for submission for publication in the Rajasthan

Government Gazette to officially notify my name change.
