TERMS & CONCITIONS

Tender should be in two parts (technical/ commercial) in two separate sealed envelop mention

there on technical tender / commercial tender.

Both tender (technical/ commercial) should be in prescribed Performa enclosed with terms and

conditions and also mention break up rate of all attachment/ accessories of the machine/

equipment.

All relevant document, authority letter, of authorized importer of India or Eastern

India/Manufacture quality certificate, sales tax Clearance certificate, specification of the machine/

equipment, installation list of machine/ equipment should be attach along with Technical Tender,

photo copy of pan card. All the above documents should be attached in the technical bid if any

document was not attached with technical bid than the tender will be rejected.

Minimum warranty (with part) should be three years of above one lakh Machine and it will count

from date of installation and working certificate.

Before payment of cost of machine/ equipment supplier will have submit 5% bank guarantee for

warranty period of 3 years and agreement of CMC for rest 2/7 years on Non judicial stamp paper.

Only power plug, space for the installation will be provided by the Institution. Rest expenditure

will be borne in by the supplier.

Equipment should be ISO/FDA/CE other national or international Quality certification approved

copy of certificate should be enclosed with technical bid.

The Company/Tenderer should provide the list of organization to whom they have supplied the

above machine and performance report from the said organization.

Life of machine/ equipment – Machine having cost from one Lakh to Five Lakh is fixed for 5

years and machine carrying the value more than five Lakh the life should not be fixed less than

Ten years.

14.

The models quoted should be as per specification with facilities of up graduation as per

requirements and It is mandatory that the name of the model should be mention in the tender.

The company has to give a certificate/ affidavit for the above purpose at the time of technical bid.

Manufacturing Companies having turnover Rs. 10 crore and above can apply for machine,

costing more than Rs. 5 lakh however companies quoting for machines less than 5 lakh should

have annual minimum turnover of Rs. 2 crores in the three consecutive years.

15.

For calculations of rate cost of machine/ equipments(full system, warranty and CMC for 2/7

years of machine /equipment (full system)

Tenderer can attend purchase committee meeting on tender Opening date.

17.

Tenderer should assure a service centre at Patna for the proper maintenance of said machine.

18.

Tenderer should be registered in Bihar sales Tax. If they are not registered at the time of tender.

Then after selection, they have to be registered with Bihar sale tax before the order is placed.

The tendered have to give technical details/specification of the product/ equipment in CD along

with technical bid (Specification of the equipment / Product should be typed only) .it should not

be scanned Name of the Tendered should be mention on the above of the CD along with the

tender number and address.

Tender for Drugs/ Various Surgical items

Patna Medical College & Hospitals is a 1675 bedded hospital with an occupancy rate of more

than 95% having all specialties with 8 intensive Care Unit Catering to the need of people of Bihar,

neighboring states and countries. To ensure good quality healthcare PMCH is aiming towards rational

drug management and procurement to ensure good quality drugs and services as per the guidelines of

government.

Bids are invited from interested companies /firms/manufacturers /direct importers/ authorized C/F agents

/sole distributors to supply essential and generic drugs /various surgical items for the year 2011-12.

College & Hospital during office hours on demand draft of Rs. 100/ or on websitewww.prdbihar.org and

www.pmch.in

1.Minimum Eligibility Criteria : Bidders should meet the following criteria to be eligible for bidding:

Manufacturer/Bidders having minimum average annual turnover of Rs. 25 crores or above in the last

3 consecutive years for medicines for surgical items, the average annual turnover will be5.0 crores in last

three consecutive years, product list, market standing of minimum 3 years. They should possesses WHO-

GMP (Good Manufacturing Practice)/ ISO certification, along with PAN card and Sales Tax Clearance

Certificate, Tenders should be registered with Bihar Sales Tax. If they are not registered at the time of

tender, then after selection they have to be registered with Bihar sale Tax before the order will be placed.

Bidders will be required to deposit Rs. 1,00,000/- (One Lacs) security /tender money for medicine &

Rs. 1,00,000/- (One Lacs) for surgical items in the form of demand draft / NSC favouring

superintendent, PMCH, Patna. They should have a demonstrated ability of handling large scale

manufacturing and experience of supplying required items in medicine only.

Prefence will be given to those party which have its own manufacturing unit.

2.Submission Requirements:

Interested company/ firm, wishing to apply for the bid, may submit in a large envelop marked

“Tender bid for Drug & Various Surgical items on separate sealed envelops tender for each group (either

drug or various surgical item) If the both tender for drugs and surgical items in the same envelop the

tender bid automatically rejected. Cover ‘A’ for technical bid and cover ‘B’ for financial bid. In no

condition the hand written tender will be accepted, if the both financial and technical bids are received in

the same envelop, the tender bid shall be automatically rejected. The company / firm shall respond with

each of the following:

License of the product quoted with latest license renewal certificate, Minimum three years market

standing, minimum turnover Rs. 25 Crores or above for the last three consecutive years for medicine and

for surgical items, the average annual turnover will be 5 crores in the last three consecutive years/ product

list. Non conviction certificate for the last 3 years of manufacturer and up-to-date non convictional

certificate of tenderer. Tenderer should enclose their physical verification report obtained by competent

authority. WHO-GMP /GMP certificate/ISO certificate/ Registration under the Drug Act, Copy of PAN

card/ an affidavit of name and full address or tenderer with his telephone numbers/ Sales tax clearance

certificate/ Income tax assessment report for the last 3 years. Rs. 1,00,000/- (One Lacs) for medicine and

Rs. 1,00,000/- (One Lacs) for Surgical items favouring “Superintendent, PMCH, Patna”. An Affidavit

attached the firm is not Black Listed from any Govt. Institutions.

Please Note: Any bid not accompanied by earnest money or any of the above documents will be

automatically rejected.

Cover ‘B’

Printed sheet with a landed price/ basic rate excluding Local Sales Tax, then rates for sale taxes

applicable and then the final rate. Also mentioned the MRP in the financial bid. Additionally a column

should be printed besides each drugs item/ surgical items mentioning their pack size, manufacturer and

column mentioning their presence in the tender & product list. An affidavit has to be given in this envelop

that the rate quoted by the agency in the financial bid is at list 20% less than the MRP. DPCO form V

price list also enclosed in the cover B. in case the quoted price in the financial bid is not more than 20%

less than MRP, it will not be accepted.

(2) In which drug/ surgical items form V is not required should attached a affidavit with

supporting documents in this regard with product list. Rate of this item must be less more than 20% of

MRP. Enclosed a MRP list of manufacturer.

(3) Attached the Tender Hospital / Institution supply rate.

(4) Tenderer/ Manufacturer Unit attached an Affidavit which rate is quoted in the tender is more

than quoted by me in any Govt. institution.

(5) Final rate must be quoted in the word also.

(6) In case of discrepancy in “in figure” price and “in words” price then “in words” would be

prevail.

(7) In case of Non Submission of any of the above, financial bid will not be considered.

You are requested to clearly indicate by index each of the submission requirements mentioned

in cover A and cover B along with relevant page number in your cover letter/ application

accompanying the technical proposal.

3. Tender Conditions:

i. The tab/Cap./bottle should have mark of “Hospital supply, not for sale”.

ii. Samples of Supplies in each batch will chosen for testing by Government of Bihar notified

laboratories.

iii. Liquidated damages will be levied for delay supply (0.5% per day subject to a maximum of 15%)

iv. Minimum expiry period of the products should not be less than 18 months from the date on

which the store receives the products. If the self life of the products is less than 18 months only,

the above condition shall be reduced to 1 year.

v. If the product fails in quality test, the entire batch will be taken back by the supplier at the

supplier’s cost and the hospital shall not be responsible for any damage during this period.

vi. Leaked, soiled, broken containers with damage labels shall not be accounted for the purpose of

supply.

vii. The inner label as well as label on the catch cover/ carton shall bear conspicuously the words,

“HOSPITAL SUPPLY NOT FOR SALE” and bolder than those already printed on the label. The

above caption shall run from the lower left corner to the top right hand corner.

viii. Bidder should quote one rate for one product.

ix. The rate contract will be applicable for the financial year only.

x. All affidavit submitted by the first class Magistrate in original copy.

xi . If the awarded supplier is unable to meet the fully supply at the L¹rate in given time frame, the

award can be given to L²bidders by decision of purchase committee.

4. Deadline for Submission of Proposal:

The technical and financial bid in separate sealed envelops must reach the office of the

undersigned within 21 (Tewenty one) days of the publication of the tender by Registered post/ Speed

Post. In no circumstances the tender will be accepted by hand.

5. Opening of Bids:

Financial bid of those parties will be opened who qualify in technical bid. The decision of the

Purchase Committee of Patna Medical College & Hospital, Patna will be final. The committee reserves

the right to cancel the bid at any time without assigning any reason. For further enquiries contact

Superintendent Office during office hours on working days.

After being awarded the contract, the awardees will be required to be registered with the Bihar

Sales Tax if not registered. The selected Company/ Firm will be immediately informed about the final

award. In case the selected Company Firm declines to take up the task at the quoted and approved rates, it

would be barred from applying for any tender from PMCH, Patna and will be recommended for black

listing by Govt. of Bihar. Also the tender/Security money will be forfeited. Any matter according to

dispute leading to court case will be within preview of Patna district only.
