No 18/Lo.Se.A.-12-01/2016(Part)-11567—In exercise of the powers conferred by the

Section-3 of Bihar Right to Public Services Act ,2011 (Bihar Act,4,2011), the following Services of

Social Welfare Department and Health Department are hereby added by the State Government, as

serial no 27 and serial no 28, respectively at the end of Appendix-1 of the notification no 1423

dated 11.05.2011 (Substituted by the notification no 1943 dated 23.12.2013) (as amended from

time to time) of the General Administration Department, Bihar, Patna with immediate effect :-

2. To avail these services under Bihar Right to Public Service Act, 2011,the schemewise application

form shall be in prescribed performa-I and checklist of necessary documents to be attached with

the application form in performa-II, which are enclosed herewith.

3. The Social Welfare Department and the Health Department may issue other forms after

Consultation with the law Department, as per requirement.
