I, Shirien Rastogi daughter of Sh. Atul Rastogi resident of C-40, Fourth Floor, Sector-54, Metro Station, Suncity, Sikanderpur Ghosi, Gurugram, Haryana-122001 have changed my minor daughter's name from Suhanie Mittal to Suhanie Rastogi Mittal for all future purpose.
