I, Dijen Baruah an inhabitant of Town- Duliajan, P.O.- Duliajan, P.S.- Duliajan in the District of Dibrugarh have changed my wife's name from Deepsikha Boruah to Dipsikha Baruah by swearing an affidavit in the Court of Judicial Magistrate 1st class at Dibrugarh on dated 21st December, 2021.

Henceforth, she will be known as Dipsikha Baruah instead of Deepsikha Boruah for all purposes.
