सं. एन-15/13/6/8/2003-यो. एवं वि. कर्मचारी राज्य बीमा ( सामान्य ) विनियम 1950 के विनियम 95 - क के साथ पठित कर्मचारी राज्य बीमा अधिनियम , 1948 ( 1948 का 34 ) की धारा -46 (2 ) द्वारा प्रदत्त शक्तियों के अनुसरण में महानिदेशक ने 1 मार्च , 2004 ऐसी तारीख के रूप में निश्चित की है जिससे उक्त विनियम 95 - क तथा केरल कर्मचारी राज्य बीमा नियम -1957 में निर्दिष्ट चिकित्सां हितलाभ केरल राज्य में निम्नलिखित क्षेत्रों में बीमांकित व्यक्तियों के परिवारों पर लागू किए जाएंगे, अर्थात्:--
" जिला तथा तालुक तिरूवनन्तपुरम के राजस्व ग्राम- उल्लूर के अधीन आने वाले क्षेत्र " ।

[भाग III-खण्ड 4 एस. के. जैन, ए. के. चतुर्वेदी को पैरा 4 ( 1 ) के परन्तुक के अन्तर्गत छत्तीसगढ़ राज्य के लिए क्षेत्रीय समिति ( क.भ.नि. ) में नियोक्ताओं के प्रतिनिधि के रूप में अतिरिक्त सदस्य नियुक्त करता है तथा दिनांक 8 नवम्बर, 2003 को भारत के राजपत्र के खंड 4 भाग 3 में प्रकाशित ( जारी संख्या 45 ) कर्मचारी भविष्य निधि संगठन, मुख्यालय की अधिसूचना संख्या सम्मेलन -5 ( 19 )/2003/छत्तीसगढ़/ 13933 दिनांक 22.10.2003 में निम्नलिखित संशोधन करता है ।
उक्त अधिसूचना में नियोक्ताओं के प्रतिनिधि पक्ष के अन्तर्गत योजना के पैरा 4 ( 1 ) में अतिरिक्त सदस्य के रूप में क्रम संख्या 10 से 13 को शामिल किया जाए, अर्थात्:--
( 10) श्री सुरेन्द्र कुमार जैन, बी-4, अनुपम नगर, टी. वी. टावर के पास , रायपुर, छत्तीसगढ़ ।

No. N- 15 /13 /6/8/2003 /P &D.-In pursuance of powers conferred by Section 46( 2) of the Employees' State Insur- ance Act, 1948 (34 of 1948 ), read with Regulation 95 -A of the Employees' State Insutrance (General ) Regulations , 1950, the Director General has fixed the 1st March 2004 as the date from which the medical benefits as laid down in the said Regulation 95- A and the Kerala Employees' State Insurance (Medical Benefit) Rules , 1957 shall be extended to the families of insured persons in the following area in the State of Kerala namely.
"Ulloor Revenue Village in Thiruvananthapuram Taluk & District " .
