सं. यू-16/53/1/99 -धि. 2( गुजरात ) कर्मचारी राज्य बीमा (साधारण ) विनियम, 1950 के विनियम-105 के अधीन निगम की शक्तियां महानिदेशक को प्रदान करने के संबंध में कर्मचारी राज्य बीमा निगम द्वारा 25-4-95 की बैठक में पारित किए गए संकल्प के अनुसरण में तथा महानिदेशक के आदेश संख्या 1024 ( जी ) दिनांक 23-5-1993 द्वारा ये शक्तियां आगे मुझे सौंपी जाने पर मैं इसके द्वारा बड़ौदा केन्द्र व क्षेत्रीय उप चिकित्सा आयुक्त ( उत्तर- पश्चिम जोन ) द्वारा नियत किए गए क्षेत्रों के लिए, बीमाकृत व्यक्तियों की स्वास्थ्य जांच करने और मूल प्रमाण- पत्र की सत्यता संदिग्ध होने पर उन्हें और प्रमाण- पत्र प्रदान करने के प्रयोजन के लिए मौजूदा मानकों के अनुसार मासिक पारिश्रमिक पर चिकित्सा प्राधिकारी के रूप में कार्य करने के लिए डॉ. डी. एम. खालसा की सेवाएं कार्यभार ग्रहण करने की तिथि से एक वर्ष के लिए या पूर्णकालिक चिकित्सा निर्देशी के कार्य- ग्रहण करने तक, जो भी पहले हो, बढ़ाती हूं।

सं यू-16/ 53/99-चि.-2 ( केरल ) कर्मचारी सछ बीग विनियम, 1950 के विनियम -105 के तहत महानिदेशक को शक्तिया प्रदान करने के संबंध में कर्मचारी राज्य बीमा निगम को दि 25 अप्रैल, 1951 को हुई बैठक में पास किए गए संकल्प के अनुसरण तथा महानिदेशक के आदेश संख्या 1024( जी ) दिनांक 23- द्वारा ये शक्तियां आगे मुझे सौंपी आने पर मैं इसके द्वारा निम्नलिखित डॉक्टर को मानकों के अनुसार देय पारिश्रमिक पर निम्नलिखित तिथि तक एक वर्ष के लिए या पूर्णकालिक चिकित्सा निर्देशी के कार्यग्रहण करने तक, जो भी पूर्व हो , को उप चिकित्सा आयुक्त ( दक्षिण जोन ) , बंग और द्वारा निर्धारित क्षेत्र के लिए बीमाकृत व्यक्तियों की स्वास्थ्य परीक्षा दे तथा मूल प्रमाण- पत्र की सत्यता संदिग्ध होने पर उन्हें आगे प्रमाण- पत्र जारी करने के प्रयोजन के लिए चिकित्सा अधिकारी के रूप में कार्य करने के लिए प्राधिकृत करती हूं: --

अषचार का दोषी पाया गया है । उपर्युक्त अवचार की बाबत इन्स्टीट्यूट की परिषद् ने उक्त श्री गुप्ता हेमन्त कुमार को उक्त अधिनियम की धारा 21 (4) के अधीन सुनवाई का अवसर देने के पश्चात् यह आदेश किया है कि उनका नाम एक माह की कालावधि के लिए सदस्यों के रजिस्टर से हटा दिया जाए । इसके अनुसरण में , एतद्वारा, यह अधिसूचित किया जाता है कि उक्त श्री गुप्ता हेमन्त कुमार ( सदस्यता संख्या -82016 ) का नाम 29 दिसम्बर, 2001 से एक माह की कालावधि के लिए सदस्यों के रजिस्टर से हटा दिया जाएगा ।
