AFFIDAVIT

I, Aachu Devi wife of Dulla Ram, Aged 51 year Resident of Lega Ki Dhani, Toshina

Tehsil Chhoti Khatu, District Didwana-Kuchaman State Rajasthan&341319 India do hereby

solemnly affirm and declare as under:

1- That my above mentioned Name Aachu Devi is recorded in my Voter card No- AZB1858612]

Ration Card No- 200002390994 and Jan-aadhaar card No.4853306282.

2- That my name is wrongly entered as Panchi Devi in my Aadhaar card No- 847846822749.

3- That both Aachu Devi and Panchi Devi is one and same person that is myself.

4- I executed this affidavit for updation of my name in my aadhaar card accordingly my

Voter/Ration/Janaadhaar card.

5- That I shall at all times hereinafter in all records, deeds and writing and in all proceedings

dealing with transactions, private as well as under all occasions whatsoever be known only as

AACHU DEVI.
