का.आ. 128(अ).—जब(cid:19)क मैसस(cid:23) (cid:4)हमालयन हेल(cid:8) स(cid:28)व(cid:23)स (cid:30)ा. ल मटेड का Ecureuil AS350 बी3 हे लकॉ&टर VT- JKB कटरा से सांझी छत के लए उड़ान के दौरान (cid:4)दनांक 23.11.2015 को कटरा, ज(cid:12)मू म1 दुघ(cid:23)टना का शकार हो गया था ।
और जब(cid:19)क, के 9:(cid:8)य सरकार को यह उ;चत (cid:30)तीत होता है (cid:19)क इस घटना के कारण और अंशदायी कारक@ का अ9वेषण और Bनधा(cid:23)रण (cid:19)कया जाना, और भ(cid:28)वEय म1 ऐसी दुघ(cid:23)टना कG पुनरावृBत से बचने के लए सफाKरश1 करना समीचीन है।
अब, वायुयान (दुघ(cid:23)टनाओं एवं घटनाओं का अ9वेषण) Bनयमावल(cid:8), 2012 के Bनयम 11 (1) म1 (cid:30)दOत शिQतय@ का (cid:30)योग करते हुए, के 9:(cid:8)य सरकार एतदRवारा इस गंभीर घटना कG औपचाKरक जांच का Bनदेश देती है।
तदनुसार, के 9:(cid:8)य सरकार Sी यश पाल, पूव(cid:23)-उप महा(cid:30)बंधक (अनुरUण), पवन हंस ल मटेड (PHL) कG अVयUता म1 Bन(cid:12)न लWखत सदYय@ के साथ एक जांच स मBत BनयुQत करती है:-
(i) Sी के . रामचं:न, (cid:28)वमान सुरUा अ;धकार(cid:8), एएआईबी —सदYय
(ii) कै &टन जे.पी.एस. Bनगार —सदYय
स मBत, जब कभी अपे\Uत होगा, अ9य (cid:28)वशेष]@/एज1 सय@ कG सहायता भी लेगी।
स मBत का मु_यालय नई (cid:4)द(cid:6)ल(cid:8) म1 होगा।
स मBत अ;धमानतः छह मह(cid:8)ने के भीतर इसकG जांच पूर(cid:8) करके अपनी Kरपोट(cid:23) के 9:(cid:8)य सरकार को (cid:30)Yतुत करेगी।

S.O. 128(E).—Whereas M/s Himalayan Heli Services Pvt. Ltd. Ecureuil AS350 B3 helicopter VT-JKB met with accident at Katra, Jammu on 23.11.2015 while operating flight from Katra to Sanjhi Chhat.
And whereas, it appears to the Central Government that it is expedient to investigate and determine the causes and contributory factors leading to the said accident and make recommendations to avoid recurrence of such accidents in future.
Now, in the exercise of the powers conferred by the Rules 11(1) of the Aircraft (Investigation of Accidents and Incidents) Rules, 2012, the Central Government hereby directs that a formal inquiry in the serious incident be made.
Accordingly, the Central Government hereby appoints a Committee of Inquiry headed by Shri Yash Pall, Ex DGM (Maintenance), PHL with following members:
(i) Shri K. Ramachandran, Air Safety Officer, AAIB as member
(ii) Capt. J.P.S. Kniggar as member
The committee will also take the assistance of other experts/agencies whenever required.
The headquarters of the Committee will be at New Delhi.
The Committee will complete its inquiry and submit its report to the Central Government preferably within six months.
