I, Shri Mithun Ghosh son of Digendra Kumar Ghosh an inhabitant of Village- Sachinpur, P.O.- Sachinpur , P.S.- Dholai in the District of Cachar have changed my father's name from Digendra Kumar Ghosh to Digendra Chandra Ghosh by swearing an affidavit in the Court of Magistrate at Cachar on dated 18th April , 2018.
Henceforth, he will be known as Digendra Chandra Ghosh instead of Digendra Kumar Ghosh for all purposes .
