3. Enclosure of Department's notification No. 17408 dated 30-12-2016 will remain as
Documents to be submitted for the specified Service (appendixe-II), General Instruction, Pro-forma
for new Ration Card under Bihar Right to Publice service act (Pro-forma-K) and Pro-forma for
modification in Ration Card or for surrendering/ Cancelation of Ration Card under Bihar Right to
Publice service act (Pro-forma-kh) earlier.
4. Department of Food and Consumer Protection, Bihar, Patna may issue other required
formats, from time to time, after consulatation with the Law Department.
