Rc.No.2172/E6.D (VC)/2018,- In the reference 1st cited, the Commissioner, Andhra Pradesh Vaidya Vidhana Parishad, Gollapudi, Vijayawada has informed that Dr.A.Madhurima, CAS, CHC, Puttur, Chittoor District went on Unauthorised Absence for duty w.e.f. 13-06-2015.
Accordingly a Charge Memo was issued to Dr.A.Madhurima, CAS, CHC, Puttur, Chittoor District for her unauthorized absence for duty w.e.f. 13-06-2015 vide this office reference 2nd cited.
2 ANDHRA PRADESH GAZETTE EXTRAORDINARY [Part-II the post of Civil Assistant Surgeon on her personal affairs.In view of the above, the resignation tendered by Dr.A.Madhurima, CAS, CHC, Puttur, Chittoor District is hereby accepted from the date of her last working i.e. w.e.f. 13-06-2015 in terms of Rule 30 (a) (i) and (ii) of A.P. State and Subordinate Service Rules 1996.
The resignation is laid down in General Rule has stated above is that not only the service rendered by the member of service in a particular post held by her at the time of resignation but also all her previous service under the Government will stand forfeited.
The District Co-ordinator of Hospital Services, Chittoor is requested take necessary further action in the matter accordingly duly making an entry in Service Register of the individual and furnish the extract copy of SR where such entry is made with proper attestation along with compliance report.
