Section 10(1) (a) of the Goa Registration of Tourist
Trade Act, 1982, I Shri Rajesh Kale, Prescribed
Authority, hereby remove the name of “Kamakshi
Guest House”, c/o Mr. Uttam N. Kolwalkar, H. No.
303/1, Dabholwada, Chapora, Anjuna, Bardez, North
Goa from the record of Hotel Keepers maintained
under the aforesaid Act, as Mr. Uttam N. Kowalkar
has ceased to operate the said Guest House in her
premises at Chapora, Bardez, North-Goa.
Consequently, the certificate of registration No.
HOTNOO1186-D issued under the said Act stands
cancelled.

“Sonal Pandey”, which is registered in the
Office of Registrar of Births and Deaths,
Sasaram Bihar.
3. I say that as per the Educational Qualification
Certificates of my daughter “Miss Mansi
Pandey” i.e. 10th Certificate, 12th Certificate,
and Graduation 1st and 2nd years Certificate,
my name is recorded as “Sonali Sumi Pandey”.
4. I further say that in the name of my daughter
“Miss Mansi Pandey” Passport bearing No.
GO1078068502714, issued to her by
Government of India, my name reads as “Sonali
Sumi”.
5. I say that “Sonal Sumi” “Sonal Pandey”, “Sonali
Sumi Pandey and “Sonali Sumi” is one and the
same person and that is myself.
6. I say that I have sworn this Affidavit in order
to prove the fact that “Sonal Sumi” “Sonal
Pandey”, “Sonali Sumi Pandey and “Sonali
Sumi” is one and the same person and that is
myself wherever necessary.
