In exercise of the powers conferred on me under

Dy. Supdt. of Police, Traffic (North), Panaji, I, Mamu

The Executive Engineer, Works Division XVIII
(Road), P.W.D., Ponda-Goa, shall take necessary action
for erection of Hump Type Speed Breaker and
cautionary signboards. The works be strictly as per
the specifications along with installation of
necessary cautionary signboards and road markings
of thermoplastic paint, fixing of cat eyes, as per IRC
specifications laid down by the Ministry of Road
Transport and Highways and as published in the
Official Gazette, Series I No. 15 dated 15th July,
2005 by the Government of Goa. The signboards
shall be installed as per specifications laid in the
Seventh Scheduled under the Goa Motor Vehicles
(Amendment) Rules, 2005 & shall be located 40
meters in advance of the hump type speed breaker
and at the placement of the speed breaker.
Action taken report including photographs of
the work executed must be submitted to this office
within 30 days.
Non compliance of order shall attract penal
provisions under Section 188 of I.P.C., 1860.

1. Wholly renounce/relinquish and abandon the
use of my former name of Josna Volaba Camotim
and in place thereof do assume from the date
thereof the name of Jyotsna Vallabh Kamat and
so that I may hereafter be called, known and
distinguished not by my former name but by my
assumed name of Jyotsna Vallabh Kamat.
2. For the purpose of evidencing such
determination, declare that I shall at all times
hereafter in all records, deeds, writings and in
