(Raj.), do hereby solemnly affirm and declare as under:-

1- That I am citizen of India.

2- That in various documents my name is mentioned as Durga Devi Gupta (old name)

3- That Durga Devi and Durga Devi Gupta both are my names and same person.

4- That my correct name is Durga Devi and I, therefore, request to the concerned

authorities to may please update and rectify name as Durga Devi

5- That I am enclosing a self attested copy of my Aadhar card and PAN card as a proof

of my current name Durga Devi
