संख्याः ई.एक्स.एन-एफ(18)-2/1999-I.—हिमाचल प्रदेश की राज्यपाल का समाधान हो गया है कि हिमाचल प्रदेश के राज्यक्षेत्र में प्रवेश करने वाले अन्य राज्यों के ट्रकों को राज्य के बाहर सेब और आलू के परिवहन के लिए, हिमाचल प्रदेश में यात्रियों तथा सामान पर कर लगाने का अधिनियम, 1955 की धारा 3 के अधीन उद्‌ग्रहणीय माल कर (गुडज टैक्स) के संदाय से छूट देना लोक हित में समीचीन है ।

93-राजपत्र/2014-7-8-2014 (2493) 2494 राजपत्र, हिमाचल प्रदेश, 7 अगस्त, 2014/16 श्रावण, 1936 अतः हिमाचल प्रदेश की राज्यपाल, पूर्वोक्त अधिनियम की धारा 10 द्वारा प्रदत्त शक्तियों का प्रयोग करते हुए, हिमाचल प्रदेश राज्य क्षेत्र में प्रवेश करने वाले अन्य राज्यों के उपरोक्त प्रयोजनार्थ ट्रकों को, तारीख 15-07-2014 से 15-10-2014 तक, उक्त अधिनियम की धारा 3 के अधीन उद्‌ग्रहणीय माल कर (गुडज टैैक्स) के संदाय से छूट प्रदान करने के आदेश देती है ।

Shimla-2, the August, 2014

No. EXN-F (18)-2/1999-I.—Whereas, the Governor of Himachal Pradesh is satisfied that it is expedient in the public interest to exempt the trucks of the Other States entering in the territory of Himachal Pradesh for transportation of apples and potatoes from out side the State, from the payment of goods tax leviable under section 3 of the Himachal Pradesh Passengers and Goods Taxation Act, 1955.

Now, therefore, in exercise of the powers conferred by section 10 of the Act ibid the Governor, Himachal Pradesh is pleased to order the exemption from the payment of goods tax leviable under section 3 of the said Act, the trucks for above purpose, of the other States entering in the territory of Himachal Pradesh w.e.f. 15-07-2014 to 15-10-2014.

No.EDN-A-Kha(2)23/2013.—In pursuance of the decision arrived in CWP No.2336 of 2009-E with CWP NO.5047 of 2010 & CWP No.8709 of 2011 decided on 11th April 2013 and in consultation with the Law Department, the Governor, Himachal Pradesh is pleased to order to regularise the services of the following Assistant Professors (College Cadre) in the pay scale of Rs.15,600 – 39,100+6,000 (GP) per month with effect from 1st June 2014 in the subject shown against their names:—

Terms and conditions:—

(i) That the above regularization will be treated as fresh direct appointment and will be effective from the date of joining on regular appointment.

(ii) That these appointees will be placed at the initial of the pay scale prescribed for newly appointed Assistant Professors (College Cadre) i.e. Rs.15,600 – 39,100+6,000 (GP), subject to the conditions that they are qualified as per R & P Rules in vogue, on the date of their initial appointment.

(iii) The candidates who are not eligible as per the R & P Rules in vogue at the time of their appointment, they shall have to pass the NET / SLET examination till 30th June 2016 otherwise their services shall be terminated. Till the completion of their requisite educational qualification they are not entitled for any annual increment(s).

(iv) That the seniority of these Assistant Professors will be fixed at the tail of seniority list i.e. below those Assistant Professors who have been last appointed on regular basis through HPPSC of the calendar year.

(v) The services of these Assistant Professors shall be subject to the condition that the reservation in service for Scheduled Castes / Scheduled tribes / OBC / other categories shall be treated as per the category from which the candidate belongs and the Assistant Professor(s) in this regard shall submit a certificate of SC/ST/OBC/other categories etc. at the time of submitting their acceptance report so that the points as per reservation roster be filled up through the direct recruitment.

(vi) That these Assistant Professors will not claim the benefit of pay fixation, seniority etc. of previous service rendered on contract basis on account of their regularization.

(vii) That the regularization of these Assistant Professors are subject to the final outcome of the CWP (T) No.6330 / 2008 pending in the Hon'ble High Court.

(viii) That the appointment is subject to the verification of their character antecedents, which will be verified later on. For this purpose, they will have to furnish the Attestation Forms to the Government through the Director of Higher Education through proper channel. If on verification, his / her character antecedents are not found satisfactory, the regularization shall be cancelled.

(ix) That they will remain on probation for a period of two years and during the period of probation his / her service can be terminated at any time by giving one month notice of pay in lieu thereof by the appointing authority without assigning any reason.

(x) That the appointment shall carry with it the liability to serve in any part of India / Himachal Pradesh.

(xi) That they will have to produce a Medical fitness certificate from the State Medical Board.

(xii) That they will have to give a declaration for the effect that he / she has only one living spouse.

(xiii) That they will have to take an oath of allegiance / faithfulness to the Constitution of India or making a solemn affirmation.

(xiv) That they will have to produce all the certificates in original at the time of joining this appointment.

(xv) That all the candidates shall have to pass the Department Examination as prescribed in the Himachal Pradesh Departmental Examination Rules 1997, as amended from time to time.

If the above terms and conditions are acceptable to him/her, he/she should submit his/her acceptance report within a week's time from the issue to this notification to the Government through the Principal Government College mentioned against his/her name, failing which this offer of appointment/regularisation shall stand cancelled and no further extension will be accorded. No further correspondence shall be entertained in this behalf.

संख्याः ई.डी.एन.-ए.-ख(3)5/1999-लूज-III.—हिमाचल प्रदेश कीे राज्यपाल, भारत के संविधान केे अनुच्छेद 309 के परन्तुक द्वारा प्रदत्त शक्तियों का प्रयोग करते हुए, हिमाचल प्रदेश लोक सेवा आयोग के परामर्श से, अधिसूचना संख्याः ई.डी.एन.-ए.-ख(3)5/99-लूज, तारीख 29-10-2011 द्वारा अधिसूचित, हिमाचल प्रदेश उच्चतर शिक्षा विभाग, सहायक आचार्य (महाविद्यालय संवर्ग), वर्ग-I (राजपत्रित) और संविदा भर्ती और प्रोन्नति नियम, 2010 का और संशोधन करने के लिए निम्नलिखित नियम बनाती हैंें, अर्थात्‌ः- 1. संक्षिप्त नाम और प्रारम्भ.--(i) इन नियमों का संक्षिप्त नाम हिमाचल प्रदेश उच्चतर शिक्षा विभाग, सहायक आचार्य (महाविद्यालय संवर्ग), वर्ग-I (राजपत्रित) और संविदा भर्ती और प्रोन्नति (संशोधन) नियम, 2014 है ।

(ii) ये नियम राजपत्र, हिमाचल प्रदेश में प्रकाशित किए जाने की तारीख से प्रवृत्त हाेंगे । 2. उपाबन्ध ''क'' का संशोधन.--हिमाचल प्रदेश उच्चतर शिक्षा विभाग, सहायक आचार्य (महाविद्यालय संवर्ग), वर्ग-1 (राजपत्रित) और संविदा भर्ती और प्रोन्नति नियम, 2010 के उपाबन्ध ''क'' मेंः- (अ) क्रम संख्या 7 के सामने विद्यमान उपबन्ध के स्थान पर निम्नलिखित रखा जाएगा, अर्थात्‌ः-2501 राजपत्र, हिमाचल प्रदेश, 7 अगस्त, 2014/16 श्रावण, 1936 ''क) अनिवार्य अर्हताएं :

(i) कम से कम पचपन प्रतिशत अंकों के साथ अच्छा शैक्षणिक रिकॉर्ड या पचपन प्रतिशत के समतुल्य, जहॉं भी भारत सरकार द्धारा मान्यता प्राप्त किसी भारतीय विश्वविद्यालय या किसी विदेशी विश्वविद्यालय से सुसंगत विषय में स्नातकोत्तर की उपाधि स्तर में श्रेणीकरण प्रणाली का अनुसरण किया है ।

(ii) ललित कला में सहायक आचार्य अर्थात वाणिज्यिक कला, रेखाचित्र और रंगचित्र एवं मूर्तिकला के लिए भी, ललित कला में स्नातकोत्तर उपाधि में कम से कम पचपन प्रतिशत अंकों सहित अच्छा शैक्षणिक रिकार्ड, या पचपन प्रतिशत के समतुल्य, जहां श्रेंणीकरण प्रणाली का अनुसरण किया जाता है ।

(iii) अनुसूचित जाति/अनुसूचित जनजाति प्रवर्गों के लिए स्नातकोत्तर उपाधि में पांच प्रतिशत की छूट, पचपन प्रतिशत अंकों से पचास प्रतिशत अंकों तक उपबन्धित है। और

शारीरिक और दृष्टि विकलांग व्यक्तियों को, स्नातकोत्तर स्तर पर, सहासक आचार्य के रूप में नियुक्ति हेतु पांच प्रतिशत की छूट, (पचपन प्रतिशत से पचास प्रतिशत अंको तक) ।

(iv) उन पी0एच0डी0 उपाधि धारकों को, जिन्होने अपनी स्नातकोत्तर उपाधि सितम्बर, 1991 से पूर्व उत्तीर्ण की है पचपन प्रतिशत से पचास प्रतिशत अंको तक उपबन्धित है, पांच प्रतिशत अंको की छूट ।

(v) जहां कहीं भी श्रेणीकरण प्रणाली का अनुसरण किया जाता है, शब्द ग्रेड ओ0 ए0 बी0 सी0 डी0 ई0 और एफ0 के साथ सात बिन्दु स्केल में "बी" को पचपन प्रतिशत अंकों के समतुल्य माना जाएगा (सात बिन्दु स्केल फुटनोट में दर्शाया गया है)। (vi) उपरोक्त अर्हताओं को पूर्ण करने के अतिरिक्त अभ्यर्थी ने, सहायक आचार्य के लिए, यू0 जी0 सी0, सी0 एस0 आई0 आर0 द्वारा संचालित पात्रता टेस्ट या राज्य लोक सेवा आयोग द्वारा प्रत्यायित समरूप टेस्ट पास किया हो ।

(vii) विश्वविद्यालयों/महाविद्यालयों/संस्थानों में सहायक आचार्य की भर्ती और नियुक्ति के लिए एनईटी/एसएलईटी कम से कम/न्यूनतम पात्रता शर्त, रहेगी । परन्तु, तथापि अभ्यर्थी जो पीएचडी उपाधिधारक हैं या जिन्हे ''विश्वविद्यालय अनुदान आयोग (पीएचडी उपाधि को प्रदान करने हेतु-न्यूनतम मानक और प्रक्रिया) विनियम, 2009 के अनुपालन में, पीएचडी की उपाधि प्रदान की गई है, को विश्वविद्यालयों/ महाविद्यालयों/संस्थानों में सहायक आचार्य या समतुत्य पदों पर नियुक्ति के लिए एनईटी/एसएलईटी की न्यूनतम पात्रता शर्त की अनिवार्यता से छूट दी जाएगी । सहायक आचार्य (शारीरिक शिक्षा) (महाविद्यालय संवर्ग) के पद हेतू

(i) अच्छे शैक्षणिक/शारीरिक रिकार्ड के सहित कम से कम पचपन प्रतिशत अंकों या बिन्दु स्केल में समतुल्य ग्रेड जहॉं श्रेणीकरण प्रणाली का अनुसरण किया जाता है के साथ स्नातकोत्तर उपाधि, या खेल विज्ञान में में स्नात्कोत्तर उपाधि ।2502 राजपत्र, हिमाचल प्रदेश, 7 अगस्त, 2014/16 श्रावण, 1936 (ii) अन्तर्विश्वविद्यालय/अन्तर्महाविद्यालय प्रतियोगिताओं या राज्य और/या राष्ट्रीय खेल स्पर्धाओं में विश्वविद्यालय/महाविद्यालय का प्रतिनिधित्व करने का रिकॉर्ड ।

(iii) विश्वविद्यालय अनुदान आयोग द्वारा या किसी अन्य अभिकरण द्वारा जो विश्वविद्यालय अनुदान आयोग द्वारा अनुमोदित हो, प्रयोजन के लिए संचालित राष्ट्रीय स्तर की परीक्षा अर्हित की हो । (iv) तथापि, अभ्यर्थी जो पी0एच0डी0 उपाधिधारक है। या जिन्हें ''विश्वविद्यालय अनुदान आयोग (पीएचडी उपाधि को प्रदान करने हेतु-न्यूतम मानक और प्रक्रिया) विनियम, 2009 के अनुपालन में, पीएचडी की उपाधि प्रदान की गई है को, सहायक आचार्य शारीरिक शिक्षा की भर्ती और नियुक्ति के लिए एनईटी/एसएलईटी/एसईटी की न्यूनतम पात्रता शर्त की अनिवार्यता से छूट दी जाएगी ।

(v) निम्न उपबंधों के अनुसार संचालित शारीरिक समर्थता टेस्ट उतीर्ण किया हो :-

शारीरिक समर्थता टेस्ट सन्नियमः-

(क) इन विनियमों के उपबंधों के अध्यधीन, सभी अभ्यर्थियों को, जिन्हे शारीरिक समर्थता टेस्ट देना अपेक्षित है, यह प्रमाणित करते हुए चिकित्सा प्रमाण पत्र प्रस्तुत करना होगा कि वह ऐसे टेस्ट को देने से पूर्व चिकित्सीय रूप से समर्थ/स्वस्थ हैं ।

(ख) उपरोक्त उप-खण्ड (क) में वर्णित ऐसे चिकित्सा प्रमाण पत्र को प्रस्तुत करने पर अभ्यर्थी को निम्नलिखित सन्नियमों के अनुसार शारीरिक समर्थता अपेक्षित होगीः-

पुरूषों के लिये सन्नियम

महिलाओं के लिय सन्नियम

(ख) वांछनीय अर्हताएं.--हिमाचल प्रदेश की रूढ़ियों, रीतियों और बोलियों का ज्ञान और प्रदेश में विद्यमान विशिष्ट दशाओं में नियुक्ति के लिए उपयुक्तता ।''

(ब) स्तम्भ संख्या '15-क (VII) (ग) के विद्यमान उपबन्ध के स्थान पर निम्नलिखित रखा जाएगा, अर्थात्‌ः-

''संविदा पर नियुक्त व्यक्ति, एक मास की सेवा पूरी करने के पश्चात्‌ एक दिन के आकस्मिक अवकाश का हकदार होगा। तथापि, संविदा पर नियुक्त कर्मचारी 12 सप्ताह के प्रसूति अवकाश तथा 10 दिन के चिकित्सा अवकाश का हकदार होगा/होगी । वह चिकित्सा प्रतिपूर्ति और2503 राजपत्र, हिमाचल प्रदेश, 7 अगस्त, 2014/16 श्रावण, 1936 एल0टी0सी0 इत्यादि के लिए भी हकदार नहीं होगा/होगी। संविदा पर नियुक्त व्यक्ति को उपरोक्त के सिवाय किसी भी प्रकार का अन्य कोई अवकाश अनुज्ञात नहीं होगा।

परन्तु अनुपयुक्त आकस्मिक अवकाश और चिकित्सा अवकाश केवल एक कलैण्डर वर्ष तक संचित किया जा सकेगा और उसे अगले कलैण्डर वर्ष के लिए अग्रनित नहीं किया जाएगा ।'' (ख) उपाबन्ध ''ख'' का संशोधन.--उपाबंध ''ख'' के खण्ड 4 के सामने विद्यमान उपबन्ध के स्थान पर निम्नलिखित रखा जाएगा, अर्थात्‌ः-

''संविदा पर नियुक्त सहायक आचार्य (महाविद्यालय संवर्ग), एक मास की सेवा पूरी करने के पश्चात्‌ एक दिन के आकस्मिक अवकाश का हकदार होगा। संविदा पर नियुक्त कर्मचारी 12 सप्ताह के प्रसूति अवकाश तथा 10 दिन के चिकित्सा अवकाश का हकदार होगा / होगी । वह चिकित्सा प्रतिपूर्ति और एल0टी0सी0 इत्यादि के लिए भी हकदार नहीं होगा/होगी। संविदा पर नियुक्त व्यक्ति को उपरोक्त के सिवाय किसी भी प्रकार का अन्य कोई अवकाश अनुज्ञात नहीं होगा।

परन्तु अनुपयुक्त आकस्मिक अवकाश और चिकित्सा अवकाश केवल एक कलैण्डर वर्ष तक संचित किया जा सकेगा और उसे अगले कलैण्डर वर्ष के लिए अग्रनित नहीं किया जाएगा ।''

"HIGHER EDUCATION DEPARTMENT"

NOTIFICATION

Shimla-171002, 24th July, 2014

No. EDN-A-Kha(3)5/1999-L-III.—In exercise of the powers conferred by proviso to Article 309 of the Constitution of India, the Governor, Himachal Pradesh, in consultation with the H.P. Public Service Commission, is pleased to make the following Rules, further to amend the Himachal Pradesh, Higher Education Department, Assistant Professors (College Cadre) Class-I (Gazetted) and Contract Recruitment and Promotion Rules 2010, notified vide Notification No.EDN-A(Kha) 3-5/99-Loose dated 29th Oct. 2011, namely :—

1. Short Title and Commencement.—(i) These rules may be called the Himachal Pradesh Higher Education Department, Assistant Professor (College Cadre) Class-I (Gazetted) and Contract Recruitment and Promotion (First Amendment) Rules, 2014.

(ii) These rules shall come into force from the date of publication in the Rajpatra, Himachal Pradesh.

2. Amendment of Annexure "A".—In Annexure "A" to the Himachal Pradesh, Higher Education Department, Assistant Professors (College Cadre) Class-I (Gazetted) and Contract Recruitment and Promotion Rules, 2010 :—

(A) for the existing provisions against Col. No.7, the following shall be substituted, namely:—

"a) Essential Qualifications:

(i) A good academic record with atleast 55% marks or an equivalent of 55%, wherever the grading system is followed in Masters Degree level in the relevant subject from a Indian University or an equivalent degree from Foreign University recognized by the Government of India.

(ii) For the Assistant Professor in Fine Arts also i.e. Commercial Arts, Drawing & Painting and Sculpture, Master Degree in Fine Arts having good academic record with 55% marks or equivalent of 55% wherever grading system is followed.

(iii) A relaxation of 5% is provided from 55% to 50% marks, at the Masters Degree for the SC/ST categories.

AND

A relaxation of 5% (from 55% to 50%) of marks, at the Master's level to the Physically and Visually Handicapped persons in appointment as Assistant Professors.

(iv) A relaxation of 5% is provided from 55% to 50% of the marks to the Ph.D. Degree holders who have passed their masters degree prior to the September, 1991.

(v) "B" in the 7 point scale with letter grades O, A, B, C, D, E & F shall be regarded as equivalent of 55% wherever the grading system is followed (7 point scale is displayed in the Foot Note)

(vi) Candidate besides fulfilling the above qualifications should have cleared the eligibility test (NET) for Assistant Professor conducted by the UGC, CSIR or similar test accredited the State Public Service Commission.

(vii) "NET/SLET shall remain the minimum eligibility condition for recruitment and appointment of Assistant Professor in Universities / Colleges / Institutions. Provided, however, that candidate, who are or have been awarded Ph.D degree in compliance of the "University Grant Commission (minimum standards and procedure for award of Ph.D Degree), regulation 2009, shall be exempted from the requirement of minimum eligibility condition of NET/SLET for appointment of Assistant Professor or equivalent positions in University / Colleges / Institutions."

For the post of Assistant Professor of Physical Education (College Cadre):—

(i) A Master's Degree in Physical Education or Master's Degree in Sports Science with at least 55% marks or an equivalent grade in a point scale wherever grading system is followed with a consistently good academic record.

(ii) Record of having represented the university / college at the inter-university /inter college competitions or the State and / or national championships.

(iii) Qualifying in the national level test conducted for the purpose by the UGC or any other agency approved by the UGC.

(iv) However, candidates, who are, or have been awarded Ph.D degree in accordance with the "University Grants Commission (Minimum Standards and Procedure for Award of Ph.D Degree), Regulations 2009, shall be exempted from the requirement of the minimum eligibility condition of NET / SLET /SET for recruitment and appointment of Assistant Professor Physical Education.

(v) Passed the Physical Fitness Test conducted in accordance with the provisions as under:-

PHYSICAL FITNESS TEST NORMS

(a) Subject to the provisions of these Regulations, all candidates who are required to undertake the physical fitness test shall be required to produce a medical certificate certifying that he / she is medically fit before undertaking such tests.

(b) On production of such certificate mentioned in sub-clause(a) above, the candidate would be required to undertake the physical fitness in accordance with the following norms:—

(b) Desirable Qualification (s).—Knowledge of customs, manners and dialects of Himachal Pradesh and suitability for appointment in peculiar conditions prevailing in the Pradesh.

(B) for the existing provisions against Col. No. 15-A(VII)(c), the following shall be substituted, namely:—

"(c) Contract Appointee will be entitled for one day's casual leave after putting one month service. However, the contract employee will also be entitled for 12 weeks Maternity Leave and 10 day's Medical Leave. He/She shall not be entitled for Medical Re-imbursement and LTC etc. No leave of any other kind except above is admissible to the contract appointee.

Provided that the un-availed Casual Leave and Medical Leave can be accumulated upto the Calendar Year and will not be carried forwarded for the next Calendar Year."

(3) Amendment of Annexure "B".—for the existing provisions against Clause 4 of Annexure "B", the following shall be substituted, namely:—

"Contractual Appointee Assistant Professor (Colleges) will be entitled for one day's casual leave after putting in one month service. No. EDN-A-Kha(2)23/2013.—In continuation to this department notification of even no. dated 14th July 2014, the Governor, Himachal Pradesh is pleased to order to regularise the services of the following Assistant Professors (College Cadre) in the pay scale of Rs.15,600 – 39,100+6,000 (GP) per month with effect from 1st June 2014 in the subject shown against their names:—

Terms and Conditions :

I. That the above regularization will be treated as fresh direct appointment and will be effective from the date of joining on regular appointment.

II. That these appointees will be placed at the initial of the pay scale prescribed for newly appointed Assistant Professors (College Cadre) i.e. Rs.15,600 – 39,100+6,000 (GP), subject to the conditions that they are qualified as per R & P Rules in vogue, on the date of their initial appointment.

III. The candidates who are not eligible as per the R & P Rules in vogue at the time of their appointment, they shall have to pass the NET / SLET examination till 30th June 2016 otherwise their services shall be terminated. Till the completion of their requisite educational qualification they are not entitled for any annual increment(s).

IV. That the seniority of these Assistant Professors will be fixed at the tail of seniority list i.e. below those Assistant Professors who have been last appointed on regular basis through HPPSC of the calendar year.

V. The services of these Assistant Professors shall be subject to the condition that the reservation in service for Scheduled Castes / Scheduled tribes / OBC / other categories shall be treated as per the category from which the candidate belongs and the Assistant Professor(s) in this regard shall submit a certificate of SC/ST/OBC/other categories etc. at the time of submitting their acceptance report so that the points as per reservation roster be filled up through the direct recruitment

VI. That these Assistant Professors will not claim the benefit of pay fixation, seniority etc. of previous service rendered on contract basis on account of their regularization.

VII. That the regularization of these Assistant Professors are subject to the final outcome of the CWP (T) No.6330 / 2008 pending in the Hon'ble High Court.

VIII. That the appointment is subject to the verification of their character antecedents, which will be verified later on. For this purpose, they will have to furnish the Attestation Forms to the Government through the Director of Higher Education through proper channel. If on verification, his / her character antecedents are not found satisfactory, the regularization shall be cancelled.

IX. That they will remain on probation for a period of two years and during the period of probation his / her service can be terminated at any time by giving one month notice of pay in lieu thereof by the appointing authority without assigning any reason.

X. That the appointment shall carry with it the liability to serve in any part of India / Himachal Pradesh.

XI. That they will have to produce a Medical fitness certificate from the State Medical Board.

XII. That they will have to give a declaration for the effect that he / she has only one living spouse.

XIII. That they will have to take an oath of allegiance / faithfulness to the Constitution of India or making a solemn affirmation.

XIV. That they will have to produce all the certificates in original at the time of joining this appointment.

XV. That all the candidates shall have to pass the Department Examination as prescribed in the Himachal Pradesh Departmental Examination Rules 1997, as amended from time to time.

If, the above terms and conditions are acceptable to him / her, he / she should submit his / her acceptance report within a week's time from the issue to this notification to the Government through the Principal Government College, presently posted, failing which this offer of appointment / regularization shall stand cancelled and no further extension will be accorded. No further correspondence shall be entertained in this behalf.

श्री सुरेश चन्द पुत्र श्री अजमेर सिंह, निवासी कौलांवाला भूड, तहसील नाहन, जिला सिरमौर (हि0 प्र0) ने अधीन धारा 13(3) जन्म एवं मृत्यु पंजीकरण अधिनियम, 1969 के अन्तर्गत आवेदन-पत्र प्रस्तुत करके प्रार्थना की है कि उसके पुत्र अजय कुमार जिसकी जन्म तिथि 7-8-1997 है, का नाम ग्राम पंचायत कौलांवाला भूड, तहसील नाहन, जिला सिरमौर के रिकॉर्ड में दर्ज नहीं करवाया गया है। जिसे प्रार्थी अब दर्ज करवाना चाहता है।

अतः सर्वसाधारण को इस इश्तहार द्वारा सूचित किया जाता है कि इस सम्बन्ध में यदि किसी व्यक्ति को उजर या एतराज हो तो वह स्वयं अथवा अपने प्रतिनिधि द्वारा मिति 20-8-2014 को सुबह दस बजे अदालत में उपस्थित आकर प्रस्तुत करे। बसूरत दीगर अजय कुमार का नाम व जन्म तिथि को दर्ज करने के आदेश जारी कर दिये जायेंगे।

I, Santosh Kumar s/o Shri Saran Dass, V.P.O. Ghar Jarot, Tehsil Jawali, District Kangra, Himachal Pradesh declared that I am serving in Indian Army vide Service No. 13755980Y Rank Hav of unit 9 JAK RIF. That the correct name of my son is Sourabh Singh as is mentioned in Gram Panchayat record and my Army Record. That in the school record of my son, the name of my son is wrongly mentioned as Sourav Singh.
