No. Admn/2/373/2SIT/10345 .— The Governor of Haryana is pleased to constitute State Level Committee (SLC) and District Level Committee(DLC) in the State for monitoring the performance of M/S CSC e-Governance Services India Limited for carrying out first line maintenance (FLM), Operation and Maintenance(O&M) and provision of Wi Fi hotspots in Gram Panchayats(GPS) under phase -1 of Bharat Net Project as per Memorandum of Undertaking (MoU) executed between CSC, e-Governance Service India Limited with Universal Service Obligation Fund (USOF), Department of Telecommunication , Government of India.
2. The Constitution of the State Level Committee (SLC) shall be as under:-

| X | Assistant General Manager (C&C), Haryana State Electronics Development Corporation Limited. | Member |
| XI | State Head ,CSC- e-Governance Services India Limited | Member Secretary |
3. Roles & responsibilities of the Committeeshall be as under:-
• The Committee shall steer and monitor the performance of CSC -SPV in the State for carrying out First line maintenance (FLM), operation & maintenance and provision of Wi-Fi hotspots in their respective Gram Panchayats (GPs) covered under Phase I of BharatNet project as per the MoU signed by CSC- SPV with USOF.
• The Committee shall meet once in a month preferably on the third Monday of every month. In case of holiday, the meeting of the Committee shall be convened on next working day.
• The Committee shall review the progress on the action taken points discussed/decided in the meetings of the District Level Committees on the tasks performed by CSC -SPV under First line maintenance (FLM), operation & maintenance and provision of Wi-Fi hotspots in their respective Gram Panchayats (GPs) covered under Phase I of BharatNet project.
• Member Secretary shall prepare the minutes of meeting and get the same approved from Chairman of the Committee and send Action Taken Report (ATR) to USOF wing.
4. The Constitution of the District Level Committee (DLC) shall be as under:-

5. The roles & responsibilities of the Committeeshall be as under:-
• The Committee shall steer and monitor the performance of CSC SPV in their respective district for carrying out First line maintenance (FLM), operation & maintenance and provision of Wi-Fi hotspots in their respective Gram Panchayats (GPs) covered under Phase I of BharatNet project as per the MoU signed by CSC SPV with USOF.
• The Committee shall meet once in a month preferably on the first Monday of every month. In case of holiday ,the meeting of the Committee shall be convened on next working day.
• The Committee shall review the progress on the action taken points of the previous meetings of the District Level Committees on the tasks performed by CSC SPV under First line maintenance (FLM), operation & maintenance and provision of Wi-Fi hotspots in their respective Gram Panchayats (GPs) covered under Phase I of BharatNet project.
• Member Secretary shall prepare the minutes of meeting and get the same approved from Chairman of the Committee and send Action Taken Report (ATR) to Administrative Secretary, IT.
