श्रीमती मीना रानी पत्नी स्व0 श्री जगदीश राम, निवासी ग्राम भूपपुर, तहसील पांवटा साहिब, जिला सिरमौर, हिमाचल प्रदेश ने मय शपथ पत्र प्रार्थना-पत्र इस अनुरोध के साथ प्रस्तुुत किया है कि उसका सही नाम मीना रानी है तथा उसकी जन्म तिथि एवं विवाह की तिथि क्रमशः 15-12-1966 एवं 11-12-1986 है लेकिन गलती से पंचायत रिकार्ड पातलिंयो में उसकी जन्म तिथि व विवाह की तिथि 15-3-1966 एवं 9-11-1966 लिखी गई है जोकि गलत है जिसे अब सही करवाना चाहती है।

अतः इस सम्बन्ध में सर्वसाधारण को सूचित किया जाता है कि मीना रानी पत्नी स्व0 श्री जगदीश राम, निवासी ग्राम भूपपुर, तहसील पांवटा साहिब, जिला सिरमौर, हिमाचल प्रदेश के जन्म एवं विवाह की सही तिथि दर्ज करने बारे किसी व्यक्ति को कोई उजर/एतराज हो तो दिनांक 01-01-2017 तक असालतन अथवा वकालतन अधोहस्ताक्षरी की अदालत हजा में उपस्थित होकर प्रस्तुत कर सकता है। निर्धारित तिथि तक कोई एतराज प्राप्त न होने की सूरत में प्रार्थिया की पुत्री के जन्म पंजीकरण करने के नियमानुसार आदेश पारित कर दिये जायेंगे।
