बैंपविवि. सं. आईबीएस . 591 / 23.03.012/ 2004-05-- भारतीय रिज़र्व बैंक अधिनियम, 1934 ( 1934 का 2 ) की धारा 42 की उप - धारा ( 6 ) के खण्ड ( ग ) के अनुसरण में भारतीय रिज़र्व बैंक इसके द्वारा निदेश देता है कि उक्त अधिनियम की दूसरी अनुसूची में 13 नवंबर 2004 से निम्नलिखित परिवर्तन किया जाए :--
" जेपीमॉर्गन चेज़ बैंक " शब्दों के स्थान पर " जेपीमॉर्गन चेज़ बैंक, नेशनल एसोसिएशन " शब्द होंगे ।

सं. यू-16/ 53/2001 चिकित्सा शाखा-2 ( केरल ) कर्मचारी राज्य बीमा ( साधारण ) विनियम, 1950 के विनियम 105 के अधीन निगम की शक्तियाँ महानिदेशक को प्रदान करने के संबंध में कर्मचारी राज्य बीमा निगम की दिनांक 25 अप्रैल , 1951 को हुई बैठक में पारित किए गए संकल्प के अनुसरण में तथा महानिदेशक के आदेश संख्या 1024 ( जी ) दिनांक 23.5.1983 द्वारा ये शक्तियां आगे मुझे सौंपी जाने पर मैं इसके द्वारा निम्नलिखित डॉक्टर को मानकों के अनुसार देय पारिश्रमिक पर निम्नलिखित तिथि तक एक वर्ष के लिए पूर्णकालिक चिकित्सा निर्देशी के कार्यग्रहण करने तक, जो भी पूर्व हो, को राजकीय चिकित्सा आयुक्त ( दक्षिण क्षेत्र ) बंगलौर द्वारा निर्धारित क्षेत्र के लिए बीमाकृत व्यक्तियों की स्वास्थ्य परीक्षा करने तथा मूल प्रमाण-पत्र की सत्यता संदिग्ध होने पर उन्हें आगे प्रमाण- पत्र जारी करने के प्रयोजन के लिए चिकित्सा अधिकारी के रूप में कार्य करने के लिए प्राधिकृत करती हूँ।

"जिला, तहसील एवं परगना बरेली के राजस्व ग्राम- भोजीपुरा , पीपलसाना चौधरी , पचदियोरा दियोहरिया , विलवा, करमपुर चौधरी , करगैना, बिदारभान नगला एवं अभयपुर के अन्तर्गत आने वाले क्षेत्र । "
