संं. एन-15/13/7/1/2016/यो.एवं.वि.--कर्मचारी राज्य बीमा (सामान्य) विनियम-1950 के विनियम 95-क के साथ पठित कर्मचारी राज्य बीमा अधिनियम 1948, (1948 का 34) की धारा-46 (2) द्वारा प्रदत्त शक्तियों के अनुसरण में महानिदेशक ने 01 मई, 2016 ऐसी तारीख के रूप में निश्चित की है जिससे उक्त विनियम-95 क तथा कर्नाटक कर्मचारी राज्य बीमा नियम-1958 में निर्दिष्ट चिकित्सा हितलाभ कर्नाटक राज्य में निम्नलिखित क्षेत्रों में बीमांकित व्यक्तियों के परिवारों पर लागू किये जाएंगे, अर्थात्‌ :--

संं. एन-16/1/छत्तीसगढ़/2016/यो.एवं.वि.--कर्मचारी राज्य बीमा (सामान्य) विनियम-1950 के विनियम 95-क के साथ पठित कर्मचारी राज्य बीमा अधिनियम 1948, (1948 का 34) की धारा-46 (2) द्वारा प्रदत्त शक्तियों के अनुसरण में महानिदेशक ने 01 जून, 2016 ऐसी तारीख के रूप में निश्चित की है जिससे उक्त विनियम-95 क तथा छत्तीसगढ़ कर्मचारी राज्य बीमा नियम-1954 में निर्दिष्ट चिकित्सा हितलाभ छत्तीसगढ़ राज्य में निम्नलिखित क्षेत्रों में बीमांकित व्यक्तियों के परिवारों पर लागू किये जाएंगे, अर्थात्‌ :--

सं. एन-15/13/28/1/2016/यो.एवं.वि.--कर्मचारी राज्य बीमा निगम (सामान्य) विनियम-1950 के विनियम 95-क के साथ पठित कर्मचारी राज्य बीमा अधिनियम 1948, (1948 का 34) की धारा-46 (2) द्वारा प्रदत्त शक्तियों के अनुसरण में महानिदेशक ने 01 जून, 2016 ऐसी तारीख के रूप में निश्चित की है जिससे उक्त विनियम-95 क तथा तेंलगाना कर्मचारी राज्य बीमा नियम-1955 में निर्दिष्ट चिकित्सा हितलाभ तेंलगाना राज्य में निम्नलिखित क्षेत्रों में बीमांकित व्यक्तियों के परिवारों पर लागू किये जाएंगे, अर्थात्‌ :--

''समय-समय पर कार्यान्वित किये गए तेलंगाना राज्य के क्षेत्रों को छोड़कर, तेलंगाना राज्य के रंगा रेड्‌डी ज़िले राजस्व गांवों के सभी क्षेत्र ।''

"All the Revenue Villages in Haridawar and Udham Singh nagar district."
