तिथि तक एक वर्ष के लिए पूर्णकालिक चिकित्सा निर्देशी के कार्यग्रहण करने तक , जो भी पूर्व हो , को राज्य चिकित्सा आयुक्त, अहमदाबाद द्वारा निर्धारित क्षेत्र के लिए बीमाकृत व्यक्तियों की स्वास्थ्य परीक्षा करने तथा मूल प्रमाण - पत्र की सत्यता संदिग्ध होने पर उन्हें आगे प्रमाण पत्र जारी करने के प्रयोजन के लिए चिकित्सा अधिकारी के रूप में कार्य करने के लिए प्राधिकृत करती हूं:

परीक्षा शुल्क की राशि का भुगतान बैंक के डिमांड ड्राफ्ट द्वारा होना चाहिए। आवेदन पत्र उपयुक्त शुल्क के डिमान्ड के साथ इस प्रकार भेजा जाना चाहिए कि वह संयुक्त सचिव ( परीक्षा ) के कार्यालय में दिनांक 01.06.2005 तक पहुंच जाए। दिनांक 01.06.2005 के बाद आवेदन पत्रों को स्वीकार नहीं किया जाएगा ।

इस परीक्षा के लिए आवेदन निर्धारित आवेदन पत्र पर ही किया जाना चाहिए जो कि इंस्टीट्यूट ऑफ चार्टर्ड एकाउन्टेन्ट्स ऑफ इण्डिया के संयुक्तसचिव ( परीक्षा ) के नई दिल्ली स्थित कार्यालय से 25 रुपए प्रति आवेदन पत्र भुगतान करने पर मिल सकता है । आवेदन पत्र इंस्टीट्यूट के रीजनल और ब्रांचों के कार्यालयों में भी उपलब्ध है और नकद भुगतान करने पर 11 मई 2005 से प्राप्त किए जा सकते हैं । आवेदन पत्र इंस्टीट्यूट की वेबसाइट डब्लुडब्लुडब्लु. आई.सी.ए. ओ.आर.जी. से प्रिंट करके परीक्षा शुल्क 1025 रुपए के साथ भी भेजा जा सकता है ।

श्रेणी- II (द्वितीय)
( परीक्षा केन्द्र जिनमें आई.एस.ए. की परीक्षा होगी बशर्ते कि इस परीक्षा केन्द्र में कम से कम 25 परीक्षार्थी निवेदन करते हैं ) ।
उपरोक्त पेपर की यह परीक्षा अब बुधवार 11 मई 2005 को 8 बजे से 11 बजे सुबह तक होगी, परीक्षा केन्द्रों में कोई परिर्वतन नहीं होगा । परीक्षा प्रवेश पत्र भी वही रहेंगे जो कि पहले से ही जारी किये गये थे । परीक्षा केन्द्र

(Centre at which ISA AT would be held subject to a minimum of 25 candidates opting to appear from this centre)
1. REWARI
The candidates who opt for a centre mentioned under category - II would be required to give one of two alternate centres in order of preference from centres mentioned under Category I. In the event of a minimum of 25 candidates not opting for a centre mentioned under Category II , candidates who had opted for the said centre would be allotted to the centre of their choice in order of prefernce in Category I as mentioned in their application form . If any candidate who has opted for a centre in Category II has not given his option for any alternate centre in Category I , he will be allotted to any of the nearest centre in Category I as per the discretion of the Institute.
The Council reserves the right to withdraw any centre at any stage without assigning any reason . The above test is open only to eligible Members of the Institute who are already registered with the Institute for the said course . The fees payable for the above Assessment Test is Rs . 1000/-.
Payment of fees for the Assessment Test should be made only by Demand Draft . The Demand Draft may be of any Scheduled Bank and should be drawn in favour of the Secretary, The Institute of Chartered Accountants of India, payable at New Delhi only. application together with the prescribed fee be sent so as to reach the Joint Secretary (Exams) at New Delhi on or before 1st June 2005 .
Applications for admission to the Assessment Test is required to be made in the prescribed form which may be obtained from the Joint Secretary ( Exams ) , The Institute of Chartered Accountants of India, Indraprastha Marg , PART III -SEC . 4 ]
New Delhi - 110002 on payment of Rs . 25 /- per application form . The forms are also available in the Regional and Branch Offices of the Institute and can be obtained on cash payment on or from 11th May 2005. Alternatively, the format of application form can be downloaded from the website of the Institute viz . www.icai.org and the cost of the application form of Rs . 25/- can be added to the Assessment Test fee of Rs . 1000/- and the Demand Draft for Rs . 1025/- has to be sent . The last date for receipt of duly filled in application forms is 1st June 2005. The application together with the prescribed fee should be sent by Speed Post/Registered Post to the Joint Secretary (Exams . ) . New Delhi . The applications received after 1st June 2005 will not be entertained under any circumstances .

No. 13 - CA ( Exam )/M/2005 .---in partial modification of the Institute's Notification No. 13 - CA ( Exam )/M /2005 dated 15th January , 2005 ,it is notified for general information that Paper - 4-Corporate Laws and Secretarial Practice of Group - I of the Chartered Accountants Final Examination Scheduled to be held on Thursday, the 5th May, 2005 stands postponed .
The Examination in the said paper will nowbe held on Wednesday, the 11th May, 2005 from 8.00 A.M. to 11.00 A.M. (Morning Session) . The venues of the examination will remain unchanged . Admit Cards already issued would remain valid .
