Deed Changing of Name/Surname
47. By this deed I, the undersigned Aditi Abhay
Gaonkar (new name) lately called Sampurna Arjun
Govekar (former name) employed as Teacher at
Government Primary School at Muslimwada Bhom,
Ponda-Goa, do hereby:-
1. Wholly renounce, relinquish and abandon the
use of my former name of Sampurna Arjun
Govekar and in place thereof do assume from
the date thereof the name of Aditi Abhay
Gaonkar and so that I may hereafter be called,
known and distinguished not by my former name
of Sampurna Arjun Govekar but by my assumed
name of Aditi Abhay Gaonkar.
2. For the purpose of evidencing such my
determination declare that I shall at all times
hereafter in all records, deeds and writings and
in all proceedings, dealings and transactions
private as well as public and upon all occasions
whatsoever use and sign the name of Aditi
Abhay Gaonkar as my name in place of and in
substitution for my former name of Sampurna
Arjun Govekar.
3. Expressly authorize and request all persons at
all times hereafter to designate and address me
by such assumed name of Aditi Abhay Gaonkar
accordingly.
In witness whereof I have hereunto subscribed
my former and adopted names of Sampurna Arjun
Govekar and Aditi Abhay Gaonkar and affixed my
seal this 16th day of May, 2024.

-Goa 403516, do hereby solemnly state on oath as

1. I say that I am holder of Aadhar card bearing No.
xxxx xxxx xxxx and as per my Aadhar card my
name is recorded as “Nashon Morris Fernandes”
and the same name is reflected on my election
card bearing No. XOR0339689 and my PAN card
bearing No. ACIPF3393B respectively.
2. I say that as per my passing certificate-cum-
-statement of marks issued by Goa Board
Secondary and Higher Secondary Education,
Alto Betim-Goa my name is reflected as
“Fernandes Nashon”.
3. I further say that both the names “Nashon
Morris Fernandes” alias “Fernandes Nashon” are
of one and the same person i.e. myself.
4. I say that this Affidavit is sworn by me to produce
it before the concern authority with respect to
my different names.
5. I say that the contents of this Affidavit are true
to my personal knowledge.
