सं. 15/यू/11/पीटीएमआर नियुक्ति/एसएमसी/08 कर्मचारी राज्य बीमा ( साधारण ) विनियम, 1950 के विनियम 105 के अधीन निगम की शक्तियां महानिदेशक को प्रदान करने के संबंध में कर्मचारी राज्य बीमा निगम की दिनांक 25 अप्रैल, 1951 को हुई बैठक में पारित किए गए संकल्प के अनुसरण में मुख्यालय के पत्र संख्या Pt . File No. U- 13 / 12/ 13/ 2005 - med.1 /PTMR dated 09.09.2008 द्वारा ये शक्तियां आगे मुझे सौंपी जाने पर मैं , इसके द्वारा निम्नलिखित डॉक्टरों को मानकों के अनुसार देय पारिश्रमिक पर निम्नलिखित तिथि तक ( एक वर्ष के लिए ) या पूर्णकालिक चिकित्सा निर्देशी के कार्यग्रहण करने तक, जो भी पूर्व हो , को राज्य चिकित्सा आयुक्त, जयपुर ( राजस्थान ) द्वारा निर्धारित क्षेत्र के लिए बीमाकृत व्यक्तियों की स्वास्थ्य परीक्षा करने तथा मूल प्रमाण पत्र की सत्यता संदिग्ध होने पर उन्हें आगे प्रमाण पत्र जारी करने के प्रयोजन के लिए अंशकालीन चिकित्सा निर्देशी के रूप में कार्य करने के लिए प्राधिकृत करती हूं:--

No. 15 /U / 11 /PTMR - APP ./SMC/ 08 in pursuance of the resolution passed by ESI Corporation at its meeting held on 25-04-1951 conferring upon the Director General the powers of the Corporation under regulation 105 of the ESI (General ) regulation, 1950 and such powers further delegated to me vide H.Q. Letter No. pt File No. U- 13/ 12/ 13 /2005 - Med - 1 /PTMR dated 09-09-08 . I hereby authorize the following doctors to function as Medical Referee at a monthly remuneration in according with the norms w.e.f. the date given below for one year or till a full time Medical Referee joins , whichever is earlier , for centers as stated below for areas to be allocated by State Medical Commissioner, Jaipur (Rajasthan) for the purpose of medical examination of the insured person and grant of further certificates to them when the correctness of the original certificates is in doubt .
