Affidavit

I, Prakash Chand Bhatiya S/o Shri Roop (Chandra Bhatia) age 87 years, R/O 54,

Rangpur Road, Dadwara, Rangpur, Kota Jn. - 324002 (Raj-), do hereby solemnly affirm and

declare as under:-

1- That I am citizen of India.

2- That in various documents my name is mentioned as PC BHATIA (old name).

3- That Prakash Chand Bhatiya and PC BHATIA both are my names and same person.

4- That my correct name is Prakash Chand Bhatiya and I, therefore, request to the

concerned authorities to may please update and rectify name as Prakash Chand

Bhatiya.

5- That I am enclosing a self attested copy of my Aadhar card and PAN card as a proof

of my current name Prakash Chand Bhatiya.
