2. Representation dated 28-01-2022 from the Velagapudi Trust , Vijayawada addressed to the DM & HO , Krishna , Machilipatnam and to the Collector & District Magistrate .
3. From the Director of Public Health & Family Welfare , eoffice File No : HMF03-19021 / 1 / 2022 - E SEC - DPH ( Computer No. 1616321 ) .

ORDER :
In the circumstance state by the Velagapudi Trust , Vijayawada in their representations vide 2nd read above and in the circumstances reported by the Director of Health & Family Welfare , A.P. , Mangalagiri in the reference 3rd read 2
above , Government hereby accord permission to establish a new Urban Primary Health Centre ( UPHC) at Yanamalakuduru ( V) , Penamaluru ( M ) Tadigadapa Municipality , Krishna District
2. In the reference 1st read above , the Planning Department in Government have issued instructions for naming the institutions ( Schools & Hospitals ) after donors . According to the instructions , ( a ) the eligibility norms for donors to get naming rights for entire hospital are as follows :

3. In view the above , the Director of Health & Family Welfare , A.P. , is advised to request the donors i.e. , Velagapudi Trust , Vijayawada to bear some of the recurring costs of the Urban Primary Health Centre ( UPHC ) apart from the full cost of the upfront infrastructure and equipment .
4. The Director of Health & Family Welfare , A.P. , Mangalagiri , Guntur District and Managing Director , APMSIDC , Mangalagiri , Guntur District shall take necessary action accordingly .
5. This orders issues with the concurrence of the Finance ( FMU.HM & FW ) Department vide (C.No.1687407 ) , dated : 19-01-2023 . U.O.No.FIN01 - FMU0PC ( HMF1 ) /88 / 2022 - FMU - WORKS ( BY ORDER AND IN THE NAME OF THE GOVERNOR OF ANDHRA PRADESH )
