AFFIDAVIT

By this deed I, the undersigned FATEMA KANOR WALA (new name) of previously

called FATIMA PARTAPURWALA (old name), doing HOMEMAKER (give profession or

avocation) and 192, NAJMI VILLA, ADINATH NAGAR, FATEHPURA, UDAIPUR, RAJ.

313001 (Address) solemnly declare: -

1- That for and on behalf of my self and my husband and children and remitter issue

wholly resource/relinquish and abandon the use of my former name/surname of

FATIMA PARTAPURWALA (and in place thereof i do hereby assume from this date

the name/surname FATEMA KANOR WALA and so that I and my husband and

children remitter issue may hereafter be called, known and distinguished not my

former name/surname, but assumed name/sumame of FATEMA KANOR WALA.

2- That for the purpose of evidencing such my determination declare that I shall at all

times hereafter in all records deeds and writings and in all proceedings, dealings and

transactions, private as we ll as upon all occasions whatsoever use and sign the name

is FATEMA KANOR WALA as my name/surname in place and in substitution of my

former name/surname.

3- That I expressly authorize and request all persons in general relatives and friends in

particular at all times hereafter to designate and address me, my husband, my

children, remitter issue by such assumed name/sumame of FATEMA KANOR

WALA accordingly.

4- In witness whereof I have here unto subscribed my former and adopted name/sumame

of FATEMA KANOR WALA and affix my signature and seal, if this 12 day of
