ORDER :
In the reference 3rd read above , the Mission Director , National Health Mission , Andhra Pradesh , has reported that , the Secretary to Government of India , Ministry of Health and Family Welfare , Ministry of Health and Family Welfare , in the reference 2nd read above has informed that , as per the recommendations of the 15th Finance Commission Grants accepted by the Government of India and announced as part of the Union Budget 2021-22 includes sector specific grants for health sector, primarily for addressing the gaps in the primary health and requested to constitute a District Level Committee ( DLC ) under Chairmanship of District Collector and comprising officials from the Health , Panchayathi Raj and Urban affairs and select representatives from the three tiers of urban and rural local bodies in the district and District Medical & Health Officer as convenor , duly co - opting the experts in the field ofGovernance and reforms of PRI / ULBs available in the districts in District Level Committee .
2. After careful examination of the proposal of the Mission Director, National Health Mission , Andhra Pradesh , Government hereby constitute in each District a District Level Committee with the following composition for submitting the proposals to the State Level Committee for approvals under FC - XV : -
