I, Jhunuray Karmakar Pal daughter of Late Shyamapada Roy Karmakar wife of Ratan Gopal Paul an inhabitant of Town- Ulubam (Part), P.S.- Sonapur in the District of Kamrup (M) have changed my name from Jhunuray Karmakar Pal to Jhunu Roy Karmakar Paul by swearing an affidavit in the Court of Notary at Kamrup (M) on dated 1st April, 2021.
