Notification
The 13th March, 2024
No. Plg (HPSN)-2024/117.— The Government of Haryana hereby in supersession of notification no. Plg (HPSN)-2023/648, dated: 9th November, 2023 notify the assistance scheme namely Mukhya Mantri Haryana Karamcharis Durghatna Suraksha Yojana (MMHKDSY) for Group C & D Employees (only contractual employees) and Safai Karamcharis (only contractual employees) of Haryana from Government Departments, Public Sector Enterprises, Municipalities and Autonomous Bodies etc. on account of accidental death/ permanent disability (caused by accident) while on duty. The detailed modalities of the scheme are as under:-
Objective of the scheme: To provide socio-financial security to eligible beneficiaries of the State.
General:
(a) 'Beneficiary' Safai Karamcharis (only contractual) of Urban Local Bodies Department and Development & Panchayats Department and Group C & D employees (only contractual) who are in high risk categories namely Lineman, Assistant Lineman of Power Department, Sub-Station attendant and Shift Attendant of Power distribution Companies (DHBVN & UHBVN), Grid Sub Station Operators of Power generation companies of Power Department (HPGCL & HPVNL), Fire Driver and Fireman of Fire Department, Sewer Man and Sewer Helpers of Public Health Department.
(b) 'Stakeholder' Departments/Organization whose employees are covered:-
1. Group C & D (involved in High Risk job)
➢ Power – Lineman and Assistant Lineman
• UHBVN
• DHBVN
• HPGCL
• HVPNL
➢ Power – Sub-Station attendant and Shift Attendant of Power distribution Companies (DHBVN & UHBVN)
➢ Power - Grid Sub Station Operators of Power generation companies of Power Department (HPGCL & HPVNL)
➢ Public Health - Sewer Man & Sewer helper
➢ Fire Department - Fire Driver & Fireman

(c) 'Claimant' means the means the beneficiary (in case of permanent disability) or eligible relative (in case of death) on behalf of the beneficiary applying to claim the assistance amount under MMHKDSY.
(d) 'MMHKDSY' means Mukhya Mantri Haryana Karamcharis Durghatna Suraksha Yojana.
(e) 'Accidental Death' Accidental death as an event that strictly occurs as a result of an accident.
(f) 'Permanent Disability' means 70% or above permanent disability on account of any accident as certified by the Medical Authority.
(g) 'State' means the State of Haryana.
(h) 'Trust/HPSN' means Haryana Parivar Suraksha Nyas.
Eligibility:
(a) Resident of Haryana having Family ID.
(b) The benefits of the scheme will be made available to Safai Karamcharis (only contractual) of Urban Local Bodies Department and Development & Panchayats Department and Group C & D employees (only contractual) of Power Department (Lineman and Assistant Lineman), Sub Station Attendant and Shift Attendant of State distribution companies of Power Department (DHBVN & UHBVN) and Grid Sub Station Operators of generation companies of Power Department (HPGCL & HPVNL), Public Health (Sewer Man & Sewer helper) and Fire Department (Fire Driver & Fireman) who are involved in high - risk categories.
(c) The scheme will come into force from the date of notification in the official gazette.
(d) Under this scheme, assistance will be given only for accidental death/ permanent disability (caused by accident) while on duty.
Amount of Assistance:
In case of accidental death/ permanent disability (caused by accident), the beneficiaries will be given a assistance of Rs.10 lakh in case of Group C and D employees and Rs. 5 lakh for Safai Karamcharis.
Role of Stakeholders:
1. The stakeholders shall provide list of employees half yearly (in January & July) along with their designations to the Trust.
2. Stakeholders shall certify/send the case to the Trust along with their recommendations and required documents.
3. If the beneficiary receives any assistance/benefits from the concerned department, then that amount will be deducted from the assistance amount of MMHKDSY.
Claim Procedure:
a. The concerned department/organization must inform the Trust about the employee's accidental death/ permanent disability within thirty days of the death.
b. That the Concerned department/organization shall provide the following documents in respect of every claim submitted to the Trust.
In case of Accidental death
Claim Form must be signed by the claimant
Family ID
Police Intimation/First Information Report (FIR)/DDR.
Postmortem Report
Death Certificate
In case of permanent disability
Claim Form must be signed by the claimant
Family ID

HARYANA GOVT. GAZ. (EXTRA.), MAR. 13, 2024 (PHGN. 23, 1945 SAKA) 1065 Permanent disability certificate from Medical Authority as notified by Govt. of India vide S.O. 76(E) dated 04.01.2018 issued after the scheme came into force showing permanent disability 70% or above, on account of any accident.
Hospital discharge summary (only if hospitalised)
Payment Procedure:
1. The claim will be settled by the Trust after the submission of all the documents as provided under the procedure for claim.
2. To establish the genuineness of the claim and identity of the claimant(s), the deciding authority may seek any further documents from the claimant(s), as he deems necessary, for settlement of claim.
3. All the assistance payments will be made in the bank account registered in the PPP database or through Aadhaar based seeded bank accounts. The data of bank account/Aadhaar number will also be taken from Family Information Data Repository (FIDR) database and further the bank will transfer the same to the bank account/Aadhaar seeded account.
4. In case of death, the assistance amount shall be paid
A. In case Member of family is deceased then assistance will be given
➢ Head of the Family (HOF) below the age of 60 years.
➢ In case HOF is above the age of 60 years then eldest of the family member below the age of 60 years.
➢ In case no elder family member/HOF is below the age of 60 years then the family member immediate above the age of 60 years.
B. In case Head of Family is deceased then assistance will be given
➢ Eldest of the family below the age of 60 years.
➢ In case no elder family member is below the age of 60 years then the family member immediate above the age of 60 years.
C. In case no family member is alive after the death in the family then no assistance will be paid.
D. In case the alive family member are below the age of 18 years then the assistance will be paid only at the time of any member attaining the age above 18 years.
5. In case of permanent disability, the assistance amount will be paid to the beneficiary in his bank account registered in the PPP database or bank account linked to the Aadhaar number.
6. The claimant will be required to sign an undertaking form stating that he/she has not received any assistance/direct benefit from any scheme run by the State Government.
7. If the beneficiary receives any assistance/direct benefit from the concerned department, then that amount will be deducted from the assistance amount of MMHKDSY.
8. In case assistance is sanctioned by suppressing true information or making wrong claim or on the basis of wrong information given by the claimant in the claim form then the assistance amount would be recovered alongwith interest @ 12% per annum.
Implementing Agency:
The implementing agency for the MMHKDSY scheme will be Haryana Parivar Suraksha Nyas (HPSN), Government of Haryana. The Chief Executive Officer, Haryana Parivar Suraksha Nyas will be the overall in charge of the scheme and necessary instructions in respect to its proper enforcement and accounting procedure will be issued by HPSN from time to time.
The detailed Standard Operating Procedures (SOPs) for implementation of the scheme are being issued separately.
