सं. यू.16/53/2001 चि.2/( केरल ) कर्मचारी राज्य बीमा ( साधारण ) विनियम , 1950 के विनियम 105 के अधीन निगम की शक्तियां महानिदेशक को प्रदान करने के संबंध में कर्मचारी राज्य बीमा निगम की दिनांक 25 अप्रैल, 1951 को हुई बैठक में पारित किए गए संकल्प के अनुसरण में तथा महानिदेशक के आदेश संख्या 1024 ( जी ) दिनांक 23.5.1983 द्वारा ये शक्तियां आगे मुझे सौंपी जाने पर मैं इसके द्वारा निम्नलिखित डॉक्टर को मानकों के अनुसार देय पारिश्रमिक पर निम्नलिखित तिथि तक या पूर्णकालिक चिकित्सा निर्देशी के कार्यग्रहण करने तक, जो भी पूर्व हो, को राज्य चिकित्सा आयुक्त ( केरल ) द्वारा निर्धारित क्षेत्र के लिए बीमाकृत व्यक्तियों की स्वास्थ्य परीक्षा करने तथा मूल प्रमाण पत्र की सत्यता संदिग्ध होने पर उन्हें आगे प्रमाण पत्र जारी करने के प्रयोजन के लिए चिकित्सा अधिकारी के रूप में कार्य करने के लिए प्राधिकृत करती हूँ :

सं. एन-15 /13 /11 / 1/2007- यो. एवं वि. कर्मचारी राज्य बीमा ( सामान्य ) विनियम 1950 के विनियम 95- क के साथ पठित कर्मचारी राज्य बीमा अधिनियम, 1948 ( 1948 का 34) की धारा 46 ( 2 ) द्वारा प्रदत्त शक्तियों के अनुसरण में महानिदेशक महोदय ने 16 मार्च , 2007 ऐसी तारीख के रूप में निश्चित की है जिससे उक्त विनियम 95 - क तथा पंजाब कर्मचारी राज्य बीमा ( चिकित्या हितलाभ ) नियम 1955 में निर्दिष्ट चिकित्सा हितलाभ पंजाब राज्य में निम्नलिखित क्षेत्रों में बीमांकित व्यक्तियों के परिवारों पर लागू किए जाएंगे , अर्थात : --
1. " नगर निगम, अमृतसर की सीमाओं के अन्तर्गत आने वाले सभी क्षेत्र सिवाय उनके जिन पर कर्मचारी राज्य बीमा योजना पहले से लागू है। "
2. " नगर निगम , जालन्धर की सीमाओं के अन्तर्गत आने वाले सभी क्षेत्र निवाय उनके जिन पर कर्मचारी राज्य बीमा योजना पहले से लागू है। "
3. " नगर पालिका, पठानकोट की सीमाओं के अन्तर्गत आने वाले सभी क्षेत्र सिवाय उनके जिन पर कर्मचारी राज्य बीमा योजना पहले से लागू है। "
4. " नगर पालिका, कपूरथला की सीमाओं के अन्तर्गत आने वाले सभी क्षेत्र सिवाय उनके जिन पर कर्मचारी राज्य बीमा योजना पहले से लागू है। "
5. " नगर पालिका, मोगा की सीमाओं के अन्तर्गत आने वाले सभी क्षेत्र सिवाय उनके जिन पर कर्मचारी राज्य बीमा योजना पहले से लागू है । "
6. " नगर पालिका, होशियारपुर की सीमाओं के अन्तर्गत आने वाले सभी क्षेत्र सिवाय उनके जिन पर कर्मचारी राज्य बीमा योजना पहले से लागू है । "

संयुक्त निदेशक ( यो. एवं वि. ) सं.नि-15/ 13/1/6/2006-यो. एवं वि.कर्मचारी राज्य बीमा ( सामान्य ) विन्नियम 1950 के विनियम 95- क के साथ पठित कर्मचारी राज्य बीमा अधिनियम, 1948 ( 1948 का 34 ) की धारा -46 ( 2 ) द्वारा प्रदत्त शक्तियों के अनुसरण में महानिदेशक ने 01 फरवरी , 2007 ऐसी तारीख के रूप में निश्चित की है जिससे उक्त विनियम 95 - क तथा आन्ध्र प्रदेश कर्मचारी राज्य बीमा नियम-1955 में निर्दिष्ट चिकित्सा हितलाभ आन्ध्र प्रदेश राज्य में निम्नलिखित क्षेत्रों में बीमांकित व्यक्तियों के परिवारों पर लागू किए जाएंगे , अर्थात :--
आन्ध्र प्रदेश राज्य के महबूबनगर जिले के " कल्वकुर्थि " मण्डल में स्थित " कल्वकुर्थि" राजस्व गांव के सीमा के अन्तर्गत सभी क्षेत्र ।

II (Kerala)-In Pursuance of the resolution passed by ESI Corporation at its meeting held on 25.4.1951 conferring upon the Director General the powers of the Corporation under Regulation 105 of the ESI (General) Regulation, 1950 and such powers further delegated to me vide Director General's Order No. 1024(G) dated 23.5.1983 , I hereby authorize the following doctor to function as Medical Authority at a monthly remuneration in accordance with the norms w.e.f. the date given below for one year or till a full time Medical Referee joins , whichever is earlier, for centres as stated below for areas to be allocated by State Medical Commissioner , Kerala for the purpose of medical examination of the insured person and grant of further certificates to them when the correctness of the original certificate is in doubt .

No. N- 15/ 13/ 11/ 1 /2007 -P &D-In Pursuance of powers conferred by Section 46(2) of the Employees ' State Insurance Act , 1948 (34 of 1948 ) , read with Regulation 95 -A of the Employees' State Insurance (General ) Regulations , 1950 , the Director General has fixed the 16th March 2007 as the date from which the medical benefits as laid down in the said Regulation 95 - A and the Punjab Employees' State Insurance (Medical Benefit ) Rules , 1955 shall be extended to the families of insured persons in the following area in the State of Punjab namely:-
1. "Areas within the limits of Municipal Corporation Amritsar except the areas already notifed under ESI Scheme , "
2. "Areas within the limits of Municipal Corporation Jalandhar except the areas already notifed under ESI Scheme , "
3. " Areas within the limits ofMunicipal Committee Pathankot except the areas already notifed under ESI Scheme, "
4. "Areas within the limits of Municipal Committee Kapurthala except the areas already notifed under ESI Scheme , "
5. "Areas within the limits of Municipal Committee Moga except the areas already notifed under ESI Scheme, "
6. "Areas within the limits of Municipal Committee Hoshiarpur except the areas already notifed under ESI Scheme , "

[PART III-SEC. 4] No. N- 15 / 13 / 1 /6/2006 -P& D-In Pursuance of powers conferred by Section 46(2) of the Employees ' State Insurance Act , 1948 (34 of 1948 ) , read with Regulation 95 -A of the Employees' State Insurance (General ) Regulations , 1950, the Director General has fixed the 1st February, 2007 as the date from which the medical benefits as laid down in the said Regulation 95 - A and the Andhra Pradesh Employees ' State Insurance (Medical Benefit ) Rules , 1955 shall be extended to the families of insured persons in the following area in the State of Andhra Pradesh namely:-
"All the areas falling within the limits of Revenue Village of ' Kalwakurthy' of Kalwakurthy Mandal in Mahaboobnagar District ofAndhra Pradesh . "
