AFFIDAVIT

I, Jyoti, daughter of Shri Ramniwas, resident of Village Hanuman Ji Ki Dhani,

Bugala, District Jhunjhunu, Rajasthan, do hereby solemnly affirm and declare on oath as

under:

1- That my present name is Jyoti.

2- That henceforth, I shall be known as JYOTI CHOUDHARY for all purposes.

3- That I have changed my name from Jyoti to Jyoti Choudhary to ensure uniformity and

correction of my name in all official and other documents.

4- That this affidavit is made to declare my above name change and shall be used for all

legal, official, and personal purposes.
