अधिसूचना

क्रमांक एफ 3-32/2021/38-2. - छत्तीसगढ़ निजी विश्वविद्यालय विनियामक आयोग, रायपुर के पत्र क्रमांक 668/नि.वि.क./03/प्र.परिनियम/2011/21252, दिनांक 11-12-2024 द्वारा कलिंगा विश्वविद्यालय, ग्राम-कोटनी पलौद, तह.-आरंग, जिला-रायपुर (छत्तीसगढ़) के अनुगामी अध्यादेश क्रमांक 179 व 180 तथा संशोधित अध्यादेश क्रमांक 114 का अनुमोदन छत्तीसगढ़ निजी विश्वविद्यालय (स्थापना एवं संचालन) अधिनियम, 2005 की धारा 29(2) के तहत्‌ किया गया है।

2/ राज्य शासन, एतद्‌द्वारा, उपरोक्त अध्यादेशों को राजपत्र में अधिसूचित किये जाने की स्वीकृति प्रदान करता है। 3/ उपरोक्त अध्यादेश राजपत्र में प्रकाशन की तिथि से प्रभावशील होंगे।

छत्तीसगढ़ निजी विश्वविद्यालय (स्थापना एवं संचालन) अधिनियम, 2005 (क्रं.13 सन्‌ 2005) के अंतर्गत स्थापित

अध्यादेश क्र. - 179 बैचलर ऑफ फार्मेसी (प्रैक्टिस)- बी.फार्म (प्रैक्टिस)

1. परिचयः

इस अध्यादेश को कलिंगा विश्वविद्यालय के अध्यादेश क्रमांक - 179 बैचलर ऑफ फार्मेसी (प्रैक्टिस), संक्षिप्त नाम बी.फार्म (प्रैक्टिस) के रूप में जाना जाएगा।

2. शीर्षक :

बैचलर ऑफ फार्मेसी (प्रैक्टिस)।

3. संकाय :

बैचलर ऑफ फार्मेसी (प्रैक्टिस) फार्मेसी संकाय के अंतर्गत संचालित किया जायेगा।

4. अवधि :

पाठ्‌यक्रम की अवधि दो साल की होगी एवं प्रत्येक वर्ष की अवधि 180 कार्य दिवसों से कम नहीं होगी ।

5. पात्रता :

(1) फार्मेसी अधिनियम, 1948 की धारा 12 के तहत फार्मेसी काउंसिल ऑफ इंडिया द्वारा अनुमोदित संस्थान से फार्मेसी में डिप्लोमा पाठ्यक्रम में उत्तीर्ण हो।

(2) एक पंजी—त फार्मासिस्ट हो। (3) किसी समुदाय या अस्पताल के फार्मेसी में फार्मेसी प्रैक्टिस का न्यूनतम 4 वर्ष का अनुभव हो :-

(अ) सक्षम प्राधिकारी से एक प्रमाण पत्र प्राप्त हुआ हो जिसमें कहा गया हो कि सामुदायिक फार्मासिस्ट के मामले में प्रैक्टिस अनुभव के प्रमाण के रूप में उम्मीदवार को फार्मेसी के दवा लाइसेंस में पंजी—त फार्मासिस्ट के रूप में अनुमोदित किया गया है। (ब) प्राचार्य/चिकित्सा अधीक्षक/अस्पताल के सक्षम व्यक्ति/किसी स्वास्थ्य इकाई का एक प्रमाण पत्र जिसमें कहा गया हो कि उम्मीदवार फार्मासिस्ट के रूप में काम कर रहा है, अस्पताल फार्मासिस्ट के मामले में प्रैक्टिस अनुभव के प्रमाण के रूप में स्वीकार किया जाएगा।

(4) निर्धारित प्रारूप में नियोक्ता से अनापत्ति प्रमाण पत्र (संलग्नक - 1)

6. सीट :

कार्यक्रम में प्रवेश संख्या भारतीय भेषजी परिषद्‌ द्वारा समय-समय पर निर्धारित की जाएगी और वर्तमान में यह संख्या एक शैक्षणिक वर्ष में 40 छात्रों तक सीमित है। पी.सी.आई. एव सी.जी.पी.यू आर.सी. से पूर्व अनुमति लेकर एक से अधिक इकाइयाँ भी स्थापित की जा सकेगी।୩୬୨ (୧) नोट ः अध्ययन के पाठ्यक्रम को समय-समय पर जारी पीसीआई दिशानिर्देशों के अनुसार संशोधित किया जाएगा।

8. प्रवेश :

जैसा कि अध्यादेश क्र-1 में वर्णित है। प्रवेश डिप्लोमा परिक्षा की प्रावीण्यता योग्यता के आधार पर दिया जायेगा। छत्तीसगढ़ राज्य से संबंधित छात्रों को वरीयता दी जाएगी और राज्य सरकार द्वारा प्रवेश एवं आरक्षण से संबंधित सभी दिशा-निर्देशों का पालन किया जायेगा।

9. शैक्षणिक वर्ष :

प्रत्येक वर्ष में शैक्षणिक सत्र का एक चक्र होगा जो कि जुलाई से जून तक का होगा।୩୬୨ (୨) 10. चयनः

विश्वविद्यालय प्रत्येक शैक्षणिक वर्ष की शुरूआत से पहले विश्वविद्यालय के नोटिस बोर्ड पर और अन्य प्रचार माध्यमों, समाचार पत्रों में प्रवेश अधिसूचना जारी करेगा। मेरिट के आधार पर प्रवेश के लिए चयनित उम्मीदवारों की सूची वेबसाइट पर प्रदर्शित की जाएगी या छात्रों को सीधे उनके प्रवेश के बारे में सूचित किया जाएगा। चयनित छात्रों की सूची विश्वविद्यालय के नोटिस बोर्ड पर भी प्रदर्शित की जाएगी। जिन उम्मीदवारों के परिणाम प्रतीक्षित हैं, वे भी आवेदन कर सकते हैं। हालांकि, इस तरह के उम्मीदवार के लिए अंतिम वर्ष की परीक्षा उत्तीर्ण करना अनिवार्य होगा। अगर उम्मीदवार निर्धारित प्रतिशत के साथ योग्यता परीक्षा को पास करने में विफल रहता है, तो उसका प्रवेश रद्द माना जायेगा। इसके अलावा उम्मीदवार को प्रथम सेमेस्टर का परीक्षा फॉर्म भरने से पहले निर्धारित योग्यता के साथ उत्तीर्ण होने की मार्कशीट प्रस्तुत करनी होगी। प्रवेश एवं आरक्षण के संबंध में छत्तीसगढ़ सरकार द्वारा समय-समय पर जारी किए गए निर्देशों का पालन किया जाएगा। 11. शुल्क :

पाठ्‌यक्रम का शुल्कप्रबंधन बोर्ड/ अकादमिक परिषद के द्वारा समय-समय पर निर्धारित किया जाएगा तथा उसे छ.ग. निजी विश्वविद्यालय विनियामक आयोग के समक्ष अनुमोदन हेतु प्रस्तुत किया जायेगा। 12. प्रवेश को रद्‌द करना (निरस्तीकरण)ः

निम्नलिखित परिस्थितियों में छात्र का प्रवेश रद्द किया जा सकता है-

i. किसी भी स्तर पर छात्र को शासकीय मानदंड/दिशानिर्देश या विश्वविद्यालय के निर्धारित मानदंड के अनुसार योग्य नहीं पाया जाता है।

ii. संस्थान/विश्वविद्यालय में अनुशासनहीनता का दोषी पाए जाने पर। iii. झूठे/जाली दस्तावेज का निर्माण करके अनुचित/अवैध तरीके से प्रवेश लिया हो। iv. शुल्क का भुगतान नहीं किया जाता है। v. आवेदन पत्र किसी भी तरह से अधूरा है। vi. प्रवेश के लिए सहायक दस्तावेज संलग्न नहीं है।

सभी आवश्यक दस्तावेजों और फीस के सत्यापन और जमा करने के बाद विश्वविद्यालय द्वारा पंजीकरण संख्या छात्र को प्रदान की जाएगी।

13. माध्यमः

अध्यापन एवं परिक्षा का माध्यम अंग्रेजी रहेगा।

14. परीक्षाः

1. कलेण्डर वर्ष के अंत में एक परीचा होगी। प्रथम परीक्षा वार्षिक परीक्षा होगी और दूसरी परीक्षा अनुपूरक परीक्षा होगी।

2. परीक्षाएं सिद्धांत पक्ष के लिग एवं व्यवहार पक्ष के लिग लिखित रूप् में होंगी। छात्र अपने द्वारा किए गए समनुदेशन कार्य को रिपोर्ट के रूप में प्रस्तुत करेंगे उसके याद विषय के हर भाग के लिए अधिकतम अंकों की मोखिक परीक्षा होगी जैसा कि निम्न तालिका में दर्शाया गया हैः-୩୬୨ (୩) नोट ः परीक्षा की योजना को समय-समय पर जारी पीसीआई दिशानिर्देशों के अनुसार संशोधित किया जाएगा।

15. परीक्षा में बैठने की पात्रता

(i) (ii) विशेष परिस्थिति में विश्वविद्यालय के कुलपति के द्वारा 15 प्रतिशत तक की छूट दी जाएगी जिसमें बीमारी, एनसीसी/एनएसएस शिविर एवं परेड, अंतर्विश्वविद्यालयीन, राज्य एवं राष्ट्रीय୩୬୨ (୪) स्तर की क्रीड़ा प्रतियोगिता में सहभागिता सम्मिलित है। इसके अतिरिक्त विश्वविद्यालयीन कार्यों और शैक्षणिक भ्रमण, फील्ड वर्क, फील्ड ट्रिप में संलग्नता के आधार पर छूट दी जा सकती है बशर्ते उपस्थिति अभिलेख प्रभारी शिक्षक द्वारा प्रतिहस्ताक्षरित कर संस्थान के सक्षम अधिकारी को कार्यक्रम/गतिविधि आदि के दो सप्ताह के भीतर प्रस्तुत किया जावेगा।

(iii) बीमारी/चिकित्सा विकलांगता के मामले में पंजीकृत चिकित्सक/सार्वजनिक अस्पताल द्वारा जारी किए गए चिकित्सा प्रमाण पत्र द्वारा या तो मुख्य चिकित्सा अधिकारी (सिविल सर्जन) या विश्वविद्यालय स्वास्थ्य केंन्द्र द्वारा विधिवत रूप से प्रमाणित किया गया हो। कलिंगा युनिवर्सिटी/संस्थान/विभाग के आधिकारिक डॉक्टर। इस तरह के आवेदन या तो उपचार/अस्पताल में भर्ती होने की अवधि के दौरान या वापसी के बाद दो सप्ताह के भीतर प्रस्तुत किए जाने पर भी उपस्थिति में छूट दी जाएगी।

20. उपाधि प्रदाय करना :

एक छात्र जो उपरोक्त कंडिका 16 से 18 के सभी मानदंडों के अनुसार सभी पेपरों में और सभी वर्षों में उत्तीर्ण हुआ, वह ''बैचलर ऑफ फार्मेसी (प्रैक्टिस)'' उपाधि के लिए पात्र होगा।୩୬୨ (୫) 21. सामान्य :

(i) प्रत्येक कोर्स का विस्तृत पाठ्‌यक्रम तथा पाठ्‌यक्रम आव्यूह (Matrix) अध्ययन बोर्ड द्वारा तैयार किया जाएगा। जो अकादमिक परिषद और कुलपति द्वारा अनुमोदित किया जाएगा।

(ii) किसी विवाद की स्थिति में, उसका निराकरण छत्तीसगढ़ उच्च न्यायालय के क्षे़त्राधिकार में होगा। (iii) छत्तीसगढ़ निजी विश्वविद्यालय विनियामक आयोग (CGPURC) के पूर्व अनुमोदन से अन्य कोई प्रावधान जो ऊपर वर्णित नही है, वह कलिंगा विश्वविद्यालय द्वारा समय-समय पर बनाए गए नियमों और विनियमों से शासित होगा।

(iv) इस अध्यादेश के हिंदी संस्करण में किसी भी अस्पष्टता के मामले में, अंग्रेजी संस्करण को अंतिम माना जाएगा।

उसे पाठ्यक्रम में भाग लेने की अनुमति दी जाएगी और इस संस्थान/संगठन में पाठ्यक्रम के हिस्से के रूप में कार्य पूरा करने के लिए सुविधाएं प्रदान की जाएंगी।

अध्यादेश क्र. - 180 फार्म. डी.

1. परिचय :

इस अध्यादेश को कलिंगा विश्वविद्यालय के अध्यादेश क्रमांक- 180 फार्म. डी. के नाम से जाना जाएगा।

2. शीर्षक :

फार्म. डी. ।

3. संकाय :

फार्म. डी. फार्मेसी संकाय के अंतर्गत संचालित किया जायेगा।

4. अवधि :

(अ) फार्म. डी - पाठ्यक्रम की अवधि 6 शैक्षणिक वर्ष (अध्ययन के 5 वर्ष और इंटर्नशिप या रेजीडेंसी का 1 वर्ष) होगी और प्रत्येक शैक्षणिक वर्ष कम से कम दो सौ कार्य दिवसों की अवधि में होगी। 6 वर्ष की अवधि को दो चरणों में विभाजित किया गया है :-

चरण 1 - जिसमें पहला, दूसरा, तीसरा, चौथा और पांचवां शैक्षणिक वर्ष शामिल है। चरण 2 - छठें वर्ष के दौरान इंटर्नशिप या रेजीडेंसी प्रशिक्षण जिसमें विशेष इकाइयों में पोस्टिंग शामिल है। यह प्रशिक्षण का एक चरण है जिसमें एक छात्र को वास्तविक फार्मेसी अभ्यास या क्लिनिकल फार्मेसी सेवाओं से अवगत कराया जाता है और पर्यवेक्षण के तहत कौशल हासिल किया जाता है ताकि वह स्वतंत्र रूप से कार्य करने में सक्षम हो सके।

5. पात्रता :

(अ) फार्म. डी. भाग- 1 पाठ्यक्रम - निम्नलिखित में से किसी भी परीक्षा में उत्तीर्ण होना चाहीए :-

(1) 102 परीक्षा में भौतिकी और रसायन विज्ञान अनिवार्य विषयों के साथ-साथ निम्नलिखित विषयों में से एक होगा :

गणित या जीवविज्ञान ।

(2) फार्मेसी अधिनियम की धारा 12 के तहत फार्मेसी काउंसिल ऑफ इंडिया द्वारा अनुमोदित संस्थान से डी. फार्म पाठ्यक्रम में उत्तीर्ण हो।

(3) उपरोक्त परीक्षाओं में से किसी के समकक्ष फार्मेसी काउंसिल ऑफ इंडिया द्वारा अनुमोदित कोई अन्य योग्यता।୩୬୨ (୭) 6. सीट :

कार्यक्रम में प्रवेश की संख्या फार्मेसी काउंसिल ऑफ इंडिया द्वारा समय-समय पर निर्धारित की जाएगी और वर्तमान में एक शैक्षणिक वर्ष में 30 छात्रों तक सीमित है। पीसीआई और सीजीपीयूआरसी की पूर्व स्वी—ति के साथ कई इकाइयाँ भी स्थापति की जा सकती है। पीसीआई और सीजीपीयअू ारसी की स्वी—ति के बाद कई इकाइयाँ भी स्थापित की जा सकती है।

7. प्रवेश :

जैसा कि अध्यादेश क्र-1 में वर्णित है। प्रवेश उपरोक्त वर्णित पात्र सर्टिफिकेट्‌स की प्रावीण्यता के आधार पर दिया जायेगा। छत्तीसगढ़ राज्य से संबंधित छात्रों को वरीयता दी जाएगी और राज्य सरकार द्वारा प्रवेश एवं आरक्षण से संबंधित सभी दिशा-निर्देशों का पालन किया जायेगा।

8. शैक्षणिक वर्ष :

पाठ्‌यक्रम का शुल्क प्रबंधन बोर्ड/ अकादमिक परिषद के द्वारा समय-समय पर निर्धारित किया जाएगा तथा उसे छ.ग. निजी विश्वविद्यालय विनियामक आयोग के समक्ष अनुमोदन हेतु प्रस्तुत किया जायेगा।୩୬୨ (୮) 11. प्रवेश को रद्‌द करना (निरस्तीकरण)ः

निम्नलिखित परिस्थितियों में छात्र का प्रवेश रद्द किया जा सकता है-

i. किसी भी स्तर पर छात्र को शासकीय मानदंड/दिशानिर्देश या विश्वविद्यालय के निर्धारित मानदंड के अनुसार योग्य नहीं पाया जाता है।

12. शिक्षण का माध्यम :

अध्यापन एवं परीक्षा का माध्यम अंग्रेजी रहेगा।

13. मूल्यांकन और परीक्षा :(कलिंगा विश्वविद्यालय अध्यादेश क्र. 51 अध्याय II बिंदु क्र.8) शिक्षण और मूल्यांकन योजना के अनुसार छात्र के प्रदर्शन का मूल्यांकन आंतरिक मूल्यांकन, असाइनमेंट, शोध प्रबंध/शोध/परियोजना कार्य, क्षेत्र सर्वेक्षण, वाइवा वोस, सेमेस्टर वार विश्वविद्यालय परीक्षाओं आदि जैसे सतत मूल्यांकन के माध्यम से किया जाएगा।

14. अध्ययन का पाठ्‌यक्रमः

फार्म. डी. के अध्ययन के पाठ्‌यक्रम के नीचे दी गई तालिकाओं में दिए गये विषय शामिल होंगे। प्रत्येक विषय को सिद्धांत, व्यावहारिक और ट्‌यूटोरियल में पढ़ाने के लिए सप्ताह में समर्पित घंटों की संख्या नीचे कॉलम (3),(4) और (5) में इसके सामने उल्लिखित घंटों से कम नहीं होगीः

छठवा वर्षाः इंटर्नशिप या रेजीेडेंसी टे्रनिंग जिसमें विशेष इकाइयों में पोस्टिंग शामिल है। छात्र को आवंटित वार्डो में स्वतंत्र रूप से क्लिनिकल फार्मेसी सेवाएँ प्रदान करनी चाहिए। (i) जनरल मेडिसिन विभाग में छह महीने, और (ii) तीन अन्य विशिष्ट विभागों में दो-दो महीने

नोटः अध्ययन पाठ्‌यक्रम को समय-समय पर जारी पीसीआई दिशानिर्देशों के अनुसार संशोधित किया जाएगा।

15. परीक्षाः

1) प्रत्येक वर्ष विद्यार्थियों की जांच के लिए परीक्षा आयोजित की जाएगी।

2) प्रत्येक परीक्षा वर्ष में दो बार आयोजित की जा सकेगी। वर्ष में पहली परीक्षा वार्षिक परीक्षा होगी तथा दूसरी परीक्षा पूरक होगी।

3) परीक्षाऐं लिखित और प्रायोगिक (मौखिक प्र—ति सहित) होंगी, जिनमेें विषय के प्रत्येक भाग के लिए अधिकतम अंक होंगे, जैसा कि नीचे दी गई तालिका में दर्शाया गया हैः-୩୬୨ (୧୧) ୩୬୨ (୧୨) नोटः परीक्षा की योजना समय-समय पर जारी पीसीआई दिशानिर्देशों के अनुसार संशोधित किया जाएगा।

16. परीक्षा में बैठने की पात्रताः

(i) वही छात्र परीक्षा में बैठने का पात्र होगा जो उस संस्था के प्रमुख का, जिसमें उसने पढ़ाई की है, प्रमाण-पत्र इस सबूत के रूप में प्रस्तुत करेगा कि उसने सिद्धांत पक्ष कम से कम 80 प्रतिशत कक्षाओं में उपस्थित रहकर नियमित तथा समाधानप्रद रूप में पढ़ाई की है और उसने पर्यवेक्षक शिक्षक द्वारा सम्यकतः अनुमोदित असाइनमेंट/परियोजना रिपोर्ट प्रस्तुत कर दी ळें

(ii) विशेष परिस्थिति में विश्वविद्यालय के कुलपति के द्वारा 15 प्रतिशत तक की छूट दी जाएगी जिसमें बीमारी, एनसीसी/एनएसएस शिविर एवं परेड, अंतर्विश्वविद्यालयीन, राज्य एवं राष्ट्रीय स्तर की क्रीड़ा प्रतियोगिता में सहभागिता सम्मिलित है। इसके अतिरिक्त विश्वविद्यालयीन कार्यों और शैक्षणिक भ्रमण, फील्ड वर्क, फील्ड ट्रिप में संलग्नता के आधार पर छूट दी जा सकती है बशर्ते उपस्थिति अभिलेख प्रभारी शिक्षक द्वारा प्रतिहस्ताक्षरित कर संस्थान के सक्षम अधिकारी को कार्यक्रम/गतिविधि आदि के दो सप्ताह के भीतर प्रस्तुत किया जावेगा।

(i) बीमारी/चिकित्सा विकलांगता के मामले में पंजीकृत चिकित्सक/सार्वजनिक अस्पताल द्वारा जारी किए गए चिकित्सा प्रमाण पत्र द्वारा या तो मुख्य चिकित्सा अधिकारी (सिविल सर्जन) या विश्वविद्यालय स्वास्थ्य केंन्द्र द्वारा विधिवत रूप से प्रमाणित किया गया हो। कलिंगा युनिवर्सिटी/संस्थान/विभाग के आधिकारिक डॉक्टर। इस तरह के आवेदन या तो उपचार/अस्पताल में भर्ती होने की अवधि के दौरान या वापसी के बाद दो सप्ताह के भीतर प्रस्तुत किए जाने पर भी उपस्थिति में छूट दी जाएगी।

17. परीक्षा का ढंगः

(i) सैद्धांतिक परीक्षा तीन घंटे की होगी और प्रायोगिक परीक्षा चार घंटे की होगी (ii) कोई छात्र जो किसी विषय की सैद्धांतिक या प्रायोगिक परीक्षा में अनुत्तीर्ण हो जाता है, उसे उसी विषय की सैद्धांतिक और प्रायोगिक दोनों परीक्षाएं पुनः देनी होंगी।୩୬୨ (୧୩) (iii) प्रायोगिक परीक्षा में मौखिक परीक्षा भी शामिल होगी।

(iv) क्लर्कशिप परीक्षा- छात्रों की क्लर्कशिप पूरी होने के बाद मौखिक परीक्षा आयोजित की जाएगी। एक बाहरी और एक आंतरिक परीक्षक छात्र का मूल्यांकन करेंगे। छात्रों को आवंटित चिकित्सा मामलों को प्रस्तुत करने के लिए कहा जा सकता है, उसके बाद चर्चा की जाएगी। नैदानिक फार्मेसी सेवाएं प्रदान करने, फार्मास्युटिकल देखभाल योजना और चिकित्सा विज्ञान के ज्ञान के छात्रों की क्षमताओं का मूल्यांकन किया जाएगा।

18. उपाधि प्रदाय करना :

एक छात्र जो उपरोक्त कंडिका 15 से 17 के सभी मानदंडों के अनुसार सभी पेपरों में और सभी वर्षों में उत्तीर्ण हुआ, वह '' फार्म. डी. '' उपाधि के लिए पात्र होगा।

19. सामान्य : (i) प्रत्येक कोर्स का विस्तृत पाठ्‌यक्रम तथा पाठ्‌यक्रम आव्यूह (Matrix) अध्ययन बोर्ड द्वारा तैयार किया जाएगा। जो अकादमिक परिषद और कुलपति द्वारा अनुमोदित किया जाएगा।

(ii) किसी विवाद की स्थिति में, उसका निराकरण छत्तीसगढ़ उच्च न्यायालय के क्षे़त्राधिकार में होगा। (iii) छत्तीसगढ़ निजी विश्वविद्यालय विनियामक आयोग (CGPURC) के पूर्व अनुमोदन से अन्य कोई प्रावधान जो ऊपर वर्णित नही है,वह कलिंगा विश्वविद्यालय द्वारा समय-समय पर बनाए गए नियमों और विनियमों से शासित होगा।

(iv) इस अध्यादेश के हिन्दी संस्करण में किसी प्रकार की अस्पष्टता होने पर अंग्रेजी संस्करण को ही अंतिम माना जाएगा।୩୬୨ (୧୪)

कलिंगा विश्वविद्यालय के अध्यादेश में संशोधन

अध्यादेश क्रमांक 114 बैचलर ऑफ वोकेशनल स्टडीज (बी.वॉक.) में पुनः संशोधन

अध्यादेश क्रमांक 114 का प्रथम अनुमोदन 06 अप्रैल 2022 को किया गया था। उक्त अध्यादेश को बाद में निरस्त कर पुनः संशोधित किया गया तथा 11 जुलाई 2023 को प्रकाशित किया गया। अब पुनः छत्तीसगढ़ राजपत्र में प्रकाशनार्थ नवीन शाखाओं के साथ उक्त अध्यादेश में संशोधन हेतु आवेदन प्रस्तुत है। 1. उपरोक्त दोनों प्रकाशित मौजूदा अध्यादेश संख्या 114 को निरस्त कर प्रतिस्थापित किया जाएगा और उसके स्थान पर निम्नलिखित संशोधित अध्यादेश संख्या 114 लाया जाएगा।

4. अवधि :

पाठ्‌यक्रम की अवधि 03 वर्ष (छः सेमेस्टर) की होगी और अधिकतम अवधि 06 वर्ष की होगी।୩୬୨ (୧୫) 5. पात्रता :

10+2 परीक्षा उत्तीर्ण होना चाहिए।

6. सीट :

मूल इकई 60 सीटों की होगी। सीजीपीयूआरसी से अनुमति लेकर एक से अधिक इकाइयाँ भी स्थापित की जा सकेगी।

7. प्रवेश :

जैसा कि अध्यादेश क्र-1 में वर्णित है। प्रवेश 10+2 परीक्षा की प्रावीण्यता के आधार पर दिया जायेगा। छत्तीसगढ़ राज्य से संबंधित छात्रों को वरीयता दी जाएगी और राज्य सरकार द्वारा प्रवेश एवं आरक्षण से संबंधित सभी दिशा-निर्देशों का पालन किया जायेगा।

8. शैक्षणिक वर्ष :

प्रत्येक वर्ष में शैक्षणिक सत्र का एक चक्र होगा जो कि जुलाई से जून तक का होगा।

9. चयनः विश्वविद्यालय प्रत्येक शैक्षणिक वर्ष की शुरूआत से पहले विश्वविद्यालय के नोटिस बोर्ड पर और अन्य प्रचार माध्यमों, समाचार पत्रों में प्रवेश अधिसूचना जारी करेगा। मेरिट के आधार पर प्रवेश के लिए चयनित उम्मीदवारों की सूची वेबसाइट पर प्रदर्शित की जाएगी तथा छात्रों को सीधे उनके प्रवेश के बारे में सूचित किया जाएगा। चयनित छात्रों की सूची विश्वविद्यालय के नोटिस बोर्ड पर भी प्रदर्शित की जाएगी। जिन उम्मीदवारों के परिणाम प्रतीक्षित हैं, वे भी आवेदन कर सकते हैं। हालांकि, इस तरह के उम्मीदवार के लिए अंतिम वर्ष की परीक्षा उत्तीर्ण करना अनिवार्य होगा। अगर उम्मीदवार निर्धारित प्रतिशत के साथ योग्यता परीक्षा को पास करने में विफल रहता है, तो उसका प्रवेश रद्द माना जायेगा। इसके अलावा उम्मीदवार को प्रथम सेमेस्टर का परीक्षा फॉर्म भरने से पहले निर्धारित योग्यता के साथ उत्तीर्ण होने की मार्कशीट प्रस्तुत करनी होगी। प्रवेश एवं आरक्षण के संबंध में छत्तीसगढ़ सरकार द्वारा समय-समय पर जारी किए गए निर्देशों का पालन किया जाएगा। 10. शुल्क :

पाठ्‌यक्रम का शुल्कप्रबंधन बोर्ड/ अकादमिक परिषद के द्वारा समय-समय पर निर्धारित किया जाएगा तथा उसे छ.ग. निजी विश्वविद्यालय विनियामक आयोग के समक्ष अनुमोदन हेतु प्रस्तुत किया जायेगा। 11. प्रवेश को रद्‌द करना (निरस्तीकरण)ः

निम्नलिखित परिस्थितियों में छात्र का प्रवेश रद्द किया जा सकता है-

i. किसी भी स्तर पर छात्र को शासकीय मानदंड/दिशानिर्देश या विश्वविद्यालय के निर्धारित मानदंड के अनुसार योग्य नहीं पाया जाता है।

ii. संस्थान/विश्वविद्यालय में अनुशासनहीनता का दोषी पाए जाने पर। iii. झूठे/जाली दस्तावेज का निर्माण करके अनुचित/अवैध तरीके से प्रवेश लिया हो। iv. शुल्क का भुगतान नहीं किया जाता है।୩୬୨ (୧୬) v. आवेदन पत्र किसी भी तरह से अधूरा है। vi. प्रवेश के लिए सहायक दस्तावेज संलग्न नहीं है।

सभी आवश्यक दस्तावेजों और फीस के सत्यापन और जमा करने के बाद विश्वविद्यालय द्वारा पंजीकरण संख्या छात्र को प्रदान की जाएगी।

12. माध्यमः

अध्यापन का माध्यम अंग्रेजी एवं हिन्दी होगा एवं परीक्षा अंगे्रजी/हिन्दी में दी जा सकेगी। 13. उपस्थितिः (कंिलंगा विश्वविद्यालय अध्यादेश क्र-51, अध्याय II, बिंदु क्र.7)

(i) विद्यार्थियों को विश्वविद्यालय में अध्ययन के लिए नियमित पाठ्‌यक्रम को पूर्ण करना होगा। उसे प्रत्येक विषय में कम से कम 60% व्याख्यान में उपस्थित होना अनिवार्य होगा जिसमें व्याख्यान, प्रायोगिक एवं विशेष कक्षा के साथ 75% उपस्थिति परीक्षा में शामिल होने के लिए अनिवार्य होगी।

14. मूल्यांकन और परीक्षा :(कलिंगा विश्वविद्यालय अध्यादेश क्र. 51 अध्याय II बिंदु क्र.8) शिक्षण और मूल्यांकन योजना के अनुसार छात्र के प्रदर्शन का मूल्यांकन आंतरिक मूल्यांकन, असाइनमेंट, शोध प्रबंध/शोध/परियोजना कार्य, क्षेत्र सर्वेक्षण, वाइवा वॉइस, सेमेस्टर वार विश्वविद्यालय परीक्षाओं आदि जैसे सतत मूल्यांकन के माध्यम से किया जाएगा।

15. उत्तीर्ण होने हेतु मानदंडः

(i) प्रत्येक विद्यार्थी को प्रत्येक विषय उत्तीर्ण करने तथा उस विषय के लिए निर्धारित के्रडिट अर्जित करने हेतु न्यूनतम 40% अंक अर्जित करना आवश्यक होगा, जिसमें सेमेस्टर की मुख्य परीक्षा एवं विषय शिक्षक द्‌वारा किए गए सतत्‌ मूल्यांकन के प्राप्तांक सम्मिलित होंगे, इसके अतिरिक्त सेमेस्टर की मुख्य परीक्षा के समस्त विषयों/प्रश्नपत्रों में पृथक-पृथक न्यूनतम 40% अंक दोनो आंतरिक एवं बाह्‌य परीक्षा में अलग-अलग, तथा समस्त प्रश्नपत्रों में कुल मिलाकर୩୬୨ (୧୭) न्यूनतम 45% अंक प्राप्त करना आवश्यक होगा। ऐसा विद्यार्थी जो किसी भी विषय में 40% से कम तथा सभी विषयों में कुल मिलाकर 45% से कम अंक अर्जित करेगा उसे अनुत्तीर्ण घोषित किया जाएगा।

(ii) एक छात्र निर्धारित शुल्क के भुगतान पर एक विशिष्ट पाठ्‌यक्रम के परीक्षा स्क्रिप्ट के पुनर्मूल्यांकन के लिए, परिणाम की घोषणा की तारीख से दो सप्ताह के भीतर आवेदन कर सकता हैं। पुनर्मूल्यांकन का अर्थ यह सत्यापित करना होगा कि क्या सभी प्रश्न और उनके भाग को प्रश्न पत्र और अंकों के कुल के अनुसार विधिवत चिन्हित किया गया है। एक विसंगति पाए जाने की स्थिति में दोनों परिणामोें में उपयुक्त परिवर्तन के साथ-साथ संबंधित सेमेस्टर की अंतिम परीक्षा के मार्कशीट को भी ठीक किया जाएगा।

16. उपाधि प्रदाय करना :

एक छात्र जो उपरोक्त कंडिका 15 के सभी मानदंडों के अनुसार सभी विषयों में और सभी सेमेस्टरो में उत्तीर्ण हुआ, वह ''बैचलर ऑफ वोकेशन (....विशेषज्ञता का नाम....)'' उपाधि के लिए पात्र होगा। 17. सामान्य :

(i) प्रत्येक कोर्स का विस्तृत पाठ्‌यक्रम तथा पाठ्‌यक्रम आव्यूह (Matrix) अध्ययन बोर्ड द्वारा तैयार किया जाएगा। जो अकादमिक परिषद और कुलपति द्वारा अनुमोदित किया जाएगा। (ii) किसी विवाद की स्थिति में, उसका निराकरण छत्तीसगढ़ उच्च न्यायालय, बिलासपुर के क्षे़त्राधिकार में होगा।

Bachelor of Pharmacy (Practice) – B. Pharm (Practice)

1. INTRODUCTION:

This Ordinance shall be known as Kalinga University Ordinance No- 179. Bachelor of Pharmacy (Practice) abbreviated as B. Pharm (Practice)

2. TITLE

Bachelor of Pharmacy (Practice).

3. FACULTY:

Bachelor of Pharmacy (Practice) program will be under Faculty of Pharmacy.

4. DURATION:

The duration of the course shall be of two academic years with each year spread over a period of not less than 180 working days.

5. ELIGIBILITY:

i. A pass in Diploma course in Pharmacy from an institution approved by the Pharmacy Council of India under section 12 of the Pharmacy Act, 1948.

ii. A registered pharmacist.

iii. A minimum of four years of pharmacy practice experience in a community or hospital pharmacy –

a. A certificate from competent authority stating that the candidate is endorsed as registered pharmacist in the drug license of a pharmacy as proof of practice experience in case of community pharmacist.

b. A certificate from the Principal/Medical Superintendent/competent person of the Hospital/Health Unit stating that the candidate is working as a pharmacist will be accepted as proof of practice experience in the case of hospital pharmacist.

iv. A 'No Objection Certificate' from the employer in prescribed format (Annexure-A) 6. SEATS:

The number of admissions in the programme shall be as prescribed by the Pharmacy Council of India from time to time and presently be restricted to 40 students in an academic year. With the prior approval of PCI & CGPURC multiple units can also be set up.

7. COURSE OF STUDIES:

The course of study shall consist of the subjects as given in the Tables below. The course shall consist of class room teaching and assignment works. The assignment works shall be done at the place of work under the supervision and guidance of teaching staff of the academic institution. The number of contact hours in a week devoted to each subject for class room teaching shall not be less than that noted against it in columns (3) below.

୩୬୨ (୨୦) Note: The Couse of Studies shall be revised as per PCI Guidelines issued from time to time.

8. ADMISSION:

As specified in Ordinance No-01. Admission shall be granted on the basis of the merit of Diploma in Pharmacy. Preference will be given to students belonging to the State of Chhattisgarh and all guidelines issued by the State Government related to admission and reservation shall be followed.

9. ACADEMIC YEAR:

There will be one academic cycle per year from July to June.

10. SELECTION:

The University will issue admission notification, on the notice board of the University and in other publicity media, in newspaper before the start of every academic year. The list of candidates selected for admission on merit basis will be displayed on the website and the students will be informed directly about their admission. The list of the selected students will also be displayed on the notice board of the university. The candidates whose results are awaited can also apply. However, such a candidate must produce proof of appearing in the final year of qualifying examination, provided if the candidate fails to clear the qualifying exam with required percentage his/her admission shall be treated as cancelled. Further provided that the candidate must submit mark sheet of qualifying examination before the filling of examination form of first semester. Directives issued by the Government of Chhattisgarh, from time to time, regarding admission and reservation, shall be followed while making admissions.

11. FEE:

The course fee will be decided by the Board of Management/ Academic Council from time to time and shall be submitted for approval to CGPURC.

12. CANCELLATION OF ADMISSION:

Admission of a student may be cancelled under following circumstances:

i. At any stage, if a student is not found qualified, for the Programme, as per Government norms/guidelines or the eligibility criteria prescribed by the University.

ii. Involvement in gross indiscipline in the Institute/University

iii. She/he is found to have produced false/forged documents or found to have used unfair means to secure admission.

iv. The fee is not paid.

v. The application form is incomplete in any way.

vi. The supporting documents required for admission are not enclosed.

Registration number will be assigned to the student by the University after verification and submission of all the necessary documents and fees.

13. MEDIUM OF INSTRUCTION:

English shall be the medium of Instruction and Examination.

14. EXAMINATION:

1. There shall be an examination at the end of calendar year. The first examination shall be the annual examination and the second examination shall be supplementary examination.

2. The examinations shall be of written nature for theory and for the practicals: The students shall submit the assignments done by them in the form of a report which will be followed by viva-voce carrying maximum marks for each part of a subject as indicated in Tables below :

୩୬୨ (୨୨) Note: The scheme of Examination shall be revised as per PCI Guidelines issued from time to time.

15. ELIGIBILITY FOR APPEARING AT THE EXAMINATION:

(i) A student who produces a certificate from the Head of the Institution in which he has undergone the course in proof of his having regularly and satisfactorily undergone the course of study by attending not less than 80% of the classes held in theory and has submitted the assignments/ project report duly approved by the supervising teacher shall be eligible for appearing at the examination.

(ii) A relaxation to the maximum extent of 15% of the total attendance can be accorded to student by the Vice Chancellor on account of sickness, attendance at N.C.C/N.S.S camp and parades, participation as a member of the University team in any inter or intra university competition. Participation in the University functions and the prescribed educational tours/field trips/ field work, provided that the attendance record, duly counter signed by the teacher-in-charge, is sent to the head of the Institute concerned within two weeks of the function/activity etc.

(iii) Provided further in case of sickness/medical disability, an application for the condonation shall be supported by a medical certificate issued by a registered medical practitioner/public hospital and duly authenticated by either the Chief Medical Officer (Civil Surgeon) or the University Health center or Official Doctor of Kalinga University/Institute/ Department. Such applications must be submitted either during the period of treatment/hospitalization or within two weeks following recovery.

16. MODE OF EXAMINATIONS:

(1) Theory examination shall be of three hours duration.

(2) A student who fails in theory examination of a subject shall be permitted to re-appear in that subject

(3) Assignment work shall consist of evaluation of report by both internal & external examiners with a seminar and viva –voce (Oral) examination.

17. AWARD OF SESSIONAL MARKS AND MAINTENANCE OF RECORDS:

(1) A regular record of theory examinations conducted in an institution imparting the Bachelor of Pharmacy (Practice) Course, shall be maintained for each student in the institution and 40 marks for each subject shall be allotted as internal assessment.

(2) There shall be at least three periodic sessional examinations during each year and the highest aggregate of any two performances shall form the basis of calculating sessional marks.

18. MINIMUM MARKS FOR PASSING EXAMINATION:

A student shall not be declared to have passed examination unless he secures at least 50% marks in each of the subjects separately in the theory examinations, including sessional marks and at least 50% marks in assignment work. The students securing 60% marks or above in aggregate in all subjects in a single attempt at the examination shall be declared to have passed in first class. A student securing 75% marks or above in any subject or subjects shall be declared to have passed with distinction in the subject or those subjects provided he passes in all the subjects in a single attempt.

19. ELIGIBILITY FOR PROMOTION TO NEXT CLASS:

1. All students who have appeared for all the subjects and passed the examination are eligible for promotion to the next year.

2. The student failing in subjects of 1st year B.Pharm. (Practice) examination shall be permitted to proceed to the 2nd year of B.Pharm. (Practice). However, such students shall have to pass all the subjects of the 1st and 2nd year of B.Pharm. (Practice) course and shall complete the course within 4 academic years from the session in which he was admitted in the course, for the consideration of B.Pharm. (Practice) degree.

20. AWARD OF DEGREE:

A student who has passed as per the above criteria mentioned in clause 16 to 18 in all the papers and in all the years shall be eligible for the award of "Bachelor of Pharmacy (Practice)" Degree.

21. GENERAL:

(i) Detailed syllabus of each course and revision in course matrix shall be prepared by the Board of Studies and duly approved by the Academic Council and the Vice-Chancellor as per the requirements.

(ii) In case of any dispute the action shall lie within the Jurisdiction of High court of Chhattisgarh at Bilaspur.

(iii) Any other provision not contained in the above shall be governed by the rules and regulations framed by the Kalinga University from time to time, with the prior approval of Chhattisgarh Private Universities Regulatory Commission (CGPURC).

(iv) In case of any ambiguity in the Hindi version of this ordinance, the English version

shall be considered final.

Pharm. D.

1. INTRODUCTION:

This Ordinance shall be known as Kalinga University Ordinance No- 180. Pharm. D.

2. TITLE

Pharm. D.

3. FACULTY:

Pharm. D. program will be under Faculty of Pharmacy.

4. DURATION:

a) Pharm.D - The duration of the course shall be SIX academic years (five years of study and one year of internship or residency) full time with each academic year spread over a period of not less than two hundred working days. The period of six years duration is divided into two phases –

Phase I – consisting of First, Second, Third, Fourth and Fifth academic year.

Phase II – consisting of internship or residency training during sixth year involving posting in speciality units. It is a phase of training wherein a student is exposed to actual pharmacy practice or clinical pharmacy services and acquires skills under supervision so that he or she may become capable of functioning independently.

5. ELIGIBILITY:

a) Pharm.D. Part-I Course – A pass in any of the following examinations -

(1) 10+2 examination with Physics and Chemistry as compulsory subjects along with one of the following subjects:

Mathematics or Biology.

(2) A pass in D.Pharm course from an institution approved by the Pharmacy Council of India under section 12 of the Pharmacy Act.

(3) Any other qualification approved by the Pharmacy Council of India as equivalent to any of the above examinations.

6. SEATS:

The number of admission in the programme shall be as prescribed by the Pharmacy Council of India from time to time and presently be restricted to 30 students in an academic year. With the prior approval of PCI & CGPURC multiple units can also be set up. Multiple units can also be set up after approval of PCI and CGPURC.

୩୬୨ (୨୬) 7. ADMISSION:

As specified in Ordinance No-01. Admission shall be granted on the basis of merit. Preference will be given to students belonging to the State of Chhattisgarh and all guidelines issued by the State Government related to admission and reservation shall be followed.

8. ACADEMIC YEAR:

There will be one academic cycle per year from July to June.

9. SELECTION:

The University will issue admission notification, on the notice board of the University and in other publicity media, in newspaper before the start of every academic year. The list of candidates selected for admission on merit basis will be displayed on the website or the students will be informed directly about their admission. The list of the selected students will also be displayed on the notice board of the university. The candidates whose results are awaited can also apply. However, such a candidate must produce proof of appearing in the final year of qualifying examination, provided if the candidate fails to clear the qualifying exam with required percentage his/her admission shall be treated as cancelled. Further provided that the candidate must submit mark sheet of qualifying examination before the filling of examination form. Directives issued by the Government of Chhattisgarh, from time to time, regarding admission and reservation, shall be followed while making admissions.

10. FEE:

The course fee will be decided by the Board of Management/ Academic Council from time to time and shall be submitted for approval to CGPURC.

11. CANCELLATION OF ADMISSION:

Admission of a student may be cancelled under following circumstances:

i. At any stage, if a student is not found qualified, for the Programme, as per Government norms/guidelines or the eligibility criteria prescribed by the University.

ii. Involvement in gross indiscipline in the Institute/University

iii. She/he is found to have produced false/forged documents or found to have used unfair means to secure admission.

iv. The fee is not paid.

v. The application form is incomplete in any way.

vi. The supporting documents required for admission are not enclosed.

Registration number will be assigned to the student by the University after verification and submission of all the necessary documents and fees.

12. MEDIUM OF INSTRUCTION:

English shall be the medium of Instruction and the Examination.

13. EVALUATION & EXAMINATION: (Kalinga University Ordinance No. 51, Chapter

II point No.8.): The performance of student shall be evaluated through Continuous Evaluation like Internal Assessment, Assignment, Dissertation/Thesis/Project work, Field Survey, Viva Voce, Semester wise University examinations etc, as per the Teaching & Evaluation Scheme୩୬୨ (୨୭)

14. COURSE OF STUDY:

The course of study for Pharm.D. shall include the subjects as given in the Tables below. The number of hours in a week, devoted to each subject for its teaching in theory, practical and tutorial shall not be less than that noted against it in columns (3), (4) and (5) below.

୩୬୨ (୨୮) SIXTH YEAR:

Internship or residency training including postings in speciality units. Student should independently provide the clinical pharmacy services to the allotted wards.

(i) Six months in General Medicine department, and

(ii) Two months each in three other speciality departments

Note: The Couse of Studies shall be revised as per PCI Guidelines issued from time to time.

15. EXAMINATION:

1) Every year there shall be an examination to examine the students.

2) Each examination may be held twice every year. The first examination in a year shall be the annual examination and the second examination shall be supplementary examination.

3) The examinations shall be of written and practical (including oral nature) carrying maximum marks for each part of a subject as indicated in Tables below :

୩୬୨ (୩୦) Note: The scheme of Examination shall be revised as per PCI Guidelines issued from time to time.

16. ELIGIBILITY FOR APPEARING EXAMINATION:

(i) Only such students who produce certificate from the Head of the Institution in which he or she has undergone the Pharm.D. course, in proof of his or her having regularly and satisfactorily undergone the course of study by attending not less than 80% of the classes held both in theory and in practical separately in each subject shall be eligible for appearing at examination.

(ii) A relaxation to the maximum extent of 15% of the total attendance can be accorded to student by the Vice Chancellor on account of sickness, attendance at N.C.C/N.S.S camp and parades, participation as a member of the University team in any inter or intra university competition. Participation in the University functions and the prescribed educational tours/field trips/ field work, provided that the attendance record, duly counter signed by the teacher-in-charge, is sent to the head of the Institute concerned within two weeks of the function/activity etc.

(iii) Provided further in case of sickness/medical disability, an application for the condonation shall be supported by a medical certificate issued by a registered medical practitioner/public hospital and duly authenticated by either the Chief Medical Officer (Civil Surgeon) or the University Health center or Official Doctor of Kalinga University/Institute/ Department. Such applications must be submitted either during the period of treatment/hospitalization or within two weeks following recovery.

17. MODE OF EXAMINATIONS:

(1) Theory examination shall be of three hours and practical examination shall be of four hours duration.

(2) A Student who fails in theory or practical examination of a subject shall re-appear both in theory and practical of the same subject.

(3) Practical examination shall also consist of a viva –voce (Oral) examination.

(4) Clerkship examination – Oral examination shall be conducted after the completion of clerkship of students. An external and an internal examiner will evaluate the student.

Students may be asked to present the allotted medical cases followed by discussion.

Students' capabilities in delivering clinical pharmacy services, pharmaceutical care planning and knowledge of therapeutics shall be assessed.

18. AWARD OF DEGREE:

A student who has passed as per the above criteria mentioned in clause 15 to 17 in all the papers and in all the years shall be eligible for the award of "Pharm. D." Degree.

୩୬୨ (୩୨) AMENDMENT IN ORDINANCE OF KALINGA UNIVERSITY Re-amendment in Ordinance No. 114 Bachelor of Vocational Studies (B.Voc.)

The first approval of Ordinance Number 114 was done on 06th April 2022. The said ordinance was later repealed and amended again and was published on 11th July 2023. Now again submitting the application for amendment in the said ordinance with new branches for publication in Chhattisgarh State Gazette.

The basic unit shall be of 60 seats. Multiple units can also be set up with the approval of CGPURC.

7. ADMISSION :

(i) Students, obtaining minimum of 40% mark in aggregate in each course that includes the marks of end semester examination and the marks of teacher's continuous evaluation along with 40% marks separately in each course in end semester examination both in internal & external exam separately, and 45% marks in aggregate of all the courses shall be essential for passing the program and earning it's assigned credits. A candidate who secures less than 40% of aggregate marks in a course and less than 45% of marks in grand total in a year shall be deemed to have failed in that course.

(ii) A student may apply, within two weeks from the date of the declaration of the result, for revaluation of the examination script(s) of a specific course(s) on the payment of prescribed fees. Revaluation shall mean verifying whether all the question and their parts have been duly marked as per the question paper, and the totaling of marks. In the event of a୩୬୨ (୩୫) discrepancy being found the same shall be rectified through appropriate changes in both the results as well as marks-sheet of the concerned semester end examination.

16. AWARD OF DEGREE:

A student who has passed as per the above criteria mentioned in clause 15 in all the courses and in all the semesters shall be eligible for the award of "Bachelor of Vocation (…Specialization Name…)" Degree.
