A.P. REGISTRATION OF BIRTHS AND DEATHS RULES, 1999 - INCLUSION OF DETAILS OF HUSBAND/WIFE IN THE DEATH RECORDS - AMENDMENT TO RELEVANT RULES.

NOTIFICATION
In exercise of the powers conferred under Section 30(1) of the Registration of Births and Deaths Act 1969, the Governor of Andhra Pradesh hereby makes the following amendment to Rule-5 of the Andhra Pradesh Registration of Births and Deaths Rules, 1999, as subsequently amended :
AMENDMENT
Under column 5 of Form-2(Death Report) and Form-6(Death Certificate) appended to the said Rules, the following modifications shall be incorporated :
"5a - Name of the Husband/Wife,
UID No. of the Husband/ Wife
5b - Age of Husband/Wife 5c - Contact details of Husband/Wife
Format of Death Register (Form No.8) would be changed automatically after making necessary changes in the legal part of the Death Reporting Form".
