No. MOH/B&D/2023/6177.—In pursuance of the D.O No. 02/04/2011-VS(MCCD), Dated 5th April, 2023 issued by the Registrar General -cum-Commissioner Census, Govt. of India and in the exercise of the powers conferred under section 4(4) and 10(2) of the Registration of Births & Deaths Act, 1969 and in accordance with the provisions under Rule 7 of the Chandigarh Registration of Births and Deaths Rules, 2000, all Medical Institutions (Government as well as Private) having in-patient facility in the UT of Chandigarh are hereby directed to mandatorily send the certificate as to the cause of deaths (MCCD) in form No. 4 and 4A, the same should be sent to the concerned Registrar along with the death reporting form.
