सं. 37/जी/पी.टी.एम.आर./एसएमओ/2015-कर्मचारी राज्य बीमा निगम (साधारण) विनियम 1950 के विनियम 105 के अधीन की शक्तियां महानिदेशक को प्रदान करने के संबंध में कर्मचारी राज्य बीमा निगम की दिनांक 25 अप्रैल 1951 को हुई बैठक में पारित किए गए संकल्प के अनुसरण में तथा महानिदेशक के आदेश संख्या पी टी फाइल यू/1/18/16/एस.एम.सी/2009/मैड 1-दिनांक 2018/01/24 द्वारा ये शक्तियां आगे मुझे सौंपी जाने पर मैं इसके द्वारा निम्नलिखित डॉक्टर को मानकों के अनुसार देय परिश्रमिक पर निम्नलिखित तिथि पर एक वर्ष के लिए या पूर्णकालिक चिकित्सा निर्देश के कार्यग्रहण करने तक जो भी पूर्व हो, को हस्ताक्षरित द्वारा निम्नलिखित क्षेत्र के लिए बीमाकृत व्यक्तियों की स्वास्थ्य परीक्षण करने तथा मूल प्रमाणमत्र की सत्यता संदिग्ध होने पर उन्हें आगे प्रमाणपत्र जारी करने के प्रयोजन के लिए चिकित्सा अधिकारी के रूप में कार्य करने के लिए प्राधिकृत करता हूं।

No. 37/G/PTMR/SMO/2015—In Pursuance of resolution passed by ESI Corporation at its meeting held on 25.04.1951 conferring upon the Director General the power of Corporation under regulation 105 of the ESI (General) Regulation, 1950 and such power further delegated to me vide Director General order on file No. 16/18/1/SMC/2009/Med 1 dated 24-01-2018. I hereby authorize the following doctor to function as Medical Authority at a monthly remuneration in accordance with norms. The date given below for one year or till a full time Medical Referee joins, whichever is earlier, for centers as stated below for areas allocated by undersigned, for the purpose of medical examination of insured persons and grant of further certification to them, when the correctness of the original certificate is in doubt.
