उपरोक्त प्रार्थना-पत्र श्री सुभाष चन्द पुत्र श्री जगमाल सिंह, निवासी हाऊस नं0 235/3 कैंट एरिया नाहन, तहसील नाहन, जिला सिरमौर, हिमाचल प्रदेश ने अधीन धारा 13 (3) जन्म एवं मृत्यु पंजीकरण अधिनियम, 1969 के अन्तर्गत प्रस्तुत करके प्रार्थना की है कि उनके पुत्र पवन कुमार की जन्म तिथि 22-5-1993 है, का नाम नगर पालिका नाहन के रिकार्ड में दर्ज नहीं करवाया गया है। जिसे प्रार्थी अब दर्ज करवाना चाहता है।
