That deceased are survived by two children namely,
one son and one daughter. The said 1) Cirilo Dias,

to Eulalia Merciana Dias, 59 years of age, housewife,
and 2) Milagrina Dias, divorcee, 59 years of age, all
resident of House No. 94, Batim, Maina, P. O. Goa

perfect knowledge of all these facts and hence
declare for all legal purpose that the said, 1) Cirilo
Dias marired to Eulalia Merciana Dias and
2) Milagrina Dias are the only heirs and besides
them there are no other persons who have any
title or claim to the estate of the deceased person
and besides that there does not exist any other
person or persons who according to the prevailing
law in force in the State of Goa may have a legal
right of succession or would prefer in the said
succession or would concur to the inheritance left
behind by the said deceased person.

produce the same before Concerned Authority in

are of one and the same person in order to do the
correction of my father’s name on my PAN card.
