Whereas, the Executive Engineer-I, JICA ODA

Loan Project Office, Margao-Goa vide letter dated

27-03-2017 has informed with regard to the

permission issued by this Office vide Order dated

22-11-2016 that work could not be carried out due

to some technical problems and code of conduct

due to Legislative Assembly 2017. It is also informed

that as directed by Project Director/Principal Chief

Engineer, the work requires 2-3 months, work cannot

be taken up at this stage as reinstatement job

cannot be done during monsoon. Hence it is

proposed to commence the work from 15th October,

2017 till 31-12-2017 after monsoon period and

therefore it is requested to extend the validity of

the permission already granted vide Order dated

22-11-2016 from 15th October, 2017 till 31-12-2017;

And whereas, the Superintendent of Police,

South Goa, Margao vide letter dated 26-05-2017 has

submitted report giving no objection for the above

diversion;

And whereas, it is revealed that the work

mentioned above is of urgent nature and it requires

to divert the traffic for smoother movement of traffic;

And whereas, I am satisfied that the diversion of

traffic is essential during execution of the above

work;

Therefore, I, Swapnil M. Naik, IAS, District

Magistrate, South Goa, Margao do hereby permit

upon urgency of the work for extension of diversion

of traffic while laying 700 mm diameter pipeline for

JICA project from Arlem to Borim Junction from

15th October, 2017 till 31-12-2017.

And also Order under Sections 115 and 116 of

the Motor Vehicles Act, 1988 (Central Act 59

of 1988) read with Government Notification

No. 5/28/88/TPT (part) dated 26th September, 1989

being satisfied that it is necessary in the interest

of public safety and convenience to temporarily

divert the traffic as per the annexed plan and erect

the signages accordingly subject to compliance

with the conditions mentioned below:-

CONDITIONS:

1. The applicant/contractor is directed to ensure

that no annoyance is caused to the general

public.

2. Should adhere to all the rules and regulation

in force.

3. The applicant/contractor is directed to obtain

prior permission from other concerned

competent authorities.

4. The applicant/contractor is directed to

execute the work in phase manner of 30 mtrs.

After finishing of one phase, the work of next

phase may be undertaken.

5. The applicant/contractor is directed to display

proper signages, indicating flash light for

diversion/closure/work in progress with flash

lights for awareness of the motorists,

pedestrians and other road users particularly

during night.

6. The applicant/contractor shall deploy

sufficient number of traffic marshall/volunteers

with retro reflective jackets to regulate the

traffic at the site for smooth flow of traffic.

7. The area around the road/digging site shall

be properly barricaded and illuminated with

red light, glowing/reflective tapes to prevent

accidental falling of pedestrians, motorists, etc.

8. The applicant/contractor is directed to ensure

that there are no loose gravels of soil/stones,

leftovers on the road/shoulder of the road after

the completion of the work.

9. The applicant/contractor is directed to

publish an advertisement well in advance in

the local newspaper about the date of closure

of road for commencing the said work to create

awareness among the general public.

10. If any untoward incident arises due to poor

barricading/non-applying of glowing/

/reflective tapes then the concerned

contractor/PWD authorities shall be held

responsible for the same.

11. The applicant/contractor is directed to ensure

that the original condition of the roads of each

phase including tarring should be restored

after the completion of the work.

12. The applicant/contractor is directed to display

road diversion signages throughout the

alternate route to avoid inconvenience to the

motorists and general public.

13. The equipments/material if any kept on the

proposed road shall be covered with

illuminating warning sign during night time,

so as to avoid from any untoward incident.

14. The work should not be carried out during

school hours and peak traffic hours and extra

manpower should be engaged to complete the

work at the earliest.

15. The applicant/contractor is directed to hire

sufficient machinery and man power and to

complete the desired work within the

stipulated time frame.

16. The Senior Officer, PWD, National Highway,

Government of Goa not below the rank of

Executive Engineer should supervise the work.

17. The Senior Representative of the consultants

engaged in execution of the above work

should supervise the work at the time of

execution.

18. Necessary permissions and clearance should

be obtained from the owners of land, Public

Works Departments, National Highways of Goa

State Government before commencement of

the above work.

19. Absolute care should be taken that no damage

of any kind is caused during the course of

excavation. The executing agency and

supervisory agency should take precautions

in this regard.

20. The applicant/contractor is directed to

complete the entire works including

resurfacing of roads within stipulated time

frame by engaging additional manpower.

21. A detailed report should be submitted to the

District Magistrate, South Goa, regarding

completion of the work by 31-12-2017.

22. If the applicant/contractor fails to abide by

the conditions laid above or if the NOC issuing

authority feels so, the NOC is subject to

cancellation/revocation at any given time.

54. By this deed I, the undersigned, Mrs. Vandana

Sanjiv Prabhudesai now lately called Vandana

Shripad Prabhu Dessai, employed as Assistant

Teacher at Government High School, Paddi, Barcem,

Quepem, Goa do hereby-

Wholly renounce, relinquish and abandon the

use of my former name of Vandana Shripad

Prabhu Dessai and in place thereof do assume

from the date thereof the name of use the

name of Vandana Sanjiv Prabhudesai, and so

that I may hereafter be called, known and

distinguished not by my former name of

Vandana Shripad Prabhu Dessai but by my

assumed name of Vandana Sanjiv Prabhudesai.

For the purpose of evidencing such my

determination I declare that I shall at all times

hereafter in all records, deeds, writings and in

all proceedings, dealings and transactions,

private as well as public and upon all

occasions whatsoever use and sign the name

of Vandana Sanjiv Prabhudesai as my name in

place and in substitution for my former name

of Vandana Shripad Prabhu Dessai.

Expressly authorize and request all persons

at all times hereafter to designate and address

me by such assumed name of public and upon

all occasions whatsoever use and sign the

name of Vandana Sanjiv Prabhudesai

accordingly.
