I, Ex. NK. Army No. 2874926-F, Discharge Certificate Sr. No. 11532 Dated 29-02-1996/Pension Book, Shiv Karan S/o Sh. Sewa Ram R/o H. No. 364, Sec. 18, Huda, Distt. Panipat, have changed my name in my army record Shiv Karan to Shiv Karan Singh for all future purpose.
