सं. यू-16/53/94- चिकित्सा-2 ( राजस्थान ) कर्मचारी राज्य बीमा (साधारण ) विनियम 1950 के विनियम 105 के तहत महानिदेशक को निगम की शक्तियाँ प्रदान करने के संबंध में कर्मचारी राज्य बीमा निगम की दिनांक 25 अप्रैल , 1951 को हुई बैठक में पास किये गये संकल्प के अनुसरण में तथा महानिदेशक के आदेश संख्या 1024 (जी ) दिनांक 23.5.1983 द्वारा ये शक्तियाँ आगे मुझे सौंपी जाने पर मैं इसके द्वारा डॉ. जे . पी . धमीजा , अंशकालिक चिकित्सा निर्देशी कोमानकों के अनुसार देय पारिश्रमिक पर 01.10.2003 से 30.09 . 2004 तक की अवधि के लिए या पूर्णकालिक चिकित्सा निर्देशी के कार्यग्रहण करने पर जो भी पहले हो , उप चिकित्सा आयुक्त (उत्तर- पश्चिम जोन ) , जयपुर द्वारा निर्धारित श्री गंगानगर क्षेत्र के लिए बीमाकृत व्यक्तियों की स्वास्थ्य परीक्षा करने तथा मूल प्रमाण - पत्र की सत्यता संदिग्ध होने पर उन्हें आगे प्रमाण- पत्र जारी करने के प्रयोजन के लिए चिकित्सा अधिकारी के रूप में कार्य करने के लिए प्राधिकृत करती हूँ ।

भारतीय चार्टर प्राप्त लेखाकार संस्थाने (संसद के एक अधिनियम के अधीन स्थापित ) नई दिल्ली-110002, दिनांक 29 जून 2004 (चार्टर्ड एकाउंटेंट्स)
सं. 1.सी.ए. ( 7 ) / 76/2004--चार्टर्ड एकाउंटेंट्स विनियम 1988 के विनियम 159 ( 1 ) के अनुसरण में इंस्टीट्यूट ऑफ चार्टर्ड एकाउन्टेंट्स ऑफ इंडिया की परिषद् मध्य भारत प्रादेशिक परिषद् की एक शाखा की दिनांक 18 अप्रैल 2004 से जबलपुर में स्थापना की अधिसूचित करती है।
यह शाखा मध्य भारत प्रादेशिक परिषद् की जबलपुर शाखा के नाम से जानी जाएगी ।
इस शाखा के क्षेत्राधिकार के अन्तर्गत जबलपुर शहर आएगा । जैसा कि विनियम 159 ( 3 ) में विहित किया गया है, यह शाखा परिषद् के नियंत्रण, पर्यवेक्षण तथा निर्देशों के अधीन रहते हुए मध्य भारत प्रादेशिक परिषद् की मार्फत कार्य करेगी तथा ऐसे निर्देशों का पालन करेगी , जो परिषद् द्वारा समय - समय पर जारी किए जाएंगे ।

No. U - 16/53/94- Med.II ( Rajasthan) :-In pursuance of the resolution passed by ESI Corporation at its meeting held on 25.4.1951 conferring upon the Director General the powers of the Corporation under Regulation 105 of the ESI ( General ) Regulations , 1950 and such powers futher delegated to me vide Director General's Order No. 1024( G) dated 23.5.1983 , I hereby authorise Dr. J. P. Dhamija , PTMR to function as Medical Authority for Rajasthan Centre for the period of one year from 1.10.2003 to 30.9.2004 ог till the full time Medical Referee joins , whichever is earlier, for Shri Ganganagar region or areas to be allocated by Regional Dy . Medical Commissioner (NWZ) Ahmedabad for the purpose of medical examination of the insured persons and grant of further certificates to them when the correctness of the original certificate is in doubt .

No. 1 - CA( 7)/ 76/2004-In pursuance of Regulation 159(1 ) of the Chartered Accountants Regulations , 1988 , the Council of the Institute of Chartered Accountants of India is pleased to notify the setting up of a branch of Central India Regional Council at Jabalpur with effect from 18th April , 2004 .
The Branch shall be known as Jabalpur Branch of the Central India Regional Council and shall have Jabalpur City under its jurisdiction.
Asprescribed under Regulation 159( 3 ) , the Branch shall function subject to the control, supervision and directions of the council through the Central India Regional Council and shall carry out such directions as may from time to time be issued by the Council .
