प्रशस्तिउल्लेख
1. कमांडेन्ट अली मुत्ताहर (0104- एल ) ने 17 अगस्त 1984 से भारतीय तटरक्षक में सेवा शुरू की । 27 अप्रैल 2004 को इन्होंने भारतीय तटरक्षक पोत विवेक की कमान संभाली । ये पहले वाणिज्यक पायलट लाइसेंस ( सी पी एल) प्रवेश विमान चालक अफसर हैं, जिन्होंने अपतटीय गश्ती पोत (ओ पी वी ) की कमान संभाली । नेतृत्व में वयावसायिकता तथा कुशल संचालन का मिश्रण और उन सबसे उपर अपने कार्मिकों को प्ररेणा प्रदान करना , जिसके फलस्वरूप उन्होने पोत को एक सुगठित व्यवसायिक यूनिट के रूप में ढाल दिया ।
2. 26 दिसम्बर 2004 , को उत्तरी अण्डमान द्वीप के पूर्व में जब पोत अनन्य आर्थिक क्षेत्र में गश्त कर रहा था तो उसे भारी कंपकपाहट का अनुभव हुआ जिसका बाद में पता चला कि वह सुमात्रा के 160 कि.मी. पश्चिम में उत्केन्द्रित भूकंप के कारण हुआ था, जिसने दक्षिण पूर्व एशिया को दोलित कर दिया । रिक्टर स्केल पर भूकंप का परिमाप 9.0 था । ग्रेट निकोबार द्वीप में राहत अभियानों के लिए पोत को तुरन्त बुलाया गया । वापसी पर पोत को इन्टरव्यूह द्वीप में एक पुलिस चौकी के आठ पुलिस कर्मियों को स्थानांतरित करने का आदेश प्राप्त हुआ । केवल चार चोट- ग्रस्त पुलिस कर्मियों को स्थानांतरित किया जा सका, क्योंकि शेष चार कर्मियों को पुलिस चौकी पर शस्त्र तथा गोला बारूद के अनुरक्षण हेतु छोड़ना पड़ा । 27 दिसम्बर 2004 के प्रातः काल में पोत ने पोर्ट ब्लेयर के बंदरगाह में प्रवेश किया तथा 1800 बजे वह 12500 किलोग्राम सूखा राशन , 1200 लीटर केरोसिन तेल तथा अन्य राहत सामाग्री लेकर कैम्पबेल बे को रवाना हुआ, क्योंकि ग्रेट निकोबार क्षेत्र में बड़े पैमाने पर विनाश तथा हताहतों की सूचना प्राप्त हुई थी ।
3. कैम्पबेल बे की ओर जाते हुये , चोवरा द्वीप पर हवाई सर्वेक्षण करने के लिए पोत से हेलिकाप्टर ने उड़ान भरी । हेलिकॉप्टर ने मृतकों तथा धायलों की अधिक संख्या की सूचना दी । पोत ने तुरंत द्वीप पर अपने चिकित्सा दल को हवाई मार्ग से उतार दिया । पोत के चिकित्सा दल ने द्वीप के सभी घायलों को पूर्ण चिकित्सा उपचार उपलब्ध कराया तथा गंभीर रूप से चोट ग्रस्त 25 लोगों को पोत में स्थानांतरित किया । शेष जनसंख्या को 200 किलोग्राम चावल , 100 किलोग्राम दाल तथा दो दिन के लिए पका भोजन उपलब्ध कराया गया । घायलों को पोर्टब्लेयर स्थानांतरित करने के लिए कार निकोबार ले जाया गया । पोत 29 दिसम्बर 2004 की शाम को कैम्पबेल बे पहुंचा । पहली निकासी कार्रवाई चिंगाम 888

[भाग I-खण्ड 1 बस्ती से शुरू की गई जहां पोत के अंगभूत हेलिकॉप्टर तथा नौका ने 67 असहाय स्थानीय लोगों की निकासी की । पोत ने दक्षिणी खाड़ी में लंगर डाला हुआ था जिससे कि कम से कम समय में अधिक लोगों की निकासी की जा सके । निकासियों के लिए पोत की नौका तथा हेलिकॉप्टर का पूरे दिन व्यापक रूप से उपयोग किया गया । दिन के अंत तक 81 उत्तरजीवितों की निकासी की जा चुकी थी ।
4. 01 जनवरी 2005 को लगभग 1700 बजे , पोत के हेलिकॉप्टर ने निकासी विशेष से लौटने पर कैम्पबेल बे के पास इण्डोनेशियाई नौका के उपस्थित होने की सूचना दी । नौका तथा उसके कर्मियों की स्थिति खराब थी । वे समुद्र में अनुमानित रूप से पिछले 8-9 दिनों से असहाय बह रहे थे । चूंकि सूर्यास्त पश्चात हेलिकॉप्टर अभियान संभव नहीं था , कमान अधिकारी ने इण्डोनेशियाई कार्मिकों के बचाव के लिए पोत की नौका को भेजने का निर्णय लिया । कमान अफसर के मार्गदर्शन के अनुसार पूरी रात अभियान चालू रहा तथा 12 लोगों को बचाया गया । 5. परिस्थिति की अनिश्चितता तथा प्रतिकूल मौसम की स्थिति, कमान अफसर के रूप में अफसर की योग्यता को परख रही थी इस संकटमय स्थिति में पोत ने पोतारोहण तथा राहत सामग्री के वितरण के अत्यंत कठिन कार्य तथा सैकड़ों मूल्यवान जीवनों की निकासी एक व्यावसायिक ढंग से की । उत्कृष्ट कमान तथा नियंत्रण नजदीकी एकजुट टीम कार्य , लक्ष्य केन्द्रित अभिविन्यास तथा आश्चर्यजनक नेतृत्व विशेषताओं की पहचान के लिए कमांडेन्ट अली मुत्ताहर (0104 - एल) को तटरक्षक पदक (शौर्य) प्रदान करने की कड़ी सिफारिश की जाती है । 889 भारत का राजपत्र, सितम्बर 10, 2005 ( भाद्रपद 19, 1927 )

1. कमांडेन्ट मिलींद मनोहर पाटील ( 0252- वी) ने 14 जनवरी 1990 को तटरक्षक में सेवा आरम्भ की । इन्होंने 22 मई 2004 को तटरक्षक जिला कमांडर , निकोबार (कोमडिस 10 ) का कार्यभार संभाला ।
2. 26 दिसम्बर 2004 को कैम्पबैल बे में 8.9 रिक्टर स्केल की तीव्रता से आये भूकंप के कारण सूनामी प्रभाव से ग्रेट निकोबार द्वीप को अभूतपूर्व झटका लगा तथा बड़े पैमाने पर क्षति पहुंची । कमांडेन्ट मिलींद मनोहर पाटील , लाल टिकरी में पारावारिक आवास परिसर में रह रहे सभी भारतीय तटरक्षक के परिवारों की सुरक्षा सुनिश्चित करने के लिए पहुँचें । उसके तुरन्त बाद ही उन्होने पारिवारिक आवास में रह रहे कर्मियों को साथ लेकर अन्यत्र पारिवारिक आवास (ओ टी एम) में रह रहे कर्मियों की स्थिति की जांच की । एकल आवास में रह रहे अभ्यावेशित व्यक्तियों की सुरक्षा सुनिश्चित होने पर , कमांडेन्ट मिलींद मनोहर पाटील ने अपना ध्यान कैम्पबेल बे नगर की ओर केन्द्रित किया ।
3. अफसर ने सरकारी संगठनों के स्थानीय प्रमुखों के संयोजन से एक आपदा प्रबंधन दल तैयार किया । चूँकि कैम्पबेल बे की जेट्टी पूरी तरह बर्बाद हो चुकी थी , इसलिए अफसर ने हवाई पट्टी को स्थिति जानने के लिए कूच किया । कैम्पबेल बे तथा शेष विश्व के बीच विमान ही अकेला माध्यम है । हालांकि सूनामी के कारण हवाई - पट्टी पर भारी मात्रा में मलबा गिरा था , परन्तु अफसर ने अनुमान लगाया कि हवाई- पट्टी अभी भी इस्तेमाल योग्य है । अफसर ने सैन्य इंजीनियरिंग सेवा ( एम ई एस) की सहायता से हवाई पट्टी (रनवे) को साफ करवाने का कार्य शुरू किया । अफसर की दूरदृष्टि तथा दृढ संकल्प के परिणाम स्वरूप हवाई पट्टी रिकार्ड समय . में चालू हो गयी , फलस्वरूप कैम्पबेल बे में भारी मात्रा में राहत सामग्री प्राप्त हो सकी ।
4. कमांडेन्ट मिलींद मनोहर पाटील ने नजदीकी क्षेत्र में नौचालन कर रहे भारतीय तटरक्षक पोत कनकलता। बरूआ को विभिन्न स्थानों से चोटग्रस्त लोगों की निकासी के लिए , तैयार किया । उन्होंने , सभी संभव संसाधनों को एकत्र कर , शिविरों की व्यवस्था करने तथा ज्यादा चोटग्रस्त तथा प्रभावित लोगों को राहत सुनिश्चित करने के लिए , अथक कार्य किया । मलबों में दबे वाहनों तथा नौकाओं 2A-231 -G1/2005 890

[भाग I-खण्ड 1 को, निकालने तथा उनको क्रियाशील बनाने में अफसर सहायक रहे । अफसर ने भारतीय तटरक्षक द्वारा व्यवस्थित सुधार शिविरों में स्थानांतरित कर लाये गये लगभग 3000 लोगों को आहार तथा चिकित्सा सहायता की प्राथमिकता सुनिश्चित की । ऐसी स्थिति में टीम कार्य के महत्व को समझते हुए , कमांडेन्ट मिलींद मनोहर पाटील से अण्डमान तथा निकोबार कमान तथा अन्य स्थानीय संगठनों के अन्य अवयवों की प्रर्याप्त सहायता का प्रबंध किया । उन्होंने जहाजरानी सेवाएं निदेशालय के पोत एम वी कैचल को संभारिकी सहायता दी तथा उसे लिटिल निकोबार द्वीप समूह के लिए तैनात करवाया जहां पोत ने लगभग 200 उत्तरजीवितों की निकासी की । उन्होने सैन्य इंजीनियरिंग सेवा , अण्डमान लोकनिर्माण विभाग तथा अण्डमान तथा लक्षद्वीप बंदरगाह निर्माण विभाग के प्रतिनिधियों के संयुक्त दल को इंदिरा प्वांइट की जा रहे रोड़ का सर्वेक्षण करने तथा उसे अस्थायी रूप से मरम्मत करने की संभाव्यता का मूल्यांकन करने के लिए तैनात किया । कुछ ही दिनों के अर्न्तगत वे 14 राहत शिविरों में रह रहे लगभग 5800 लोगों के प्रशासन का पर्यवेक्षण कर रहे थे । अफसर ने विभिन्न संस्थाओं के मध्य अपेक्षित समन्वय को सुनिश्चित किया ताकि राहत अभियानों से वाछिंत परिणाम प्राप्त हो सकें । उनकी नेतृत्व योग्यता के परिणाम स्वरूप हजारों असहाय लोगों की निकासी तथा मूल्यवान जीवनों का बचाव सफलतापूर्वक किया जा सका ।
5. जिला मुख्यालय (निकोबार ) के अन्यत्र पारिवारिक आवास ( ओ टी एम) परिसर के नीचे क्षेत्र में स्थित होने के कारण वह सूनामी की वजह से पूर्णतया बर्बाद हो चुका था । कमांडेन्ट मिलींद मनोहर पाटील के उत्कृष्ट जनसंपर्क के कारण उन्होंने जिला मुख्यालय निकोबार के अन्यत्र पारिवारिक आवास ( ओ टी एम) परिसर की पुर्नस्थापना के लिए स्थानीय प्रशासन से भारतीय तटरक्षक को 9.18 हेक्टेयर राजस्व भूमि दिलवाने में सफलता हासिल की । अफसर द्वारा शुरू की गई सामयिक कार्रवाई के कारण जिला मुख्यालय को पुनः शुरू करने में भारतीय तटरक्षक को मंदद मिली ।

1. कमांडेंन्ट ( क श्रे ) पी एस मल्होत्रा (0305 - एस ) ने 11 जनवरी 1992 को भारतीय तटरक्षक में सेवा शुरू की । 28 जून 2002 को इन्होंने 848 तटरक्षक हेलिकॉप्टर स्क्वाड्रन के स्टाफ पायलट का कार्यभार ग्रहण किया । इन्हें 2265 घंटों का उड़ान अनुभव है तथा ये चेतक हेलिकॉप्टर के एक पूर्णतया संक्रियात्मक मास्टर ग्रीन पायलट हैं । अफसर एक अति-अभिप्रेरक तथा एक कुशल उड़ाक हैं जोकि स्क्वाड्रन की सभी संक्रियात्मक तथा सौंपे गये कार्यों को शत प्रतिशत पूरा करने में सहायक रहा है ।
2. 26 दिसम्बर के दुर्भाग्यपूर्ण रविवार को 0845 बजे तटरक्षक वायु स्टेशन(चेन्नई) से एक खोज तथा बचाव ( एस ए आर) संदेश प्राप्त हुआ जिसमें ऊँची लहरों द्वारा मरीना समुद्रतट(बीच) ग्रस लेने तथा लोगों के डूबने की सूचना दी गयी थी । कोई एक पल गंवाये बिना , कमांडेंट (क श्रे) पी एस मल्होत्रा, तात्कालिक विमानवाहित सह - पायलट तथा दो वायुकर्मी गोताखोरों के साथ , भारतीय तटरक्षक हेलिकाप्टर 806 से उड़ान भरने के लिए चल पड़े । मरीना बीच पर पायलटों को सूनामी लहरों की वजह से हुई बर्बादी दिखाई दी । नौकाओं , कुटिरों तथा लोगों के साजो- सामान को क्षुब्ध लहरें ऊपर तथा नीचे की ओर उछाल रही थीं । स्थिति की गंभीरता को समझते हुए , कमांडेन्ट (क श्रे) पी एस मल्होत्रा तुरन्त हरकत में आये तथा समुद्र में एक संगठित खोज प्रतिरूप कार्रवाई शुरु कर दी । थोड़ी ही देर में हेलिकॉप्टर ने एक लकड़ी के छोटे तख्ते के सहारे तैरते हुए तथा व्यग्र हिल्लोरों पर मदद के लिए पुकारते एक आदमी को ढूंढ निकाला । उन्होंने तुरन्त अपने गोताखोंरों को डूबते व्यक्ति के नजदीक उतारा । गोताखोर ने व्यक्ति को बचाव पट्टे से बाँधकर हेलिकॉप्टर में ऊपर खींच लिया । व्यक्ति को तुरन्त चिकित्सा सहायता के लिए क्षेत्रीय मुख्यालय (पूर्व) के सुपुर्द कर दिया । उन्होंने अपनी खोज पुनः शुरू कर दी तथा रोयापुरम मछुवाही बंदरगाह के पास एक डूबती नौका के सहारे जीवन के लिए संघर्ष करते ओर दो मछुवारों को ढूंढ लिया । एक बार पुनः गोताखोर को उतारा गया तथा दोनों मछुवारों को एक- एक करके हेलिकॉप्टर में चढ़ाया गया । हेलिकॉप्टर को नजदीकी एक संकरी मछुवाही जेट्टी पर उतारा गया जहां पर उन्हें इंतजार कर रहे लोगों की भीड़ को सौंप दिया गया । अब तक हेलिकॉप्टर का ईंधन कम हो चुका था तथा पायलटों ने निर्णय लिया कि पुनः वापस बेस में जाकर दूसरा एयरक्रॉफ्ट ले लिया जाये । समय खराब किए बिना कमांडेन्ट (क श्रे) पी एस मल्होत्रा ने दूसरा तैयार हेलिकॉप्टर लिया तथा पाँच मिनटों के अंदर 892

[भाग I-खण्ड 1 वे फिर हवा में कार्यरत थे । उन्होंने खुले समुद्र में कुछ मछुवारों को जीवन के लिए सघर्ष करते देखा, जिनकी नौकाओं के टुकड़े- टुकड़े हो चुके थे । मुक्त गोताखोर ने समुद्र में उतरकर उन्हें बचाव पट्टा पहनने में मदद की । उत्तरजीवितों को समुद्र से ले जाकर, तरंग - रोध पर एम्बुलेन्स के साथ इंतजार कर रहे लोगों के सुपुर्द कर दिया । तरंग- रोध के बहुत संकरे होने के बावजूद भी सूक्ष्म तथा अत्यंत उड़ान कौशल को प्रदर्शित करते हुए , बारंबार अवतरण (लैण्डिंग) करने की प्रक्रिया में 11 उत्तरजीवितों को प्रतीक्षारत एम्बुलेन्स तक सुरक्षित पहुंचाया ।
3. समुद्र बहुत क्षुब्ध था तथा बहुत तेज हवाएं चल रही थी । ढूंढना , मंडराना तथा उत्तरजीवितों को खींचना बहुत ही पेचीदा तथा थकाने वाला था । यह एक थका देने वाला कार्य था , परन्तु कमांडेन्ट ( क श्रे) पी एस मल्होत्रा ने अपने सह- पायलट के साथ डयूटी के लिए अत्यंत अध्यवसाय , समपर्ण तथा निश्चित मौत के मुँह से चौदह कीमती जीवनों को बचाने में असीम मनोबल व जोश को प्रदर्शित किया । वे पाँच घंटों तक उड़ान भरते रहे तथा अपनी रेंज के अंर्तगत सभी उत्तरजीवितों का बचाव कर लिया । मरीना तट के पास भारतीय तटरक्षक हेलिकॉप्टर द्वारा सामयिक बचाव अभियानों को व्यापक रूप से अखबारों तथा इलैक्ट्रॉनिक मीडिया में प्रदर्शित किया गया जिसकी विश्वभर में सराहना की गयी । 4. कमांडेन्ट (क श्रे) पी एस मल्होत्रा ने साहस , डयूटी के प्रति समर्पण , बेहतर कर्मी- समन्वय , निर्णयात्मकता तथा क्रमबद्धता का अनुकरणीय प्रदर्शन किया , जिसके परिणामस्वरूप समुद्र से चौदह कीमती प्राणों को बचाया तथा छह मृत शवों को ढूंढ निकाला गया । इन्होंने तटरक्षक सेवा की उच्च परंपराओं तथा इसके आदर्श वाक्य `वयं रक्षामः को बनाये रखा है तथा तटरक्षक सेवा के लिए प्रशंसा अर्जित की है । कमांडेन्ट (क श्रे) पी एस मल्होत्रा (0307 - क्यू) को तटरक्षक पदक (शौर्य ) प्रदान करने की कड़ी सिफारिश की जाती है । 893 भारत का राजपत्र, सितम्बर 10, 2005 ( भाद्रपद 19, 1927)

1 . कमल सिंह, उत्तम नाविक ( भंडार सहायक) , वायुयान कर्मी गोताखोर (03278 -जैड) ने 02 अगस्त 1996 को भारतीय तटरक्षक में सेवा शुरू की । 23 जनवरी 2003 को उन्होंने वायुयान कर्मी गोताखोर के रूप में 848 तटरक्षक हेलिकॉप्टर स्क्वाड्रन में कार्य ग्रहण किया । अपने उड़ान अनुभव के खाते में उन्होंने कुल 293: 55 घंटे व्यतीत किए हैं ।
2. 26 दिसम्बर 2004 के दुर्भाग्यपूर्ण , दिवस को, जब घातक सूनामी ने तमिलनाडु तटीय क्षेत्र पर प्रहार किया , तब उत्तम नाविक कमल सिंह, भारतीय तटरक्षक 814 हेलिकॉप्टर की खोज एवं बचाव (एस ए आर) उड़ानों में वायुयान कर्मी गोताखोर के तौर पर तैनात थे । उनको सूनामी लहरों द्वारा क्षुब्ध समुद्र से लोगों को बचाने में आने वाली गंभीर चुनौतियों के लिए सचेष्ट कर दिया गया था । इस प्राकृतिक आपदा का साहस के साथ सामना करने के लिए वे स्वेच्छा से आगे आये तथा अपनी व्यक्तिगत सुरक्षा की परवाह न करते हुए , क्रूर समुद्री लहरों से प्राणियों की रक्षा के लिए वे तैयार हो गये थे ।
3. जैसे ही तटरक्षक हेलिकॉप्टर 814 मरीना बीच के ऊपर पहुँचा , अधिकाधिक उत्तरजीवितों तथा घातक लहरों से हुई त्रासदी के कारण संभावित असहाय लोगों को बचाने के लिए उन्होंने तेजी से कारवाई शुरू कर दी । टूटी नौकाओं तथा कैटामारैन के बहुत से टुकड़े पानी में तैर रहे थे तथा कुछ उत्तरजीवी उन टुकड़ों को पकड़े हुए मदद के लिए पुकार रहे थे । कमल सिंह, उत्तम नाविक को तटरक्षक हेलिकॉप्टर 814 द्वारा , उन क्षतिग्रस्त तथा अपवाहित नौकाओं , जोकि अशांत समुद्र के कारण खतरनाक तरीकों से ऊपर नीचे झूल रही थी , पर तुरन्त उतारा गया । स्वंय को चोट से बचाने के लिए उसने नौकाओं की अधिरचना को उसने दूर धकेल दिया । मुश्किलों के होने पर भी कमल सिंह , उत्तम नाविक क्षतिग्रस्त नौकाओं पर उतरे तथा उत्तरजीवितों के इर्द - गिर्द बचाव पट्टा पहनाने को नियंत्रित किया । उन उत्तरजीवितों को फिर एक - एक करके हेलिकॉप्टर में चढ़ाकर तरंग - रोध के नजदीक उतार दिया । तथापि, हेलिकॉप्टर कमल सिंह , उत्तम नाविक को छोड़ कर चला गया था तथा वह नजदीकी शेष उत्तरजीवितों की देखभाल के लिए क्षुब्ध समुद्र में तैरता रहा । उस समय घायल उत्तरजीवितों द्वारा उसे सख्ती से जकड़े रहना तथा 894

[भाग I-खण्ड 1 उनके साथ स्वंय को डूबने से बचाने के लिए ढीला छोड़ना एक बड़ा मुश्किल कार्य था । यह प्रक्रिया चलती रही तथा कमल सिंह , उत्तम नविक ने विभिन्न क्षतिग्रस्त नौकाओं , जलमग्न कैटामारैन के टुकड़ों तथा तैरते हुए लकड़ी के तख्तों से लगातार ग्यारह असहाय उत्तरजीवितों का बचाव किया ।
4. कमल सिंह, उत्तम नाविक ने अपनी व्यक्तिगत सुरक्षा की परवाह न करते हुए तथा ड्यूटी के लिए अभूतपूर्व निष्ठा के प्रदर्शन द्वारा ग्यारह उत्तरजीवितों को बचाया तथा संकटमय स्थिति के होने पर भी खराब समुद्र में चार शवों को बरामद किया जिससे सेवा ने प्रंशसा अर्जित की । यह समर्पण तथा साहसिक कार्य भारतीय तटरक्षक की उच्च परंपराओं के अनुरूप है । कमल सिंह , उत्तम नाविक (एस ए ) , वायुयान कर्मी गोताखोर (03278 - जैड़ ) को तटरक्षक पदक (शौर्य) प्रदान करने की कड़ी सिफारिश की जाती है ।

मुख्य अग्निशमन अधिकारी श्री अनिल चुड़ामन महाजन अपने स्टाफ सहित तत्काल घटना स्थल की ओर रवाना हुए। घटना स्थल पर पहुंचने पर श्री अनिल चुड़ामन महाजन मुख्य अग्निशमन अधिकारी ने देखा कि चीता वन में भटकने के उपरांत होटल की इमारत में घुस गया है और मच्छर की जाली में फंस गया , और उसकी अगली टांग दरवाजे की चौखट और शटर के बीच फंस गई थी। घटना स्थल पर मौजूद वरिष्ठ अधिकारी चीते को जिंदा पकड़ने की योजना बना रहे थे , इस प्रयोजन हेतु एक बेहोश करने वाली बन्दूक और पिंजरा मंगवाया गया । अचानक चीता फंसी हुई स्थिति से बच निकलने में सफल हो गया और होटल परिसर से बाहर आ गया। आस- पास के क्षेत्र के लोगों के लिए उत्पन्न खतरे का आभास करते हुए श्री अनिल चुड़ामन महाजन और उनके अग्निशमन स्टाफ ने चीते को नायलन की जाली में फंसाने की योजना बनाई । श्री हेमन्त दलवी , एक स्वयंसेवक और श्री महेश सिंह, होटल का एक रसोईया भी उनके दल में शामिल हो गए । जब श्री महाजन अपने दल सहित चीते को फंसाने का प्रयत्न कर रहे थे , तो श्री हेमन्त और श्री महेश को चीते ने पकड़ लिया और उन्हें जख्मी कर दिया । अपनी सुरक्षा की बिल्कुल भी परवाह किए बिना श्री महाजन खाली हाथ ही चीते से भिड़ गए और शरीर पर गंभीर घाव हो जाने के बावजूद भी वे श्री हेमन्त दलवी और श्री महेश सिंह को बचाने में सफल हो गए ।
श्री अनिल चुड़ामन महाजन ने. उत्कृष्ट वीरता , अदम्य साहस, नेतृत्व , व्यवसायिक कुशलता और उच्च कोटि की कर्त्तव्य निष्ठा का प्रदर्शन किया।
यह पुरस्कार वीरता के लिए अग्निशमन सेवा पदक पुरस्कार को शासित करने वाले नियमों के नियम 3 ( 1 ) के तहत प्रदान किये जाते हैं और परिणामतः दिनांक 21.2.2004 से नियम 5 ( क ) के तहत अनुमेय विशेष भत्ता इसके साथ देय है।

दिनांक: 16.06.2004 को 05 बजकर 10 मिनट पर कटक अग्निशमन केंद्र में दूरभाष पर सूचना मिली कि दो व्यक्ति काठजोड़ी नदी, कटक के तेज बहाव में डूब रहे थे।
अग्निशमन केंद्र का एक स्टाफ सदस्य श्री पद्म नावा बेहरा तत्काल घटना स्थल की ओर रवाना हुआ । नदी तट पर पहुंचने पर उसने देखा कि श्री रंजन प्रधान और श्री कालिया मोहराना नदी के बहाव में बहे जा रहे थे। वे दोनों अपने जीवन को बचाने के लिए बुरी तरह संघर्ष कर रहे थे । दर्शकों की भीड़ में से कोई भी पीड़ितों को बचाने का साहस नहीं जुटा पा रहा था। यह भांपते हुए कि उनकी जिंदगी खतरे में है श्री पद्म नावा बेहरा , फायरमैन और श्री विवेकानंद दास, फायरमैन पूरे संतुलन और सूझ- बूझ के साथ तेज बहाव वाली नदी में कूद पड़े । वे दोनों पीड़ितो की बाजूओं को पकड़ने में कामयाब हो गए और नदी के तेज बहाव को चीरते हुए श्री रंजन प्रधान और श्री कालिया मोहराना को सुरक्षित रूप से नदी से बाहर निकालने में सफल हुए ।
अतः श्री पद्म नावा बेहरा ने उत्कृष्ट वीरता, अदम्य साहस, नेतृत्व , व्यावसायिक कुशलता और उच्च कोटि की. कर्त्तव्य- निष्ठा का प्रदर्शन किया ।
यह पुरस्कार , वीरता के लिए अग्निशमन सेवा पदक पुरस्कार की शासित करने वाले नियमों के नियम 3 ( i ) के तहत प्रदान किये जाते हैं और परिणामतः दिनांक 16.6.2004 से नियम 5 (क) के तहत अनुमेय विशेष भत्ता इसके साथ देय है।

अग्नि शमन केन्द्र के दल के सदस्य श्री प्रताप कुमार सिंह , फायरमैन तत्काल ही घटना स्थल की ओर गए । वहां पहुंचने पर उन्होंने देखा कि श्रीमती पुष्पांजली बेहेरा , आयु लगभग 35 वर्ष, एक कुंए में गिर गई थीं और वह अपना जीवन बचाने के लिए संघर्ष कर रही थीं । कुंआ बहुत संकरा था और आस - पास के व्यक्तियों में से किसी ने भी कुंए के अंदर जाकर उसे बचाने का साहस नहीं किया। श्री प्रताप कुमार सिंह, फायरमैन अपनी सुरक्षा की परवाह किए बिना रस्सी की विकसित सीढ़ी के सहारे कुंए में उतर गए । तब उन्होंने श्रीमती पुष्पांजली बेहेरा को अपनी टांगें पकड़ा कर उसे सुरक्षित किया और अग्निशमन दल के अन्य सदस्यों ने उन दोनों को कुंए से सुरक्षित बाहर खींच लिया ।
इस प्रकार श्री प्रताप कुमार सिंह ने उत्कृष्ट वीरता, अदम्य साहस, नेतृत्व, व्यवसायिक कुशलता और उच्च कोटि की कर्तव्य-निष्ठा का प्रदर्शन किया।

दिनांक 4 अगस्त, 2004 को 0640 बजे अथागढ़ अग्निशमन केन्द्र में दूरभाष पर सूचना प्राप्त हुई कि गांव खुंतकता, कटक में एक महिला गहरे कुंए में गिर गई है ।
अग्नि शशमन केन्द्र के दल के सदस्य श्री कान्तु चरण स्वेन , फायरमैन तत्काल ही घटना स्थल की ओर गए । वहां पहुंचने पर उन्होंने देखा कि श्रीमती नामी बेहेरा एक कुंए में गिर गई थीं जो कि जर्जर हालत में था । आस - पास के व्यक्तियों में से कोई भी व्यक्ति उसे बचाने के लिए कुएं के अन्दर जाने को तैयार नहीं था । श्री कान्हु चरण स्वेन, फायरमैन अपनी सुरक्षा की परवाह किए बिना रस्सी की विकसित सीढ़ी के सहारे कुंए में उतर गए और उन्होंने देखा कि श्रीमती नामी बेहेरा बेहोश थीं और कुंए की दीवार के साथ झुकी हुई थीं। सतत प्रयासों और अपने फायरमैन के उच्च कौशल का प्रयोग करके श्री स्वेन ने महिला को अपने कंधों पर बांध लिया और अग्निशमन दल के अन्य सदस्यों ने उन दोनों को कुंए से बाहर खींच लिया । इस कार्य में श्री स्वेन के सिर पर चोंटें आईं ।
इस प्रकार श्री कान्हु चरण स्वेन ने उत्कृष्ट वीरता , अदम्य साहस , नेतृत्व , व्यवसायिक कुशलता और उच्च कोटि कीकर्तव्य-निष्ठाका प्रदर्शन किया ।

दिनांक 27 नवंबर , 2003 को 0205 बजे पुलिस नियंत्रण कक्ष से दूरभाष पर सूचना प्राप्त हुई कि 12 यात्रियों को ले जा रही एक टाटा सूमो ; जानी खुर्द, मेरठ के समीप एक पुल की रेलिंग से टकरा कर गंगा नहर में गिर गई है। अग्नि शमन केन्द्र के दल के सदस्य श्री कंवर पाल , फायरमैन तत्काल ही घटना स्थल की ओर गए । वहां पहुंचने पर उन्होंने देखा कि टाटा सूमो में यात्रा कर रहे 12 यात्री गंगा नहर में अपने बचाव के लिए संघर्ष कर रहे थे । ठंडे मौसम के कारण पूरा क्षेत्र घने कोहरे से ढका हुआ था और बहुत कम दिखाई दे रहा था । श्री कंवर पाल , अपनी सुरक्षा की परवाहं किए बिना अग्निशमन सीढ़ी के सहारे गंगा नहर में उतर गए, तब वे नहर के ठंडे पानी में बड़ी कठिनाई से, बहुत दूर तक तैर कर पीड़ितों तक पहुंचे और नहर से पांच व्यक्तियों को बचाने में सफल हुए ।
इस प्रकार श्री कंवर पाल ने उत्कृष्ट वीरता, अदम्य साहस, नेतृत्व, व्यवसायिक कुशलता और उच्च कोटि की कर्तव्य-निष्ठा का प्रदर्शन किया ।

(i) भारत सरकार का संगठनात्मक ढाँचा ।
(ii ) शासन में आचार संहिता ।
(iii) कार्मिक प्रशासन को नया रूप देना ।
(iv) वित्तीय प्रबन्धन प्रणालियों को सुदृढ़ करना ।
(v) राज्य स्तर पर प्रभावी प्रशासन सुनिश्चित करने संबंधी कदम ।
(vi ) प्रभावी जिला प्रशासन सुनिश्चित करने संबंधी कदम ।
(vii ) स्थानीय स्व-शासन/पंचायती राज्य संस्थाएं ।
(viii ) सामाजिक धन, ट्रस्ट एवं सहभागितापूर्ण लोक सेवा प्रदान करना ।

[भाग I-खण्ड । प्रत्येक शीर्ष के अंतर्गत जाँच किये जाने वाले कुछ मुद्दे इस संकल्प की अनुसूची के रूप में संलग्न विचारार्थ विषयों में दिए गये हैं ।
4. यह आयोग रक्षा , रेलवे , विदेश मामले , सुरक्षा तथा आसूचना प्रशासनों की विस्तृत जाँच के साथ - साथ केन्द्र- राज्य संबंधों , न्यायिक सुधारों आदि विषयों, जिनकी पहले से ही अन्य निकायों द्वारा जाँच की जा रही है , को अपने दायरे से बाहर रख सकता है । तथापि , आयोग सरकार के अथवा इसकी किसी सेवा एजेंसी के तंत्र की पुनर्संरचना की अनुशंसा करते हुए इन क्षेत्रों की समस्याओं को ध्यान में रखने के लिये स्वतंत्र होगा ।
5. आयोग राज्य सरकारों के साथ परामर्श की आवश्यकता पर भी पर्याप्त ध्यान देगा ।
6. आयोग अपनी कार्यप्रणाली ( आयोग द्वारा समुचित समझे जाने पर राज्य सरकारों के साथ परामर्श सहित ) स्वयं तय करेगा और अपनी सहायता हेतु समितियाँ , परामर्शदाता/ सलाहकार नियुक्त करेगा । आयोग विषय से संबंधित उपलब्ध मौजूदा सामग्री तथा रिपोर्टों पर विचार करेगा और सभी मुद्दों पर नये सिरे से सोचने की अपेक्षा उन्हीं के अनुसार आगे कार्रवाई करने हेतु विचार करेगा ।
7 . भारत सरकार के मंत्रालय एवं विभाग आयोग द्वारा अपेक्षित सूचना और दस्तावेज तथा अन्य सहायता प्रदान करेंगे । भारत सरकार को आशा है कि राज्य सरकारें तथा अन्य सभी संबंधित आयोग को अपना पूरा सहयोग और सहायता प्रदान करेंगे ।
8. आयोग अपनी रिपोर्ट (रिपोर्ट) कार्मिक, लोक शिकायत तथा पेंशन मंत्रालय , भारत सरकार को इसके गठन के एक वर्ष के अन्दर प्रस्तुत करेगा ।

आदेश दिया जाता है कि इस संकल्प की एक प्रति भारत सरकार के सभी मंत्रालयों और विभागों , राज्य सरकारों , संघ शासित प्रदेशों के प्रशासनों आदि को प्रेषित की जाए ।

3. कार्मिक प्रशासन को बेहतर बनाना
3.1 सिविल सेवाओं में सभी भर्ती के तरीकों, प्रशिक्षण और नियुक्ति में संबंधित नीति का पुनरीक्षण एवं यथा आवश्यक परिवर्तनों को सुझाना ।
3.2 सिविल सेवकों के कार्य निष्पादन को बढ़ाने एवं इसके मूल्यांकन के लिए दिशा- निर्देश उपलब्ध कराना ।
3.3 कैरियर उन्नयन, प्रेरणा और उत्पादकतावृद्धि पर संवर्ग प्रबन्धन के तरीकों को बेहतर बनाना । 3.4 सिविल सेवकों और प्रशासनिक संवर्गों की दक्षता और सक्षमताओं को अद्यतन बनाने हेतु नीतियाँ बनाना तथा क्षमता निर्माण के लिये समुचित मध्यस्थताएं करना ।

4. वित्तीय प्रबन्धन पद्धतियों को सुदृढ़ करना
4.1 कार्यक्रमों/ परियोजनाओं के लिये निधियों का सरल प्रवाह, लेखाओं के उचित रखरखाव तथा इस उद्देश्य के लिये आवश्यक सूचना/दस्तावेज समय से प्रस्तुत करना सुनिश्चित करने के लिये शासन के सभी स्तरों पर वित्तीय प्रबन्धन पद्धतियों में क्षमता निर्माण करना । 4.2 जिन उद्देश्यों/ नतीजों के लिये निधियाँ प्रदान की गई हैं, उनका उन्हीं के लिये समुचित उपयोग सुनिश्चित करने के लिए आंतरिक लेखा परीक्षा पद्धति सुदृढ़ करना और इस बात की जाँच करना कि सुपुर्दगी की इकाई लागत इस उद्देश्य के लिए विकसित किये गये बैंचमार्क के मुताबिक है ।
4.3 कार्यक्रमों के प्रदान एवं प्रभाव के बाह्य लेखा परीक्षा एवं आकलन की एक संस्थानिक पद्धति ।
5. राज्य स्तर पर प्रभावी प्रशासन सुनिश्चित करने के लिए उपाय
5.1 राज्यों द्वारा अवधारित परिणाम प्राप्त करने के लिए राज्य प्रशासन में समुचित परिवर्तनों को प्रेरित एवं संवर्धित करना ।
5.1.1 उद्देश्यों की प्राप्ति के लिये राज्य प्रशासन में अपेक्षित परिवर्तन लाना ।
6. प्रभावी जिला प्रशासन सुनिश्चित करने के लिए उपाय
6.1 में जनसाधारण के लिए निम्नतम स्तर पर सेवाओं के विनियमन सुगमीकरण और प्रदानीकरण जिला प्रशासन की केन्द्रीयता को ध्यान में रखते हुए , उसके रूप और अन्तर्वस्तु में प्रगामी आधुनिकीकरण और रूपान्तरण लाना ।
6.2 उत्साह और उत्तरदायित्व से ओत- प्रोत करने और उसे बनाए रखने के लिए प्रक्रियात्मक परिवर्तन लाना ।
6.3 लोक शिकायतों पर कार्रवाई करने और उनके निवारण तंत्र को सुप्रवाही बनाना और उसे व्यापक एवं सुगम बनाना ।
6.4 स्थानीय स्तरों पर कार्यों और संसाधनों का अधिक अंतरण एवं प्रत्यायोजन । 908

8.1 सरकारी प्रभावकारिता को बढ़ाने के एक उपकरण के रूप में सरकार के सभी स्तरों पर सामाजिक पूंजी के निवेश और संवर्द्धन के उपाय करना ।
8.2 स्थानीय समुदाय को विशेष रूप से दूरदराज के क्षेत्रों में सक्रिय रूप से साझीदार बनाने के लिये प्रशासन की क्षमता में सुधार करना और उसे सुदृढ़ बनाना ।

CITATION
1. Commandant Ali Muttaher ( 0104-L) joined the Indian Coast Guard on 17 August 1984. He took over the command of Indian Coast Guard Ship Vivek on 27 April 2004. The officer is the first ever Commercial Pilot Licence (CPL) entry aviator to command an Offshore Patrol Vessel (OPV) . The professionalism , dynamism and his ability to motivate the men have transformed the ship into a well-knitafloat unit.
2. On26 December 2004 , while the ship was on routine Exclusive Economic Zone ( EEZ) patrol , east of North Andaman Islands , she experienced heavy vibrations . It was later confirmed that the vibrations were due to the earthquake , which rocked South- East Asia with its epicentre lying at 160 kms , west of Sumatra . The magnitude of the quake was measured as 9.0 on the Richter scale . The ship was immediateiy directed for relief operations in the Great Nicobar Island . On passage , the ship received an order to evacuate stranded police personnel from a police post in the Interview Island. Only four injured police personnel were evacuated , as the others were left to man the post alongwith arms and ammunitions . In the wee hours of 27 December 2004, the ship entered Port Blair harbour and left at 1800 hrs on the same day with 12.5 tonnes of dry provisions , 12 KL of kerosene and other relief material embarked. for Campbell Bay as large scale destructions and loss of life was reported in the GreatNicobar Islands .
3. Onits wayto Campbell Bay , the ship launched a helicopter for aerial recce off Chowra Islands . The helicopter reported large scale casualties . The ship immediately air dropped its medical team in the Islands . The survivors reported approximately 800 inhabitants either dead or missing . critically injured people to the ship . Rest of the population were provided with 200 kg rice, 100 kg dal and cooked food for two days . The casualties were evacuated to the Car Nicobar Island for onward transportation to Port - Blair. The ship reached Campbell Bay on PM 29 December 04. The first evacuation operation started from Chingam Basti where the ship's helicopter and boat evacuated 67 stranded locals. The ship anchored in the South Bay so that maximum casualties could be evacuated in minimum time . The ship's boat and helicopter were used extensively throughout for evacuations. The day ended with an evacuation of 81 survivors .
4. On 01 January 2005 , the ship's helicopter, whilst returning after an evacuation operation , reported the presence of an Indonesian boat off Campbell Bay . The state of boat and the crew were reported to be unhealthy. They were: presumed to be adrift and as the helicopter operations were not possible after the sunset , the Commanding Officer took the decision to send the ship's boat for the rescue of the Indonesian crew . Under the guidance of the Commanding Officer , the operation continued throughout the night and twelve precious lives were saved.
5. The uncertainty of the scenario and adverse weather conditions tested the officer's calibre as Commanding Officer to the hilt , in which the ship undertook the herculean task of embarkation and distribution of relief material and evacuation of valuable lives in a professional manner . In recognition of the excellent command and control , focused goal orientation and tremendous leadership qualities , Commandant Ali Muttaher (0104- L) is strongly recommended for Tatrakshak Medal ( Gallantry) .

1. CommandantMilind Manohar Patil ( 0252- V) joined the Indian Coast Guard on 14 January 1990. He took over as the Coast Guard District Commander Nicobar( COMDIS - 10 ) on 22 May 2004.
2. On26December 2004 as Campbell Bay experienced an earthquake of 8.9 on the Richter scale which resulted in the " Tsunami " effect causing unprecedented shock and large scale damage to the Great Nicobar Islands , CommandantMM Patil rushed to confirm that all the Indian Coast Guard families staying at the married accommodation complex at Lai Tekri were safe . Immediately thereafter , he rushed to the Other Than Married (OTM ) complex alongwith the personnel living in married accommodation to check their status . Onconfirming the safety of inliving enrolled personnel , Commandant MM Patil focused his attention on the Campbeil Bay town .
3. Heformed a disaster management team consisting of all local heads of the Govt organisations . Since the Campbell Bay jetty was totally destroyed , he rushed to check the state of the runway . The airfield was the only link between theCampbell Bay and the outside world . Though the tsunami had brought large quantity of debris on the runway , he assessed that the runway was still useable. The officer with help of the Military Engineering Service (MES ) got down to the task of clearing the runway. It was due to his foresight and determination the runway was made operational in record time resulting in the Campbell Bay receiving relief material in large quantities.

prepared for evacuation of injured persons from various places . He worked tirelessly to collect all possible resources to set up camps and to ensure the much sought after relief to the injured and affected persons. Hewas instrumental in pulling a number of vehicles and boats , out of the rubble and making them operational . He ensured that all the evacuated persons , as many as 3000 , were given first meal and medical aid in the improvised camps set up by the India Coast Guard . Understanding the importance of teamwork in such eventualities , Commandant MM Patil managed the unstinted support of the other components of the Andaman and Nicobar Command as well as , the other civil organisations . He provided logistic support to the Directorate Shipping Services ship MV Katchal and deployed her to Little Nicobar Islands group from where the ship evacuated approximately 200 survivors. He deputed a team of Military Engineering Service , Andaman Public Works Department and Andaman and Lakshadweep Harbour Works reps to survey the road leading to Indira Point for assessing the feasibility of undertaking temporary repairs . Within a few days , he was overseeing the administratiori of fourteen relief camps accommodating as many as 5800 persons . The officer ensured the requisite co- ordination amongst various agencies so that the relief operations had achieved the desired results. His able leadership resulted in the successful evacuation of thousands of stranded personnel and in saving valuable lives .
5. The District Headquarters ( Nicobar ) OTM complex being in low- lying area was totally devastated due to Tsunami . With excellent public relations Commandant MM Patil managed to get 9.18 hectare of revenue land allotted to the Indian Coast Guard from civil administration for relocation of the District Headquarters Nicobar OTM complex . The timely action initiated by him helped the Indian Coast Guard to restore functioning of the District Headquarters.
6. In view of the extraordinary display of courage in face of enormous danger , level-headed thinking under extremely stressful conditions , high leadership qualities and efficient resources management, Commandant Milind Manohar Patil (0252-V) is strongly recommended for the award of Tatrakshak Medal (Gallantry) .

CITATION
1. Commandant( JG ) PS Malhotra (0305-S) joined the Indian Coast Guard on 11 January 1992. He joined 848 Coast Guard helicopter Squadron as staff pilot on 28 June 2002. He has a flying experience of 2265 hrs and is a fully operational pilot on Chetak helicopter with the highest possible rating (Master Green). The officer is an efficient flier who has been instrumental in the squadron meeting all its operational and assigned tasks.
2. On the fateful day of 26 December 2004 at 0845 hrs , a Search and Rescue ( $AR) call was received by the Coast Guard Air Station (Chennai ), indicating that high waves had engulfed the Marina beach . Reacting with alacrity , Commarıdant (JG ) PS Malhotra rushed to the aircraft and got airborne instantaneously alongwith a co - pilot and two aircrew divers in no time . At Marina beach, the pilots witnessed the unprecedented havoc , which the Tsunami waves had created. Boats , hutments and personal belongings were all tossed up and down . Realising the gravity of the situation , Commandant ( JG ) Malhotra swung into action and initiated a concerted search pattern over the sea . Soon the helicopter located a man floating on a small plank, frantically waving for help . They promptly lowered the free diver near the drowning man . The diver secured the man with the rescue strop and winched him up into the aircraft. The person was immediately evacuated for medical aid . The search was resumed and two more fishermen were spotted struggling to survive by holding a sinking boat off Royapuram fishing harbour. The fishermen were winched up and evacuated to the nearest landing place , a narrow fishing jetty . The aircraft was low on fuel by then and the pilots decided to return to the base. Without wasting any time Commandant( JG ) PS Malhotra got another helicopter ready and within five minutes got airborne . sea and handed over to the persons standing on the breakwater with an ambulance . The breakwater was quite narrow, but displaying precision and utmost flying skills , they landed on it repeatedly till all 11 survivors were handed over safely to the awaiting ambulance .
3. The sea was very choppy and the winds were strong . Locating , hovering and winching up the survivors were tricky and tiring . It was a gruelling task but Commandant ( JG ) Malhotra , exhibited utmost perseverance , devotion to duty and immense moral courage in rescuing fourteen precious human lives from the clutches of certain death . They flew continuously for five hours and saved all the survivors within their range . The rescue operations carried out by the Indian CoastGuard helicopters off Marina beach were given extensive media coverage and lauded the world over .
4. Commandant (JG ) PS Malhotra exhibited exemplary bravery, devotion to duty, supreme crew co - ordination and decisiveness, which resulted in saving fourteen precious lives at sea and retrieval of six dead bodies from the sea. He has upheld the high traditions of the Indian Coast Guard service and its motto , "We Protect" and has brought immense laurels to the Indian Coast Guard Service . helicopter He was briefed about the grave challenges of rescuing people from the choppy seas caused by the Tsunami waves . He willingly came forward to fight the natural disaster and was ready to save lives of the people in distress , unmindful of his personal safety.
3. Over the Marina beach , many pieces of broken boats and catamaran were seen floating and few survivors were found holding on to these pieces , waving for help . Kamal Singh , Uttam Navik, was immediately lowered by the Coast Guard Helicopter 814 on these damaged and drifting boats , which were tossing and swinging dangerously due to the rough seas. He had to push the boat's superstructure away from him , repeatedly to protect himself from getting hurt . Braving the difficulties , Kamal Singh , Uttam Navik, landed on the damaged boats and managed to put on the rescue strop around the survivors . These survivors were picked up and dropped at the nearest breakwater . However, Kamal Singh , Uttam Navik , was left behind to take care of the remaining survivors . It was a very difficult task as the panic stricken survivors , at times , held him so tight that he had to break loose to prevent himself from drowning along with them . This process continued and Kamal Singh, Uttam Navik , rescued eleven stranded survivors from various damaged boats , submerged catamaran pieces and floating wooden planks in quick succession .
4. Kamal Singh , Uttam Navik , has displayed exceptional devotion to duty and without caring for his personal safety , saved eleven survivors and recovered four dead bodles in the rough sea, thereby bringing laurels to the service . This dedication and courageous act is in conformity with the highest traditions of the Indian Coast Guard . On 2 February , 2004 at 0645 hours a call was received at Nashik Fire Station informing that a leopard has entered the premises of Hotel Rasoi at . Nashik .
Immediately Shri Anil Chudaman Mahajan , Chief Fire Officer alongwith his staff rushed to the spot. On arrival Shri Anil Chudaman Mahajan , Chief Fire Officer , noticed that a leopard after straying in the forest entered into the hotel building and got entangled in the mosquito net with its front leg trapped between the doorframe and shutter . Senior officers present at the site were planning to trap the leopard alive for this purpose a tranquilizer gun and cage were asked for . All of a sudden the leopard succeeded to escape from trapped position and came out of the hotel premises. Sensing danger to the people in the vicinity Shri Anil Chudaman Mahajan and his fire staff planned to trap the leopard under a nylon net . Shri Hemant Dalvi , a volunteer and Shri Mahesh Singh , a cook in the hotel also joined the team . When Shri Mahajan with his team was trying to trap the leopard, Shri Hemant and Shri Mahesh were caught by the leopard and were wounded . With total disregard to his own safety Sh . Mahajan grappled with the leopard bare handed and inspite of grave injuries on his body he managed to save the life of Shri Hemant Dalvi and Shri Mahesh Singh .
Shri Anil Chudaman Mahajan thus , displayed conspicuous gallantry, exemplary courage , leadership , professional expertise and devotion to duty of the highest order .

They managed to get hold of the arms of both the victims and after struggling in the strong currents of the river succeeded in bringing Shri Ranjan Pradhan and Shri Kalia Moharana safely out ofthe river .

Pushpanjali Behera , aged about 35 years , had fallen into a well and was struggling for survival . The well was very narrow and none of the persons in the vicinity dared to enter the well to rescue her . Without regard to his personal safety , Shri Pratap Kumar Singh , Fireman , entered into the well with the help of a improvised rope ladder . He, then secured Smt . Pushpanjali Behera with his legs both of them were pulled out by the other members of the fire crew safely .

On14th August, 2004 at 0640 hours a call was received at Athagarh Fire Station informing that a lady has fallen inside a deep well at village Khuntakata, Cuttack .
Immediately Shri Kanhu Charan Swain , Fireman , a crewmember of the Fire Station , rushed to the spot . On arrival he found that Smt . Nami Behera had fallen into a well , which was in a dilapidated condition . None of the persons in the vicinity was ready to enter the well to rescue her . Shri Kanhu Charan Swain , Fireman without regard to his life , descended into the well with the help of improvised ladder rope and observed that Smt . Nami Behera was in a unconscious state and was leaning against the wall of the well . After protracted efforts and using his fireman skills of a high order , Shri Swain managed to tie her onto his shoulders and both of them were pulled out by the other members of the fire crew. Shri Swain sustained injuries to his head .

On arrival he noticed that 12 passengers traveling in Tata Sumo were struggling for survival in the Ganga canal . Due to cold weather , the area was covered with dense fog and visibility was extremely poor. Without disregard to his personal safety , Shri Kanwar Pal descended into Ganga Canal with the help of a fire brigade ladder, he then swam a long distance to reach the victims after struggling in the cold water of the canal , succeeded in rescuing five persons from the Canal .

No. K-11022/9 /2004-- RC. ThePresident is pleased to set up a Commission of Inquiry to be called the second Administrative Reforms Commission (ARC) to prepare a detailed blueprint for revamping the public administration system .

3. The Commission will suggest measures to achieve a proactive , responsive , accountable , sustainable and efficient administration for the country at all levels of the government . The Commission will , inter alia , consider the following :

(iii ). Refurbishing of Personnel Administration
(iv) Strengthening of Financial Management Systems
v) Steps to ensure effective administration at the State level
( (vi ) Steps to ensure effective District Administration

Some of the issues to be examined under each head are given in the Terms of Reference attached as a Schedule to this Resolution .
4. The Commission may exclude from its purview the detailed examination of administration of Defence , Railways , External Affairs , Security and Intelligence , as also subjects such as Centre- State relations , judicial reforms etc. which are already being examined by other bodies . The Commission will , however , be free to take the problems of these sectors into account in recommending re- organisation of the machinery of the Government or of any of its service agencies .
5. The Commission will give due consideration to the need for consultation with the State Governments .
6. The Commission will devise its own procedures ( including for consultations with the State Government as may be considered appropriate by the Commission ) , and may appoint committees, consuitants/ advisers to assist it . The Commission may take into account the existing material and reports available on the subject and consider building upon the same rather than attempting to address all the issues ab initio .

of India trusts that the State Governments and all others concerned will extend their fullest cooperation and assistance to the Commission .
8. The Commission will furnish its report (s ) to the Ministry of Personnel , Public Grievances & Pensions , Government of India , within one year of its constitution .

TERMS OF REFERENCE FOR THE SECOND ADMINISTRATIVE REFORMS COMMISSION
1. Organizational Structure of the Government of India
1.1 Reorganization of Ministries and Departments
1.1.1 Revisiting and redefining the role of the Ministries and Departments in the context of evolving role of governance and need for greater collaboration .
1.2 Manpower planning and Process re - engineering .
1.3 Suggest ways to position the administrative services in the modern context of global integration , emergence of markets and liberalisation .
1.4 To examine if the present system of governance is optimally suited to the environment of the times
1.4.1 To suggest a framework for possible areas where there is need for governmental regulation (regulators) and those where it should be reduced.
1.4.2 To strengthen the framework for efficient , economical, sensitive , clean , objective and agile administrative machinery.
2. Ethics in Governance
2.1 Vigilance and Corruption:
2.1.1 Strengthening pro - active vigilance to eliminate corruption and harassment to honest civil servants including , wherever necessary , limiting executive discretion .
2.1.2 Addressing systemic deficiencies manifesting in reluctance to punish the corrupt .

2.2.1 To suggest improvements in the institutional arrangements for smooth , efficient and harmonious relationship between civil service and the political executive . :

3. Refurbishing of Personnel Administration
3.1 Review the policy relating to, and all methods of recruitment , training and placement and suggest changes , ifrequired.

3.3 . Improved methods of cadre management focussing on career progression , motivation and productivity enhancement .
3.4 Strategies for up - gradation of skills and competencies of civil servants and administrative cadres and appropriate interventions for capacity building .
3.5 Linking of performance of Civil Servants and Government personnel to social and economic objectives and outcomes..
4. Strengthening Financial Management Systems
4.1 Capacity building in financial management systems at all levels of Governance , to ensure smooth flow of 940

funds for programmes/ projects , proper maintenance of accounts and timely furnishing of necessary information/ documents for this purpose .
4.2 Strengthening of internal audit systems, to ensure proper utilisation of funds for the purposes/outcomes for which they have been provided , and checking that unit cost of delivery/outcome is as per benchmark developed for this purpose.
4.3 An institutionalised method of external audit and assessment of the delivery and impact of programmes .
5. Steps to ensure effective administration at the State level
5.1 To encourage and promote appropriate changes in State Administration in the governance of the States to achieve envisaged outcomes .

6. Steps to ensure effective District Administration
6.1 Progressive modernization and transformation of district administration in form and content keeping in mind the centrality thereof in regulating , facilitating and delivering services at the grass - root level .

6.3 Streamlining and fine-tuning a comprehensive and accessible public grievance handling and redress mechanism .
