Read: Letter No. DYSP/TRF/NORTH/232/2023 dated
04-02-2023 of the Dy. Superintendent of Police
Traffic, North, Altinho, Panaji-Goa.
In exercise of the powers conferred on me under
Section 112 of the Motor Vehicles Act, 1988 and
Rule 264A of the Motor Vehicles Rules, 1991 as
amended in 2005 and as recommended by the Dy.
Supdt., of Police (Traffic) North, Altinho Goa,
I, Mamu Hage, IAS, District Magistrate, North Goa
hereby notify “Speed Breakers” on the village road
at the place mentioned in Column No. 2 of the
schedule below in the jurisdiction of Village
Panchayat Canca Parra in Bardez Taluka.

The above Speed Breakers shall be painted with
white thermoplastic fluorescent paint and fixed
with blinkers for better visibility.
Further, in exercise of the powers conferred on
me under Section 116 of the above Act, I also
authorize the erection of cautionary signboards
“Speed Breaker Ahead” at about 40 mts. on the left
side of the road at the placement of the speed
breaker in order to regulate the motor vehicular
traffic.
The Secretary Village Panchayat Canca Parra,
Bardez is directed to publish this notification
through the Director of Information and Publicity
in newspapers and P.W.D., to make necessary
arrangements to erect the signboards at the places
indicated above as per the specification laid down
by the Ministry of Road Transport and Highways
and as published in the Official Gazette by the
Government of Goa and report compliance within
thirty days time.

Goa 403005, Goa do hereby state on oath as under:
1. I say that my maiden’s name was Katpadi Baby
Kini alias K. Baby Kini and that I was working
in State Bank of India, Yallapur even before my
marriage.
2. I say that “Katpadi” is the name of the place in
Karnataka, where I use to live and as a tradition
in Karnataka the title “Katpadi” was associated
with my maiden’s name.
3. I say that my name on my Xth standard School
Certificate is mentioned as Baby Kini K and in
my employee ID card issued by State Bank of
India, Yallapur is mentioned as K. Baby Kini.
4. I say that I was married to Mr. Ramakrishna
Vasudev Bhat alias R.V Bhat alias Ramakrishna
V. Bhat on 17-07-1988 and the said marriage was
registered on 15-04-1997 in Karnataka and as
per the marriage certificate my name is
mentioned as K. Baby Kini.
5. I say that after my marriage with Mr.
Ramakrishna Vasudev Bhat alias R.V Bhat alias
Ramakrishna V. Bhat and as per Hindu tradition
I changed my name from Katpadi Baby Kini alias
K. Baby Kini to Radhika Ramakrishna Bhat.
6. I say that subsequently after the name change
all my identity proof documents were changed
from Katpadi Baby Kini alias K. Baby Kini to
Radhika Ramakrishna Bhat and at present all
the documents I hold are in my new name i.e.,
Radhika Ramakrishna Bhat.
7. I say that I along with my husband have
purchased 630 shares of State Bank of India by
vide Share certificate No. 000074064 and Share
certificate No. 000074065.
8. I say that the State Bank of India reissued me
the Share Certificates on 22nd November, 2014
in which my name appears as Katpadi Baby Kini
whereas all other old documents refer to me as
K. Baby Kini.
9. I say that the names Katpadi Baby Kini alias K.
Baby Kini and Radhika Ramakrishna Bhat all
these names belong to the same person and
that is me.
10. I say that the contents of para 1 to 9 of my
affidavit are true to the best of my knowledge.
11. I am executing this declaration to be submitted
to be published on the Official Gazette of Goa
informing the concerned authorities of my name
change and request the concerned authorities
to make necessary corrections on the Share
certificates issued to me by the State Bank of
India.
