in the village Colva of Salcete Taluka, South Goa
District, Goa.

The Chief Officer, Bicholim Municipal Council, is
directed to publish this notification through the
Director of Information and Publicity in local
newspapers and P.W.D. (Engineering Cell) to make
necessary arrangements to construct the Speed
Breakers as also Rumbles Speed Breakers and to
erect the signboards at the places indicated above
as per the specification laid down by the Ministry
of Road Transport and Highways and as published
in the Official Gazette Series I No. 15 dated
15-07-2005 by the Government of Goa and report
compliance within thirty days time.

7. I say that my mother has been married to Shri
Mukund Tivrekar hence Hospital at GMC
recorded her name as “Kesar Mukund Tivrekar”
as per the Adhar card.
8. I say that the Health card issued by my company
been my mothers name as “Kesar Karmalkar”.
9. I say that my mother name “Kesar Mukund
Karmalkar” and “Kesar Mukund Tivrekar” is one
and the same person.
10. I say that the said the Affidavit is done to submit
the same to the Insurance company as regards
to correction in my mothers name from “Kesar
Mukund Karmalkar” to “Kesar Mukund
Tivrekar”.
11. I say that I have sworn this Affidavit to submit
the same the Government Printing Press in order
to publish the same in the Official Gazette of
Government of Goa.
12. I say that content of the above Affidavit from
para’s 1 to 6 are true to my own knowledge and
belief.
