विषय.—प्रार्थना-पत्र जेर धारा 13(3) जन्म एवं मृत्यु पंजीकरण अधिनियम, 1969.
उपरोक्त प्रार्थी ने अधो-हस्ताक्षरी की अदालत में प्रार्थना-पत्र, ब्यान-हल्फी वमय अन्य कागजात इस आश्य से गुजारा है कि उसकी जन्म तिथि 28-05-1989 है, जोकि ग्राम पंचायत बगढार के रिकॉर्ड में दर्ज न है, जिसे दर्ज किया जावे ।
इस सम्बन्ध में सर्व-साधारण जनता को बजरिया इश्तहार सूचित किया जाता है कि प्रार्थी शाम लाल की जन्म तिथि, ग्राम पंचायत बगढार के रिकॉर्ड में दर्ज करने पर, यदि किसी को कोई उजर-एतराज हो तो वह असालतन या वकालतन अदालत अधो-हस्ताक्षरी दिनांक 18-03-2013 को हाजिर आकर अपना एतराज दर्ज करवा सकता है। हाजिर न आने कि सूरत में एकतरफा कार्यवाही अमल में लाई जा करके नाम व जन्म तिथि दर्ज करने के आदेश दे दिए जाएंगे ।

उपरोक्त प्रार्थी ने अधो-हस्ताक्षरी की अदालत में प्रार्थना-पत्र, ब्यान-हल्फी वमय अन्य कागजात इस आश्य से गुजारा है कि उसके पिता का सही नाम किशन चन्द उर्फ किशनू है, जोकि ग्राम पंचायत मेल के रिकॉर्ड में सही दर्ज है। लेकिन राजस्व विभाग के मुहाल मेल, पटवार वृत्त मेल में गलती से किशन चन्द दर्ज है, जिसकी दरुस्ती की जावे।

अभिलेख मुहाल जोल में दरुस्त करने हेतु आवेदन-पत्र गुजार रखा है कि प्रार्थी का वास्तविक नाम प्रताप चन्द है परन्तु राजस्व अभिलेख मुहाल जोल में प्रतापा गलत दर्ज हुआ है। प्रार्थी अपने नाम की राजस्व अभिलेख मुहाल जोल में दरुस्ती करवाना चाहता है। जिसे राजस्व अभिलेख में दरुस्त करने के आदेश दिए जावे।
अतः आम जनता को इस इश्तहार द्वारा सूचित किया जाता है कि यदि किसी व्यक्ति को प्रार्थी के नाम की दरुस्ती का राजस्व अभिलेख मुहाल जोल में प्रतापा के स्थान पर प्रताप चन्द दर्ज करने बारे किसी प्रकार का उजर व एतराज हो तो वह असालतन या वकालतन दिनांक 8-3-2013 को अपना उजर व एतराज न्यायालय में पेश कर सकते हैं। गैर-हाजरी की सूरत में एकतरफा कार्यवाही अमल में लाई जाएगी। आज दिनांक 12-2-2013 को मेरे हस्ताक्षर व मोहर अदालत से जारी किया गया।
