Rule 264A of the Motor Vehicles Rules, 1991 as
amended in 2005 and as recommended by the Asstt.

Dy. Superintendent of Police (Traffic), North, Altinho-
Goa, I, Sanjeev C. G. Dessai, District Magistrate,
North Goa hereby notify “Speed Breakers” on the
Village Road at the place mentioned in Column No.
2 of the schedule below in the jurisdiction of Village
Panchayat Guirim in Bardez Taluka.

said Mrs. Sharona Manuel Rebelo and her husband
Mr. Fraizer D’Costa have relinquished all their
Undivided and Illiquid Right that they have or are
entitled to the estate and inheritance left upon the
death there father/father-in-law late Manuel Rebelo,
in favour of other co-heirs, in terms of section 30 of
the Goa Succession Special Notaries and Inventory
Proceeding Act, 2012 in force in this State of Goa.
That in view of Relinquishment of Rights given by
the said daughter Mrs. Sharona Manuel Rebelo and
her husband Mr. Fraizer D’ Costa, the remaining
persons namely (one) Slicy Rebelo married to Benny
Santana Fernandes and (two) Jeo Rebello, bachelor
are the sole and universal heirs and successors of
the said deceased. There being no one else or no
other person or heirs who in terms of Law of
Succession still in force in this State of Goa may

(1) Every contesting bidder other than applicant
should deposit an EMD in the Office of Administrator
of Comunidades of North Zone, Mapusa, Bardez-
Goa, in cash an amount equivalent to five times of
the initial minimum amount lease bid as fixed
above. The said EMD amount should deposited in
this office on the previous day of auction between
10.00 a.m. to 12.00 noon and only those persons
who have deposited such amounts a list thereof

403110, do hereby solemnly affirm on oath as under:
1. I say that, I am residing at the above mentioned
address along with my family.
2. I say that, on my birth certificate my name is
recorded as Lolita Ramanata Veluscar.
3. I say that, on my marriage certificate my name
is recorded as “Lalita Ramanata Veluscar”.
4. I say that, on my cancelled Indian passport
bearing No. R9887050 my name is recorded as
“Snehali Narayan Naik”.
5. I say that the three names “Lolita Ramanata
Veluscar, Lalita Ramanata Veluscar and Snehali
Narayan Naik”, are of one and the same person
i.e. myself.
6. I say that, on my birth certificate my mother’s
name is recorded as Menacxi Ramanata
Veluscar.
7. I say that, on my cancelled Indian passport
bearing No. R9887050 my mother’s name is
recorded as “Minaxi Ramanata Veluscar”.
8. I say that the two names “Menacxi Ramanata
Veluscar and Minaxi Ramanata Veluscar”, are
of one and the same person i.e. my mother.
9. I say that, this affidavit is sworn by me to
submit to the concerned authorities, for OCI
purpose, in order to prove discrepancies of all
the above stated names of myself, my mother
and I justify that all the mentioned names are
of one and same person.

47. I, the undersigned Mr. Eknath Maddu Sawant,
son of late Shri Maddu Sawant, age 52 years,
married, Indian National, residing at H. No. 7/1,
Chodanwado, Ecoxim, Pomburpa, Bardez-Goa, do
hereby state on solemn affirmation as under:-
1. I say that in my Birth certificate, Pan card and
Aadhar card my name reflected as Eknath
Madhu Sawant.
2. I say that on my School Leaving Certificate my
name is reflected as Eknath Maddu Sawant.
3. I say that Eknath Madhu Sawant & Eknath
Maddu Sawant is one and same persons name
i.e. myself.
4. I say that this Affidavit is sworn by me in order
to publish in Gazette Official, Panaji-Goa. That
both the names are one and the same persons
and same to produce the same is required to
produce before IIFL Finance Limited, having
corporate office at Vijay Nagar, Andheri East,
Mumbai 400069 to clarify ambiguity in respect
of my above mentioned two names that
pertains to me only.
5. I say that the contents of above paras 1 to 4 are
true to my personal knowledge and belief and
I have not concealed or misrepresented any I
facts therein.
